PCOS is a life-long condition and although the exact cause is yet to be identified, it is believed to have epigenetic origins, influenced by the uterine environment and behavioural factors [19]. Being overweight exacerbates all aspects of PCOS due to underlying metabolic disturbances [3]. Signs and symptoms are mediated by hormonal disorder including elevated androgens and fasting insulin, and abnormal relative ratio of the gonadotropins luteinising hormone (LH) and follicle stimulating hormone (FSH) [19]. Endocrine imbalances occur within the framework of disordered ovarian folliculogenesis, chronic anovulation, clinical signs of hyperandrogenism and metabolic syndrome [19].
For women who don’t receive timely, appropriate care for PCOS in early adolescence, the development of symptoms such as facial-hair growth can become more challenging to treat. Brandy Cramer, 33, a program officer at The Cameron Foundation, from Midlothian, Virginia says her doctors told her she just wasn’t trying hard enough to lose weight and dismissed her when she requested they run blood tests or suggest alternatives to the birth-control pills that gave her intense migraines. Cramer grew facial hair and has only been able to remove 50 percent of it, even after expensive laser hair-removal treatment.
The prevalence of infertility in women with PCOS varies between 70 and 80%. According to the American Society for Reproductive Medicine, the evaluation of infertility in women with PCOS or other causes of subfertility should start after six months of attempting pregnancy without success if the couple has regular sexual intercourse (2 to 3 times/week) without using contraceptive methods 7. To optimize the efficacy of the treatment of infertile women with PCOS, evaluations of tubal patency (hysterosalpingography or laparoscopy with chromotubation) and semen analysis (spermogram) are mandatory before deciding on treatment. However, tubal patency evaluation may not be necessary prior to initiating clomiphene citrate (CC) treatment. Notably, if a patient is resistant to this drug and/or requires the use of gonadotropins and/or presents with other causes of infertility, a tubal patency evaluation becomes mandatory prior to initiating the therapeutic treatment of infertility 8.

The second-line pharmacological treatment of infertility in anovulatory women with PCOS includes the use of gonadotropins [recombinant follicle-stimulating hormone (FSHr) or human menopausal gonadotropin (HMG)] for timed intercourse or intrauterine insemination (IUI) 9. Due to the higher cost of this therapeutic modality, an evaluation of the tubal patency is recommended prior to initiating the ovarian stimulation with gonadotropins if this procedure was not performed prior to initiating CC treatment. If the fallopian tube is opened and the sperm concentration is suitable for in vivo fertilization, the ovarian stimulation begins with low doses of gonadotropins (37.5 to 75 IU/day or every other day) to achieve monofollicular growth and reduce the risk of complications (OHSS and multiple gestation) 25. US monitoring of the follicular growth (follicular diameter measurement) is mandatory in this case and the endogenous secretion of gonadotropins does not need to be inhibited with gonadotropin-releasing hormone analogues (GnRH-a) during the timed intercourse cycles. The administration of hCG (used to simulate the endogenous peak of luteinizing hormone for final oocyte maturation and ovulation triggering) is unnecessary because it does not increase the probability of conception during ovulation induction cycles for timed intercourse 21. It is important to note that if gonadotropin is chosen as the treatment option, the IUI has a higher likelihood of successful pregnancy compared with timed intercourse in patients with subfertility 26.


In anovulatory women with PCOS defined according to the Rotterdam consensus (includes all phenotypes except the one defined by the association of hyperandrogenism with ultrasound (US) findings), CC treatment is the first choice for ovulation induction 9,15. This drug is an estrogen receptor modulator (it can act as an estrogen agonist or antagonist) and its mechanism of action is controversial but can be explained as follows. In physiological menstrual cycles, low levels of estrogen promote negative feedback in the hypothalamus and pituitary gland and inhibit the endogenous secretion of gonadotropin during the early follicular phase. When CC is administered in this phase of the cycle, it competes with estrogen for its receptors in the hypothalamus and pituitary, which will block the negative feedback mechanism. Consequently, increased levels of endogenous gonadotropins are released and the dominant follicle is recruited (follicle that has the highest number of follicle-stimulating hormone (FSH) receptors) between the sixth and ninth day of the menstrual cycle 16.

