Diagnosis of polycystic ovarian syndrome (PCOS) is relatively straightforward. Common criteria established by the Rotterdam Conference in 2003 include at least two of three characteristics (oligomenorrhea, clinical and/or biochemical hyperandrogenism and ultrasound criteria) in the absence of other disease. PCOS is the most common hormonal disorder in women worldwide with prevalence estimates between 4%-8% but as high as 25% in some populations[1]. Women often initiate medical care for a cluster of PCOS symptoms (infertility, hirsutism and irregular menstrual cycles) that ultimately are not the most concerning medical consequences of PCOS [diabetes mellitus (DM), coronary artery disease (CAD), endometrial hyperplasia/cancer]. Here exists an important paradigm in the recognition and treatment of PCOS.
The genetic component appears to be inherited in an autosomal dominant fashion with high genetic penetrance but variable expressivity in females; this means that each child has a 50% chance of inheriting the predisposing genetic variant(s) from a parent, and, if a daughter receives the variant(s), the daughter will have the disease to some extent.[24][26][27][28] The genetic variant(s) can be inherited from either the father or the mother, and can be passed along to both sons (who may be asymptomatic carriers or may have symptoms such as early baldness and/or excessive hair) and daughters, who will show signs of PCOS.[26][28] The phenotype appears to manifest itself at least partially via heightened androgen levels secreted by ovarian follicle theca cells from women with the allele.[27] The exact gene affected has not yet been identified.[7][24][29] In rare instances, single-gene mutations can give rise to the phenotype of the syndrome.[30] Current understanding of the pathogenesis of the syndrome suggests, however, that it is a complex multigenic disorder.[31]
Clomiphene is an ovulation induction agent that has been used and studied in patients with and without PCOS.6–8,15,35,36 Studies have found that letrozole (Femara) regulates ovulation and improves pregnancy rates in women with PCOS6,37,38; however, this use is controversial because the drug is FDA pregnancy category D. It is embryotoxic and fetotoxic in animal studies, and there are no studies in pregnant women.

There is no cure yet, but there are many ways you can decrease or eliminate PCOS symptoms and feel better. Your doctor may offer different medicines that can treat symptoms such as irregular periods, acne, excess hair, and elevated blood sugar. Fertility treatments are available to help women get pregnant. Losing as little as 5% excess weight can help women ovulate more regularly and lessen other PCOS symptoms. The ideal way to do this is through nutrition and exercise.
Two laboratory based RCT’s examined the effects of Tribulus Terrestris in rats with polycystic ovaries induced with oestradiol valerate [46, 47] (Table 1). Both studies demonstrate significantly improved ovulation rates for animals treated with two doses of Tribulus terrestris extracts compared to controls. Although the endocrinological effects were not described in either study, laboratory findings of ovulation induction are supported by the clinical findings of elevated FSH following treatment with Tribulus terrestris[56] (Table 2).
This thread fucks me up. I feel so much pity and sadness for this poor girl. Sort of hits close to home, when I turned 18 I moved in with a boy and got really sick and hooked on drugs for 2 years. I left him and got better, never did I ever get as bad as Luna but it could have happened had I been complacent like she is. I hope she finds the courage to leave. PCOS treatment is different for different people. While there is no cure for PCOS, taking medicine and losing weight can help  your symptoms. If you don’t want to become pregnant, your doctor or nurse may recommend hormonal birth control, like the hormonal IUD, birth control implant, pill, patch, ring, or shot to treat your PCOS. The pill, patch, or ring may be particularly helpful if you’re struggling with acne or want more regular periods. If you’re trying to get pregnant, drugs that treat insulin resistance may help, as well as certain fertility drugs that can help you ovulate. Losing weight can also help with ovulation and fertility.


