Three RCTs corroborate the positive fertility effects for Cimicifuga racemosa in women with PCOS, used in conjunction and when compared with the pharmaceutical Clomiphene citrate (clomiphene), [65, 68, 71] (Table 2). Results were reported for 441 women and show improved pregnancy rates when Cimicifuga racemosa was added to clomiphene during one menstrual cycle. In a study including women with PCOS (n = 147), pregnancy rates for the group receiving combined therapy (clomiphene 150 mg plus Cimicifuga racemosa 20 mg per day (Klimadynon®)) were 43.3% compared to 20.3% for women receiving only clomiphene [65] (Table 2). In another study using similar methodology (n = 100) pregnancy rates were 34.8% for the group treated with Cimicifuga racemosa plus clomiphene compared to 17.2% for women treated with clomiphene alone [68] (Table 2). Another study included women with PCOS and infertility (n = 100) compared Cimicifuga racemosa (Klimadynon®) and clomiphene over three months for hormone concentrations and pregnancy rates. Pregnancy rates were higher in the women in taking Cimicifuga racemosa compared to clomiphene, 14% and 8% respectively; however differences were not statistically significant. This study found significant effects for lowered luteinising hormone for women with PCOS receiving Cimicifuga racemosa compared to clomiphene (p = 0.007) [67]. Findings from clinical studies concur with laboratory and animal studies; however potential risks for bias include performance and collection bias due to lack of blinding (Table 2).

This review has some limitations. We used a methodological approach which was deductive and not consistent with traditional rationale for herbal selection. Our inclusion criteria for clinical studies were specific and relied upon our identification of herbal medicines with preclinical (laboratory based) evidence explaining the mechanisms of reproductive endocrinological effects in oligo/amenorrhoea, hyperandrogenism and PCOS. Clinical studies were excluded from this review due to the absence of evidence for whole herbal extracts. This was the case for Camellia sinensis (green tea) for which only one laboratory study investigated the effects of injecting epigallocatechin, a catechin found in green tea in animals [76]. High quality clinical evidence for Camellia sinensis was not presented in this review due to the absence of pre-clinical data explaining the mechanism for effect for the whole herbal extract [77]. Mentha spicata (spearmint) was another herbal medicine excluded from this review despite the availability of high quality clinical evidence demonstrating testosterone lowering effects in women with PCOS [78]. We found no laboratory evidence describing the mechanism of action for Mentha spicata in hyperandrogenism. Camilla sinensis and Mentha spicata are examples of herbal medicines excluded from this review due to not meeting the inclusion criteria. Studies investigating western herbal medicines excluded from this review are provided in Tables 3, ​,44 and ​and55.

Pioglitazone (Actos®) and Rosiglitazone (Avandia®) are insulin-sensitizing agents that improve glucose tolerance and insulin resistance. These drugs are approved by the Food and Drug Administration (FDA) for the treatment of diabetes. Although they are not approved for treatment of PCOS, they have been shown to be effective for this purpose in many studies.
Due to the high cost of the procedure, the need for hospitalization, general anesthesia and higher complications risks, ovarian drilling presents low cost effectiveness compared with gonadotropin plus timed intercourse. Moreover, the lack of standardization of the surgical technique and the absence of studies that have evaluated the repercussions of long-term of ovarian drilling demonstrate that this procedure should not be routinely performed but should only be considered as second line of therapy in women with PCOS who will be undergoing laparoscopy for another reason (adnexal mass or pelvic pain, for example). Additionally, ovarian drilling could be an alternative before the assisted reproduction treatment (ART) in individuals without financial conditions for the realization of ART and those who are resistant to CC.
Women with polycystic ovary syndrome (PCOS) may suffer from irregular periods, excessive hair growth (hirsutism) and acne (pimples). High levels of serum androgens (male hormone) are one of the main features of PCOS. There is no good evidence from this review that statins improve menstrual regularity, spontaneous ovulation rate, hirsutism or acne, either alone or in combination with the combined oral contraceptive pill. There is also no good evidence that statins have a beneficial effect on hirsutism or acne (pimples) associated with PCOS. In women with PCOS, statins are effective in reducing serum androgen levels and decreasing bad cholesterol (LDL), but statins are not effective in reducing fasting insulin or insulin resistance. There is no good evidence available on the long‐term use of statins (alone or in combination) for the management of PCOS.
This review synthesises the evidence for mechanisms of effect for herbal medicine in oligo/amenorrhoea, hyperandrogenism and PCOS. Laboratory, animal and clinical studies demonstrate that the herbal medicines Vitex agnus-castus, Cimicifuga racemosa and Tribulus terrestris initiate endocrine effects in the pituitary as measured by lowered prolactin and LH and raised FSH. Four herbal medicines, Tribulus terrestris, Glycyrrhiza spp., (alone and in combination with Paeonia lactiflora), Paeonia lactiflora (in combination with Cinnamomum cassia) and Cinnamomum cassia demonstrated morphological changes in polycystic ovaries and steroidogenesis, including reduced ovarian volume and cysts, lowered androgens, improved insulin sensitivity and increased oestradiol.

PCOS-related hormonal dysfunction can result in irregular or absent ovulation (anovulation). A variety of drugs can be used to treat this, enhancing the quality of both the egg (oocyte) and ovulation. Typical, first-line treatments include the fertility drugs Clomid (clomiphene citrate) and Femara (letrozole). While Clomid is commonly used to enhance ovulation, Femara may work better in women with PCOS as it neither raises estrogen levels nor increases the risk of multiple births to the same degree as Clomid.
Clinically speaking, the hyperandrogenism seen in PCOS is associated with hirsutism more than acne or alopecia and therefore hirsutism is an impetus for young women seeking care[2]. Obesity in adolescents is correlated with insulin resistance (IR) and dyslipidemia[4]. PCOS related ovulatory dysfunction in adolescents often correlates to adolescent obesity[5]. Genetic predisposition to PCOS has been suspected for many years[6] and data link obesity and metabolic disturbances in PCOS with genetic polymorphisms[7,8]. In contrast, another recent meta-analysis reviewed 26 studies that evaluated the use of letrozole in women with PCOS. The name ‘Polycystic Ovarian Syndrome’ points to the ovaries because it was long considered to be a reproductive issue. This review includes 18 preclinical laboratory based studies and 15 clinical trials. We found reproductive endocrine effects in oligo/amenorrhoea, hyperandrogenism and/or PCOS for six herbal medicines. For assisted reproduction cycles, metformin use prior to or during ovarian stimulation with gonadotropins in IVF/ICSI cycles is also not associated with better clinical pregnancy or live birth rates; however, metformin may reduce the risk of OHSS 38,39 and miscarriage and improve the implantation rate because metformin may act directly on the endometrium 39 and promote better reproductive outcomes (data not confirmed) in women with PCOS 40. So i went to My mom's obgyn and talked to him about what I read and my history of not getting my period on my own, he did an exam, ran some blood tests, and did other ultrasound.