The condition, which affects five million women in the U.S. alone, can wreak havoc on your hormones and still doesn’t have a known cure. While many women successfully manage their symptoms solely with medication and hormone therapy, some prefer to supplement traditional treatments with more holistic remedies, while some opt for entirely natural alternatives.
Other insulin sensitizing agents have been advocated and studied for the treatment of IR in PCOS, principally thiazolinediones. Thiazolinediones stimulate gene transcription that alters lipid and glucose metabolism, decreases lipolysis and decreases fat deposition[90]. Thiazolinediones decrease fatty acid release, suppress gluconeogenesis and reduce tumor necrosis factor α disruption of insulin activity[64]. Pioglitazone and rosiglitazone have decreased IR (measured by clamp studies) in PCOS women[90-93]. Glitazones have also decreased IR by OGTT AUC-Insulin in PCOS women[91,93,94]. In patients with DM, thiazolinediones reduce central adiposity[95], a trait commonly shared with PCOS women. Pioglitazone by way of IR and adiponectin levels also has improved menstrual regularity in PCOS women[96,97]. Adverse outcomes have been seen in pregnant animals with limited to no human data. Therefore, as a class, thiazolinediones are not considered first line therapy for PCOS women seeking pregnancy. Rosiglitazone has even been found to decrease pro-inflammatory markers in human granulosa cells cultured following in vitro fertilization oocyte retrieval, thus showing additional target tissue for therapy[98]. However, these effects have not been adequately studied and have no current practical application.

This thread fucks me up. I feel so much pity and sadness for this poor girl. Sort of hits close to home, when I turned 18 I moved in with a boy and got really sick and hooked on drugs for 2 years. I left him and got better, never did I ever get as bad as Luna but it could have happened had I been complacent like she is. I hope she finds the courage to leave. I think there's still beauty and potential underneath all the grime and illness, I even like her art sometimes and wish she'd spend more time on her paintings instead of on drugs. The homeostatic model assessment (HOMA), a more complex fasting calculation, has been compared to clamp techniques with good results. HOMA is the product of fasting glucose (mg/dL) and insulin (μU/mL) divided by a constant[45]. One major limitation of HOMA rests on the previous reflection that many young PCOS women display stimulated but not fasting metabolic abnormalities. In fact, HOMA in young PCOS patients missed 50% of IR as compared to OGTT with insulin-AUC calculations[52]. G/I ratio correlated strongly with clamp-demonstrated IR in a small study of PCOS women - interestingly, both lean and obese PCOS women had evidence of IR. Sex hormone binding globulin (SHBG) did not correlate with IR in this study[47], as has been previously postulated[53].
That makes a lot of sense. The unfortunate truth is that there is no cure for PCOS. But because the disorder can have a profound effect on your health and well-being, treatment is a must. There is one important thing to know about your options: Because the cause of PCOS is still unknown, appropriate therapies are based on a woman’s individual concerns. Is she bothered most about excess hair growth? Does she want to have a baby? Does she need to get her periods back on track?
Polycystic ovary syndrome represents 80% of anovulatory infertility cases. Treatment initially includes preconception guidelines, such as lifestyle changes (weight loss), folic acid therapy to prevent the risk of fetal neural tube defects and halting the consumption of tobacco and alcohol. The first-line pharmacological treatment for inducing ovulation consists of a clomiphene citrate treatment for timed intercourse. The second-line pharmacological treatment includes the administration of exogenous gonadotropins or laparoscopic ovarian surgery (ovarian drilling). Ovulation induction using clomiphene citrate or gonadotropins is effective with cumulative live birth rates of approximately 70%. Ovarian drilling should be performed when laparoscopy is indicated; this procedure is typically effective in approximately 50% of cases. Finally, a high-complexity reproduction treatment (in vitro fertilization or intracytoplasmic sperm injection) is the third-line treatment and is recommended when the previous interventions fail. This option is also the first choice in cases of bilateral tubal occlusion or semen alterations that impair the occurrence of natural pregnancy. Evidence for the routine use of metformin in infertility treatment of anovulatory women with polycystic ovary syndrome is not available. Aromatase inhibitors are promising and longer term studies are necessary to prove their safety.
Fasting insulin level or GTT with insulin levels (also called IGTT). Elevated insulin levels have been helpful to predict response to medication and may indicate women needing higher dosages of metformin or the use of a second medication to significantly lower insulin levels. Elevated blood sugar and insulin values do not predict who responds to an insulin-lowering medication, low-glycemic diet, and exercise. Many women with normal levels may benefit from combination therapy. A hypoglycemic response in which the two-hour insulin level is higher and the blood sugar lower than fasting is consistent with insulin resistance. A mathematical derivation known as the HOMAI, calculated from the fasting values in glucose and insulin concentrations, allows a direct and moderately accurate measure of insulin sensitivity (glucose-level x insulin-level/22.5).[citation needed]
A secondary analysis of two randomized, double blind, placebo-controlled trials that included 182 children of mothers with PCOS reported that children exposed to metformin had higher BMI and increased prevalence of overweight/obesity at 4 years of age. The study found that at 4 years of age, the metformin group had higher weight z-score than the placebo group; difference in means 0.38 (0.07 to 0.69), p=0.017, and higher BMI z-score; difference in means 0.45 (0.11 to 0.78), p=0.010. There were also more overweight/obese children in the metformin group; 26 (32%) than in the placebo group; 14 (18%) at 4 years of age; odds ratio (95% CI): 2.17 (1.04 to 4.61), p=0.038. More studies are needed to examine this association. [94]
A study supervised by Columbia University School of Nursing professor Nancy Reame, MSN, PhD, FAAN, and published in the Journal of Behavioral Health Services & Research, identifies the PCOS complications that may be most responsible for psychiatric problems. While weight gain and unwanted body hair can be distressing, irregular menstrual cycles is the symptom of PCOS most strongly associated with psychiatric problems, the study found.