Lack of ovulation (monthly release of an egg from the ovary) is the predominant cause for infertility in PCOS women, although some individuals also may have a higher rate of miscarriage. In some PCOS women who are overweight, diet combined with exercise can restore ovulation without the use of medications. Otherwise, several medications are available to restore ovulation in PCOS women. Oral clomiphene citrate stimulates FSH secretion from a woman’s own pituitary (located at the base of the skull) to induce ovarian follicle growth. Subcutaneous injections of gonadotropins with FSH and LH can induce the same events, if necessary. Both medications increase the chance of multiple births, although FSH injections are the most powerful. Other medications include metformin, an insulin sensitizer that slowly lowers insulin and androgen levels to gradually induce ovulation with a lower chance of multiple births. Medications that block estrogen production to stimulate FSH secretion (letrozol) also exist, but additional studies are needed before they can be routinely recommended for ovulation induction. Since most PCOS women ovulate following medical therapy, surgery or in-vitro fertilization (IVF) are generally reserved for PCOS women who fail to ovulate with medication or have other fertility problems.

A prospective, observational clinical trial examined the endocrine effects of Tribulus terrestris 750 mg per day, over five days in eight healthy women (aged 28–45). A significant increase in mean serum FSH concentration from 11 mIU/ml before treatment to 17.75 mIU/ml following treatment (P < 0.001) was demonstrated. Pre-treatment FSH levels returned following cessation of treatment (Table 1). Another clinical study evaluated the equivalence of Tribulus terrestris (Tribestan®) and pharmaceuticals for ovulation induction in women with oligo/anovular infertility (n = 148), [60]. During the three month follow up, ovulation rates were highest with epimestrol (74%), followed by Tribulus terrestris (60%), clomiphene (47%) and cyclofenil (24%). However, the evidence for Tribulus terrestris should be interpreted with caution due to risks for bias in clinical studies. One study was uncontrolled with a small number of healthy participants [56], the second study did not report baseline characteristics, methods for allocation to treatment groups and data were not statistically analysed [60] (Table 1).

The exact causes of PCOS are unknown; there are probably more than one cause and these may be different between individual girls or women. Androgen excess (male hormone excess), seen in 60-80% of girls and women with PCOS, is a key problem in the disorder and likely comes from ovaries in most women. Insulin resistance or elevated insulin levels may worsen androgen excess. Abnormalities in how the brain or pituitary gland communicate with the ovaries may also lead to androgen overproduction. PCOS is the most common endocrinopathy among reproductive-aged women in the United States, affecting approximately 7% of female patients.1 Although its exact etiology is unclear, PCOS is currently thought to emerge from a complex interaction of genetic and environmental traits. From what I know, all women with PCOS don't have regular period at all! Those who were even trying to conceive needed to a tool to track their ovulation, most of my friends use a free bbt from cocneiveeasy. I have a friend who's trying for months now, unfortunately she has PCOS and still not pregnant, she's taking conceiveeasy along with Clomid now, I hope it works for her.
they do seem vain but to be fair, she did get them years ago, before she turned into the grimy junkie she is today. at the time she got them (i've been following her since like 2011-2012), they seemed really clever to me. i can't really recall her scamming people for money and whatnot at that point. she was just a young girl who mentioned her abusive mother and stuff and i think most people just thought of the tattoos like "oh she's got such amazing self confidence! i'm inspired!"
Treatments for hirsutism in women with PCOS are similar to those in women without PCOS, such as patients with idiopathic hirsutism. There are many nonpharmacologic treatment options, including electrolysis, waxing, bleaching, plucking, depilatory creams (a form of hair removal that dissolves the hair), thermolysis (use of heat), and laser therapy. Several medications have been studied for the treatment of hirsutism in women with PCOS. First-line agents include spironolactone (Aldactone)22,23,28–30 and metformin,13,16,20,22,31–33 as well as eflornithine (Vaniqa) for facial hirsutism.9

