PCOS is the most common endocrinopathy among reproductive-aged women in the United States, affecting approximately 7% of female patients.1 Although its exact etiology is unclear, PCOS is currently thought to emerge from a complex interaction of genetic and environmental traits. Medical management of PCOS is aimed at the treatment of metabolic derangements, anovulation, hirsutism, and menstrual irregularity. The use of insulin-sensitizing drugs to improve insulin sensitivity is associated with a reduction in circulating androgen levels, as well as improvement in both the ovulation rate and glucose tolerance. [3] The Endocrine Society has published a clinical practice guideline on hirsutism evaluation and treatment in premenopausal women. [51] ACOG notes that eflornithine in conjunction with laser treatment is superior to laser therapy alone in treating hirsutism. [3]

Clomiphene (Clomid) is an oral medication that is the most common treatment used to induce ovulation. The use of both metformin and clomiphene has about the same fertility results as clomiphene use alone. Diagnosis of PCOS may be difficult because the signs and symptoms can be subtle and varied. The most common manifestations include hirsutism, infertility, insulin resistance, and menstrual irregularities.2 Physicians can diagnose PCOS when other causes of the symptoms or laboratory abnormalities are excluded; when oligo-ovulation or anovulation, usually manifested as oligomenorrhea or amenorrhea, is present; and when there is clinically confirmed hyperandrogenism (e.g., hirsutism, acne). Although the ovaries may be polycystic, this is usually not necessary for diagnosis. There is debate over which criteria should be used (e.g., 1990 National Institutes of Health criteria,3 2003 Rotterdam consensus workshop criteria4). Guidelines suggest screening women with PCOS for other disorders, such as hyperlipidemia, and treating accordingly.5

A team approach involving care by primary care and subspecialist physicians can be helpful to address the multiple manifestations of the syndrome.
