Human data regarding metformin improvement in IR in PCOS women shows mixed results and is complicated by varying methods of assessing IR. Short term (3 mo) treatment with metformin (1500 mg per day) failed to affect IR as measured by AUC-Insulin after 75-g OGTT. Metformin (1600 mg per day) in obese PCOS women treated for 6 mo failed to reduce IR as measured by QUICKI. This is in contrast to similar length studies on obese PCOS women who demonstrated decreased IR as measured by HOMA-IR, QUICKI and ISI, and correlated with alterations in phosphoproteins related to IR. Longer term metformin therapy (2 years, 1600 mg per day) in young, obese PCOS women reduced fasting insulin, hyperandrogenism and produced borderline reductions in HOMA-IR (P = 0.05). Metformin was compared prospectively to naltrexone and prenisolone in combination with oral contraceptive pills (OCPS). IR was unchanged despite lowered androgen levels. Metformin has been compared to orlistat and pioglitazone over a 4 mo treatment course and although each treatment reduced IR as measured by HOMA-IR, metformin (1500 mg per day) had the least reduction (< 20%).
Glucose tolerance testing (GTT) instead of fasting glucose can increase diagnosis of impaired glucose tolerance and frank diabetes among people with PCOS according to a prospective controlled trial. While fasting glucose levels may remain within normal limits, oral glucose tests revealed that up to 38% of asymptomatic women with PCOS (versus 8.5% in the general population) actually had impaired glucose tolerance, 7.5% of those with frank diabetes according to ADA guidelines.

The treatment of polycystic ovarian syndrome has evolved considerably since the influence of insulin has been discovered. In the past, treatment was often aimed exclusively at specific symptoms rather than trying to impact some of the underlying influences. Symptoms can definitely be improved individually but addressing influencing conditions like insulin resistance often produces broader results. If insulin resistance is a factor in your PCOS (some women with PCOS are not insulin resistant), then the most effective course of action is to improve your diet. Start a regular exercise program and take supplements or medication recommended by your health care provider.

This study synthesises the evidence for reproductive endocrine effects for six whole herbal medicine extracts that may be used to treat PCOS and associated oligo/amenorrhoea and hyperandrogenism. The findings were intended to add to clinicians understanding for the mechanisms of action for herbal medicine for treatment in these common conditions and reveal herbal medicines with reproductive endocrinological effects, currently demonstrated in scientific literature.
New evidence suggests that using medications that lower insulin levels in the blood may be effective in restoring menstruation and reducing some of the health risks associated with PCOS. Lowering insulin levels also helps to reduce the production of testosterone, thus diminishing many of the symptoms associated with excess testosterone: hair growth on the body, alopecia (scalp hair loss), acne and possibly cardiovascular risk.

Polycystic ovary syndrome is the most common endocrinopathy among reproductive-aged women in the United States, affecting approximately 7% of female patients. Although the pathophysiology of the syndrome is complex and there is no single defect from which it is known to result, it is hypothesized that insulin resistance is a key factor. Metabolic syndrome is twice as common in patients with polycystic ovary syndrome compared with the general population, and patients with polycystic ovary syndrome are four times more likely than the general population to develop type 2 diabetes mellitus. Patient presentation is variable, ranging from asymptomatic to having multiple gynecologic, dermatologic, or metabolic manifestations. Guidelines from the Endocrine Society recommend using the Rotterdam criteria for diagnosis, which mandate the presence of two of the following three findings—hyperandrogenism, ovulatory dysfunction, and polycystic ovaries—plus the exclusion of other diagnoses that could result in hyperandrogenism or ovulatory dysfunction. It is reasonable to delay evaluation for polycystic ovary syndrome in adolescent patients until two years after menarche. For this age group, it is also recommended that all three Rotterdam criteria be met before the diagnosis is made. Patients who have marked virilization or rapid onset of symptoms require immediate evaluation for a potential androgen-secreting tumor. Treatment of polycystic ovary syndrome is individualized based on the patient's presentation and desire for pregnancy. For patients who are overweight, weight loss is recommended. Clomiphene and letrozole are first-line medications for infertility. Metformin is the first-line medication for metabolic manifestations, such as hyperglycemia. Hormonal contraceptives are first-line therapy for irregular menses and dermatologic manifestations.
