If the fallopian tube is opened and the sperm concentration is suitable for in vivo fertilization, the ovarian stimulation begins with low doses of gonadotropins (37.5 to 75 IU/day or every other day) to achieve monofollicular growth and reduce the risk of complications (OHSS and multiple gestation) 25. US monitoring of the follicular growth (follicular diameter measurement) is mandatory in this case and the endogenous secretion of gonadotropins does not need to be inhibited with gonadotropin-releasing hormone analogues (GnRH-a) during the timed intercourse cycles. The administration of hCG (used to simulate the endogenous peak of luteinizing hormone for final oocyte maturation and ovulation triggering) is unnecessary because it does not increase the probability of conception during ovulation induction cycles for timed intercourse 21. It is important to note that if gonadotropin is chosen as the treatment option, the IUI has a higher likelihood of successful pregnancy compared with timed intercourse in patients with subfertility 26.

My brother was a Heroin addict who started taking Benzos between his shooting up, like Luna, versus the other way around. He was an addict, he took benzos to keep up whatever feeling he was chasing. Thats inherently different than an anxiety or depression patient taking them for relief of symptoms. Im sure it does happen, there's always that risk but I wouldn't go out there and say that Benzo use is always a gateway.

A case control study examining 100 infertile women with PCOS found that those who supplemented a daily 1500 mg dose of metformin, a medication commonly used to treat PCOS symptoms, with calcium and vitamin D saw improvements in BMI, menstrual abnormalities, and other symptoms. The women in the study added 1,000 mg of calcium a day and 100,000 IU of vitamin D a month to their daily metformin dose for six months.
The clinical presentation of PCOS is variable. Patients may be asymptomatic or they may have multiple gynecologic, dermatologic, or metabolic manifestations. Patients with PCOS most commonly present with signs of hyperandrogenism and a constellation of oligomenorrhea, amenorrhea, or infertility.19,22 Workup for PCOS is sometimes prompted by an incidental finding of multiple ovarian cysts after ultrasonography.
In the case of Polycystic Ovarian Syndrome, excess insulin causes the ovaries to produce excess testosterone, which can prevent ovulation and result in infertility. High insulin levels can also increase the conversion of testosterone into estrogen, which affects weight gain and the formation of ovarian cysts.5 Excess glucose is stored in fat cells that continue to pump out excess estrogen, further destabilizing the hormone system. In the vascular system, insulin increases the risk of heart disease and it is a known precursor to diabetes.6 Unfortunately, these risks increase as a woman ages.

If a woman's weight is excessive, the physician should be aggressive in championing a weight-loss program. Medications effective for weight loss (in addition to lifestyle modifications) that have been specifically studied in women with PCOS include metformin, acarbose, sibutramine, and orlistat (Xenical). Metformin is probably the first-line medication for obesity or weight reduction in patients with PCOS. Metformin results in a decrease in body mass index (BMI) of 1 to 2 kg per m2 or weight loss up to 6 lb, 10 oz to 8 lb, 13 oz (3 to 4 kg)10,46,49; acarbose results in an approximate 3 kg per m2 decrease in BMI24; sibutramine results in a decrease in BMI of 5.8 kg per m2 and weight loss of 31 lb, 11 oz (14.4 kg)26; and orlistat results in weight loss of approximately 11 lb (5 kg).50 However, a recent systematic review suggested that metformin is not effective for lowering BMI in patients with PCOS.1
Lifestyle change is considered the first-line treatment for infertility in obese women with PCOS. Preconception counseling, administering folic acid to reduce the risk of fetal neural tube defects, encouragement of physical activity and identification of risk factors, such as obesity, tobacco use and alcohol consumption, should be performed. A 5 to 10% loss in body weight over a period of six months regardless of body mass index may be associated with improvement in central obesity, hyperandrogenism and ovulation rate 9. However, no studies with the proper methodology have assessed the live birth rate, which is the primary reproductive outcome 10.
The differential diagnosis of PCOS is broad and includes both endocrinologic and malignant etiologies. Figure 119 provides an algorithm for the workup of select presentations. For any woman with suspected PCOS, the Endocrine Society recommends excluding pregnancy, thyroid dysfunction, hyperprolactinemia, and nonclassical congenital adrenal hyperplasia.19 Depending on presentation, conditions such as hypothalamic amenorrhea and primary ovarian insufficiency should also be excluded. In women with rapid symptom onset or significant virilization, such as deepening voice or clitoromegaly, an androgen-secreting tumor should be ruled out. Finally, Cushing syndrome or acromegaly should be excluded in patients with physical findings that suggest either condition.19 There is no need to order laboratory testing for these conditions if the patient does not have suggestive physical findings.

Mean platelet volume (MPV) is a marker associated with adverse cardiovascular events, and women with newly diagnosed PCOS appear to have significantly elevated MPV levels. [47] Kabil Kucur et al reported that use of ethinyl estradiol/cyproterone acetate or metformin for the treatment of women with PCOS seemed to have similar beneficial effects in reducing MPV. [47]

Polycystic ovary syndrome is a condition present in approximately 5 to 10 percent of women of childbearing age. Diagnosis can be difficult because the signs and symptoms can be subtle and varied. These may include hirsutism, infertility, menstrual irregularities, and biochemical abnormalities, most notably insulin resistance. Treatment should target specific manifestations and individualized patient goals. When choosing a treatment regimen, physicians must take into account comorbidities and the patient's desire for pregnancy. Lifestyle modifications should be used in addition to medical treatments for optimal results. Few agents have been approved by the U.S. Food and Drug Administration specifically for use in polycystic ovary syndrome, and several agents are contraindicated in pregnancy. Insulin-sensitizing agents are indicated for most women with polycystic ovary syndrome because they have positive effects on insulin resistance, menstrual irregularities, anovulation, hirsutism, and obesity. Metformin has the most data supporting its effectiveness. Rosiglitazone and pioglitazone are also effective for ameliorating hirsutism and insulin resistance. Metformin and clomiphene, alone or in combination, are first-line agents for ovulation induction. Insulin-sensitizing agents, oral contraceptives, spironolactone, and topical eflornithine can be used in patients with hirsutism.

PCOS's principal signs and symptoms are related to menstrual disturbances and elevated levels of male hormones (androgens). Pharmaceutical treatment for menstrual irregularity includes the oral contraceptive pill (OCP) and ovulation induction with clomiphene citrate (clomiphene) [20, 21] depending on fertility needs. Women with PCOS are however likely to exhibit contraindications for the OCP [3] and whilst induction of ovulation with clomiphene has demonstrated success, pregnancy rates remain inexplicably low [4]. Up to thirty 30% of women, particularly overweight women with PCOS, fail to respond to clomiphene therapy [4, 22, 23]. Management for hyperandrogenism includes anti-androgens and hypoglycaemic pharmaceuticals such as metformin [24]. Metfomin has demonstrated effectiveness for improving insulin sensitivity and hyperandrogenism, however use of metformin is associated with the high incidence of adverse effects including nausea, vomiting and gastro-intestinal disturbances [5].

Drop-out reasons were not reported seven in Cimicifuga racemosa group and four in clomiphene group.

2-Hour oral glucose tolerance test (GTT) in women with risk factors (obesity, family history, history of gestational diabetes)[17] may indicate impaired glucose tolerance (insulin resistance) in 15–33% of women with PCOS.[62] Frank diabetes can be seen in 65–68% of women with this condition.[citation needed] Insulin resistance can be observed in both normal weight and overweight people, although it is more common in the latter (and in those matching the stricter NIH criteria for diagnosis); 50–80% of people with PCOS may have insulin resistance at some level.[17]
