Insulin-sensitizing agents are indicated for most women with PCOS because they have positive effects on insulin resistance, menstrual irregularities, anovulation, hirsutism, and obesity. Of all the drugs used to treat manifestations of PCOS, metformin (Glucophage) has the most data supporting its effectiveness. Table 1 details the most common medications used to treat manifestations of PCOS.6–27
If these standard treatments don’t work, your doctor may recommend in vitro fertilization (IVF). Your egg is fertilized by your partner’s sperm in a lab, then transferred to your uterus to implant and grow. Your doctor may also recommend a surgical procedure called ovarian “drilling”. Lasers, or a very thin, heated needle, are used to pierce several holes in the surface of your ovary, which improves the chances of successful ovulation for about six to eight months. This is specifically effective for women with PCOS since the outer surface of the ovaries can become extra thick, interfering with the natural process of ovulation.
Hyperinsulinemic-euglycemic clamp techniques rely on an intravenous insulin infusion to maintain steady serum glucose concentrations at fasting levels to measure glucose uptake. Lower glucose uptake signifies resistance to insulin action (i.e. IR). Since the technique requires intravenous infusions, frequent blood sampling, extensive time and significant financial resources, it is experimentally useful but clinically cumbersome[45]. Clamp studies in PCOS women show conflicting results; some studies show IR only in obese PCOS women[46] and others demonstrate IR in lean PCOS patients[47]. Of importance, the studies which failed to demonstrate IR in lean PCOS women did, however, demonstrate elevated basal insulin levels compared to weight matched, non PCOS controls[46]. Other sophisticated testing methods using intravenous infusions of insulin have been attempted (insulin sensitivity test and insulin tolerance test) but they do not alleviate the time, financial and testing burdens to make them relevant for widespread clinical practice and normal cutoffs are not widely disseminated[45]. Clamp techniques have been used as comparisons to validate other modes of assessment of IR.
Ask your health care provider about a weight loss plan if you are overweight. If you’re overweight, losing weight may lessen some of the symptoms of PCOS. Talk to your health care provider or nutritionist about healthy ways to lose weight such as exercising more and following a nutrition plan that helps manage insulin levels. Healthy eating can also keep your heart healthy and lower your risk of developing diabetes.
Insulin-sensitizing drugs. A particular medication, called metformin, is usually prescribed.20 Another medication, pioglitazone (Actos) may also be suggested for women with PCOS. These two medications are FDA-approved for diabetes treatment, but plenty of research shows they can be equally effective for women with PCOS, too. These insulin-sensitizing medications can help your body respond more readily to insulin, and better control your glucose levels. Both metformin and pioglitazone can reduce the insulin resistance and high insulin levels that commonly occur with PCOS, and in turn, can reduce high androgen levels.

Treatment should be individualized based on the patient's presentation and desire for pregnancy (Figure 219,29–35). Fasting insulin level or GTT with insulin levels (also called IGTT). Elevated insulin levels have been helpful to predict response to medication and may indicate women needing higher dosages of metformin or the use of a second medication to significantly lower insulin levels. Elevated blood sugar and insulin values do not predict who responds to an insulin-lowering medication, low-glycemic diet, and exercise. Many women with normal levels may benefit from combination therapy. A hypoglycemic response in which the two-hour insulin level is higher and the blood sugar lower than fasting is consistent with insulin resistance. A mathematical derivation known as the HOMAI, calculated from the fasting values in glucose and insulin concentrations, allows a direct and moderately accurate measure of insulin sensitivity (glucose-level x insulin-level/22.5).[citation needed]

A lot of people only know things about post-junkie Luna and think that it was the drugs that turned her into a bad person. Can't really blame them; the vast majority of pre-Luna discussion/posts consist of pictures of her and her friends, her upbringing, and her short-lived college career. How are they going to know that she was a cunt prior to getting hooked?
This is A LOT of benzodiazepines. I remember the last batch she got too, and it wasn't that long ago. She's using more than I am, and I'm using them legally for a partial spinal cord injury. The FUCK, Luna. Being dependant on heroin isn't enough for you or something? This bothers me more than the heroin, honestly. The withdrawal from benzos can actually kill you, unlike heroin. Not to mention that using multiple CNS depressants together is dangerous af. Narcan won't bring you back from that shit.
A pelvic ultrasound (transvaginal and/or pelvic/abdominal) is used to evaluate enlarged ovaries. Ultrasounds are often used to look for cysts in the ovaries and to see if the internal structures appear normal. In PCOS, the ovaries may be 1.5 to 3 times larger than normal and characteristically have more than 12 or more follicles per ovary measuring 2 to 9 mm in diameter. Often the cysts are lined up on the surface the ovaries, forming the appearance of a "pearl necklace." The follicles tend to be small and immature, thus never reaching full development. The ultrasound helps visualize these changes in more than 90% of women with PCOS, but they are also found in up to 25% of women without PCOS symptoms. One strategy that may help some of you: Have a big breakfast. When a group of normal weight women with PCOS got a whopping half of their daily calories first thing in the morning, insulin levels dropped 8% and testosterone levels fell 50% in three months.13 And that’s not all. These women ovulated more frequently and had improved insulin sensitivity in comparison to another group of women in the study who more of their calories at dinner.13
The prevalence of insulin resistance in women with PCOS, as measured by impaired glucose tolerance, is substantially higher than expected compared with age-and weight-matched populations of women without PCOS.45 Although insulin resistance alone is a laboratory (not clinical) aberration, it can lead to diabetes, and it may be associated with the metabolic syndrome, thus leading to increased cardiovascular risk.2 As with diabetes, optimal treatment of PCOS requires lifestyle modifications (e.g., diet, exercise) in addition to appropriate medications.

This is an important distinction; given her longtime love for drugs, tendency to abuse everything she gets, and unwillingness to go sober even when she's not on H, there's a good chance that Luna has 'polysubstance dependence' and is chasing the feeling of being 'not sober' by abusing her meds. The drugs she abuses don't have to get her high if that is the case, just do enough to make her feel a change in her consciousness (e.g. sedation, calm). It's common in people with untreated mental illness. Luna probably hates herself so much that she just can't handle being left alone with her sober self.


Teens with PCOS may have slightly different signs and symptoms. Irregular cycles are common in normal girls in the first years after periods start (menarche). Therefore, signs of ovulation problems for girls include irregular periods more than 2 years after menarche, absence of cycles for more than 3 months, or never having a first period by the time puberty is completed. Also, heavy or overly frequent periods may be a sign of problems related to PCOS. Adolescents do not require an ultrasound for the evaluation of PCOS, since large ovaries with many follicles develop as part of normal puberty. 
Insulin resistance is a condition in which the body’s cells do not respond to the effects of insulin. When the body does not respond to insulin, the level of glucose in the blood increases. This may cause more insulin to be produced as the body tries to move glucose into cells. Insulin resistance can lead to diabetes mellitus. It also is associated with acanthosis nigricans.

Hirsutism is treated with a combination of approaches, including oral contraceptives with or without an antiandrogen, such as spironolactone, to lower levels or block actions of androgen on hair follicles. Oral contraceptives are often combined with antiandrogens to improve their clinical effect and to prevent pregnancy, since accidental exposure of the male fetus to antiantrogens can harm fetal development. A topical cream (eflornithine hydrochloride) also can be applied to treat facial hirsutism.
A surgery called ovarian drilling might make your ovaries work better when ovulation medications don't, but it's being done less often than it used to. The doctor makes a small cut in your belly and uses a tool called a laparoscope with a needle to poke your ovary and wreck a small part of it. The procedure changes your hormone levels and may make it easier for you to ovulate.
