Positive effects for Vitex agnus-castus in oligo/amenorrhoea and infertility was demonstrated in three placebo controlled RCTs [61, 62, 64]. In a study including women with menstrual irregularity and infertility (n = 96), menstrual cyclicity was significantly improved for women treated with Vitex agnus-castus (Mastodynon® 30 drops per day for three months) compared to placebo (p = 0.023)  (Table 2). Another study, including women with sub fertility (n = 67), showed improved menstrual cyclicity for a sub-group of women with oligomenorrhoea following treatment with Vitex agnus-castus (Phyto-Hypophyson® 7.5 ml per day) compared to placebo, (p = 0.023)  (Table 2). A third study including women with hyperprolactinaemia (n = 37) demonstrated improved menstrual cyclicity by an increased average number of luteal days from 3.4 days (±5.0) to 10.5 days (±4.3) (p < 0.005) following treatment with Vitex agnus-castus (Strotan® 20 mg per day) for three months. The placebo group reported average number of days in the luteal phase was 3.4 (±5.1) at baseline and 5.5 (±5.2) at three months, which was not significant (p = 0.22)  (Table 2). Methodological shortcomings included not reporting baseline characteristics for subgroups and small sample sizes; however clinical outcomes demonstrated physiological effects consistent with laboratory and animal findings (Tables 1 and and22).

Studies suggest that the following agents may improve menstrual irregularities (e.g., oligomenorrhea): spironolactone (in an open-label study),22 acarbose,24 rosigli-tazone,32 and metformin.10,11,32,47,51 Metformin is probably the best choice because it may improve insulin resistance in addition to improving menstrual irregularities.

was on a high dose of seroquel for schizoaffective (ended up going off and trying other antipsychotics) but basically i acted like a narcoleptic. i'd fall asleep in the middle of class without warning or i'd fall asleep during a ten min car ride. i was just constantly asleep and it wasnt a peaceful sleep either it was like dead sleep and i woke up feeling shitty. also like all antipsychotics make you gain weight but sometimes it's not more than 10 lbs or so so maybe between that and heroin thats why tuna's not gaining much weight?

In vitro fertilization represents the third-line treatment for infertility in women with PCOS 9. However, if the initial assessment demonstrates a bilateral tubal occlusion and/or concentration of recovered motile sperm less than or equal to 5 million, this treatment becomes the first option along with lifestyle changes. The risk of OHSS is the main complication of the highly complexity treatment in women with PCOS. Thus, to minimize this side effect, ovarian stimulation should be initiated with low doses of gonadotropins (100 to 150 IU of FSHr) and the pituitary should be suppressed with a gonadotropin-releasing hormone (GnRH) antagonist because this method is associated with a reduced risk of OHSS compared with an agonist (29 randomized control trials (RCTs); OR 0.43; 95% CI: 0.33 to 0.57) 32. If the patient presents with clinical and ultrasound signs of OHSS, final oocyte maturation should be performed with a GnRH agonist and embryos should be frozen and transferred in a subsequent cycle 33,34. Infertile women with PCOS may present with better general oocyte and embryo quality rates; however, the clinical pregnancy and live birth rates are similar to those observed in normo-ovulatory women without PCOS 35.

I thought that too at first (i've been following her since she was like 16, unfollowed for a while, then when i refollowed came back to grimy heroin luna). It's because her hair is longer and she's gotten thinner. That's really it. Everything else from her youth looks so much better. The light in her eyes, the freckles, the friendships, the cute babydoll dresses… She just looked happier…. I mean sure she was chunky and her hair was always mangled, but she was happy and that's much more important IMO.

One laboratory study and two clinical investigations provided evidence for the two herb combination, Glycyrrhiza uralensis and Paeonia lactiflora[53, 58, 59] (Table 1). An animal study found significant reductions in free and total testosterone following exposure to the combination  (Table 1). These findings were supported in two open label clinical trials including women with PCOS (n = 34)  and women with hyperandrogenism (n = 8) . Both trials examined the effects on androgens for the aqueous extract TJ-68 (equal parts Glycyrrhiza uralensis and Paeonia lactiflora), 75 grams per day for 24 weeks and 5–10 grams per day for 2–8 weeks respectively. In the trial including women with PCOS, mean serum testosterone was significantly reduced from 137.1 ng/dL (±27.6) to 85.3 ng/dL (±38), p < 0.001 at four weeks of treatment . Similar effects were observed in the women with oligomenorrhoea and hyperandrogenism which showed serum testosterone reduced from 50-160 ng/dL prior to treatment to less than 50 ng/dL . However statistical significance was not reached due to the small sample size despite positive outcomes in seven out of eight participants (Table 1).