Like, what's wrong with actually going through withdrawal? Maybe I'm old school, but I withdrew cold turkey 9 times in the process of getting clean, and that's the easiest part of recovery. Take loperamide for the diarrhea and deal w/ it. Granted, I did genuinely want to get sober, and I doubt Luna does, but still. Choices, consequences. Don't shoot heroin if you can't handle the diarrhea when you're without it. Pfffft, junkies these days.
Goats Rue (Galega officinalis) There are more clinical studies required to definitively show the benefits of Goats Rue for women with PCOS; however, it is the natural source of guanidine which is in an anti-diabetic drug class known as biguanides. A common drug for PCOS known as Metformin belongs to the biguanide drug class. This association alone should garner this herb a second look for treating polycystic ovarian syndrome.
PCOS stands for polycystic ovary syndrome. The exact cause of PCOS is unknown. What is known is that PCOS has to do with hormone imbalances. With PCOS, your body may have high amounts of two hormones: androgen and insulin. PCOS's principal signs and symptoms are related to menstrual disturbances and elevated levels of male hormones (androgens). Menstrual disturbances can include delay of normal menstruation (primary amenorrhea), the presence of fewer than normal menstrual periods (oligomenorrhea), or the absence of menstruation for more than three months (secondary amenorrhea ). Menstrual cycles may not be associated with ovulation (anovulatory cycles) and may result in heavy bleeding.
Because of its antiandrogenic effects, spironolactone is effective, but not FDA-approved, for this indication.22,23 A Cochrane review suggested that spironolactone is superior to finasteride.28 Combining spironolactone with oral contraceptives may be synergistic, but caution should be used in women taking drospirenone because each agent can cause hyperkalemia.2 Spironolactone is FDA pregnancy category C.