Results of a second study by the same research team offer doctors a clear strategy to help you lose weight.5 According to this study at Penn State Health in Philadelphia, patients who worked with their physician to follow a limited fat, low calorie diet (~1200 calories) by relying on low-calorie prepared meals (eg, SmartOnes, Lean Cuisine or Healthy Choice) and an appetite suppressant with meals and making lifestyle changes plus added fruits and vegetables.4
Androgen lowering effects for Glycyrrhiza spp. have been demonstrated in one laboratory study examining hormone concentration in female rats (Glycyrrhiza uralensis), [53] and corroborated in two clinical trials, one including healthy women [55] and the other including women with PCOS (Glycyrrhiza glabra) [54] (Table 1). The animal study reported significantly reduced free and total testosterone and increased oestradiol in sterilised rats and no hormonal changes in oophrectomised rats. The authors conclude that the hormonal effects occurred primarily in the ovary via enhanced aromatisation of testosterone to 17-beta oestradiol. The investigators also observed significantly increased oestradiol. There were no changes to FSH or LH in androgen sterilised or oophrectomised rats [53].

2. Oestrogen receptor binding affinity for Cimicifuga racemosa was studied using pituitary cell cultures from ovariectomised rats. This study followed a clinical study demonstrating significantly lowered LH in post-menopausal women following administration of Cimicifuga racemosa (2 mg for two months) against placebo control (n = 110) [45] 2. Increases luteal progesterone concentration [65, 67, 68]
According to Dr. Geoffrey Redmond, an endocrinologist specializing in female hormones, “Just because the ovaries are not functioning as much doesn’t mean the other abnormalities won’t still be present.” He goes on to point out that studies show male hormone levels climb fairly sharply with age.2 This could mean a worsening of symptoms such as excess hair growth as those hormones become more active. It could also mean insulin-related issues such as diabetes and cardiovascular health could become more problematic.

Positive effects for Vitex agnus-castus in oligo/amenorrhoea and infertility was demonstrated in three placebo controlled RCTs [61, 62, 64]. In a study including women with menstrual irregularity and infertility (n = 96), menstrual cyclicity was significantly improved for women treated with Vitex agnus-castus (Mastodynon® 30 drops per day for three months) compared to placebo (p = 0.023) [61] (Table 2). Another study, including women with sub fertility (n = 67), showed improved menstrual cyclicity for a sub-group of women with oligomenorrhoea following treatment with Vitex agnus-castus (Phyto-Hypophyson® 7.5 ml per day) compared to placebo, (p = 0.023) [62] (Table 2). A third study including women with hyperprolactinaemia (n = 37) demonstrated improved menstrual cyclicity by an increased average number of luteal days from 3.4 days (±5.0) to 10.5 days (±4.3) (p < 0.005) following treatment with Vitex agnus-castus (Strotan® 20 mg per day) for three months. The placebo group reported average number of days in the luteal phase was 3.4 (±5.1) at baseline and 5.5 (±5.2) at three months, which was not significant (p = 0.22) [64] (Table 2). Methodological shortcomings included not reporting baseline characteristics for subgroups and small sample sizes; however clinical outcomes demonstrated physiological effects consistent with laboratory and animal findings (Tables 1 and ​and22).

Yeah, PCOS isn't period-dependent. That said, there are some suspicious blood stains in Luna's apartment that look like leaks (though they could possibly be explained by shooting up). Though since she does have PCOS, painful periods are one of those things she actually has grounds to complain about. Apparently they're often excruciating beyond belief with the condition.
Often, women with PCOS tend to put on weight easily and have difficulty losing weight. This is because women with PCOS are thrifty with calories. If they eat too much, extra insulin is needed to dispose of the extra sugar. Sadly, this tendency can also increase the risk of diabetes. This is more likely in those with a family history of adult-onset diabetes. Women with PCOS are more likely to develop high blood pressure and diabetes during pregnancy, particularly if they are overweight and have higher than normal insulin levels.

Some other blood tests are suggestive but not diagnostic. The ratio of LH (Luteinizing hormone) to FSH (Follicle-stimulating hormone), when measured in international units, is elevated in women with PCOS. Common cut-offs to designate abnormally high LH/FSH ratios are 2:1[66] or 3:1[62] as tested on Day 3 of the menstrual cycle. The pattern is not very sensitive; a ratio of 2:1 or higher was present in less than 50% of women with PCOS in one study.[66] There are often low levels of sex hormone-binding globulin,[62] in particular among obese or overweight women.[citation needed]