The principle infertility treatment includes lifestyle changes. The first-line drug treatment to induce ovulation consists of CC with timed intercourse. The second-line treatment consists of the exogenous administration of gonadotropins or laparoscopic ovarian surgery in cases where laparoscopy is indicated. The third-line treatment consists of IVF/ICSI, which is indicated when the previous interventions fail; this treatment can also be the first choice in cases of bilateral tubal occlusion or semen alterations that impair the occurrence of natural pregnancy. There is no evidence for the routine use of metformin in infertility treatment of anovulatory women with PCOS. Aromatase inhibitors are promising, and long-term studies are necessary to prove their safety.
Because of its antiandrogenic effects, spironolactone is effective, but not FDA-approved, for this indication.22,23 A Cochrane review suggested that spironolactone is superior to finasteride.28 Combining spironolactone with oral contraceptives may be synergistic, but caution should be used in women taking drospirenone because each agent can cause hyperkalemia.2 Spironolactone is FDA pregnancy category C.
Women who do not wish to become pregnant can be effectively treated for hirsutism with oral contraceptives.  Oral contraceptives slow hair growth in 60-100% of women with hyperandrogenemia. Therapy can be started with a preparation that has a low dose of estrogen and a nonandrogenic progestin. Preparations that have norgestrel and levonorgestrel should be avoided because of their androgenic activity. There is also a risk of thrombotic events in obese women who use oral contraceptives; therefore, the proper precautions should be exercised to prevent such events. Oral contraceptives containing cyproterone acetate are also very effective in the treatment of more severe hirsutism  ; however, this combination of agents has not been approved by the FDA for use in the United States.
Many women with PCOS have decreased sensitivity to insulin, the hormone that regulates glucose (sugar) in the blood. This condition, known as insulin resistance, is a major risk factor for type 2 diabetes. Women with PCOS often have type 2 diabetes, which occurs more frequently in women with PCOS. Signs of insulin resistance include weight gain (especially around the waist), acanthosis nigricans (skin thickening around the neck, armpits, belly, button, and other creases), and skin tags.

Shahin  Non-blinded randomised controlled trial. Women with PCOS and infertility, n = 194. All participants received pharmaceutical ovulation induction (Clomiphene citrate 150 mg on days 3–7 of cycle); trigger injection (HCG 10000 IU Pregnyl), timed intercourse and progesterone support (oral micronized progesterone). A randomly selected group additional took Cimicifuga racemosa 120 mg per day (Klimadynon®) Primary outcomes pregnancy rates. Secondary outcomes: Pregnancy rates were 33 out of 192 cycles (17.2%) for the clomiphene alone group and 71 out of 204 cycles (34.8%) for the clomiphene plus Cimicifuga racemosa group. Non-blinding compromised the internal validity of the findings in this study. Confounding variables include variations in participant’s and clinicians attitudes and may have led to differences which were unaccounted for between the two groups. However the outcomes are objective with a statistically powered sample size.
The name ‘Polycystic Ovarian Syndrome’ points to the ovaries because it was long considered to be a reproductive issue. But it is now widely accepted that polycystic ovaries develop as a result of endocrine disorders characterized by a series of hormone imbalances: hyperandrogenism (specifically excess testosterone) and Insulin Resistance due to excess insulin that can trigger a cascade of other hormonal problems.4 From a systemic point of view, the continuing and/or increase of PCOS symptoms is likely due to a continuing hormonal imbalance.

Metformin has been studied specifically in adolescent PCOS women. Metformin therapy for 10 mo decreased fasting serum insulin levels in obese girls with PCOS.The positive effects of metformin in adolescents wore off within 3 mo of medication discontinuation. Metformin in obese PCOS adolescents has shown improvements in IR by clamp studies, fasting measurements and OGTT after just 3 mo of therapy[76,77]. Other studies have found non-significant trends to improved IR by HOMA and OGTT-AUC in adolescent PCOS patients. Metformin has also been shown to effectively contribute to BMI reduction in PCOS adolescents.

A 2017 review concluded that while both myo-inositol and D-chiro-inositols may regulate menstrual cycles and improve ovulation, there is a lack of evidence regarding effects on the probability of pregnancy. A 2012 and 2017 review have found myo-inositol supplementation appears to be effective in improving several of the hormonal disturbances of PCOS. Myo-inositol reduces the amount of gonadotropins and the length of controlled ovarian hyperstimulation in women undergoing in vitro fertilization.A 2011 review found not enough evidence to conclude any beneficial effect from D-chiro-inositol. There is insufficient evidence to support the use of acupuncture.
