Vasoactive intestinal polypeptide (VIP) stimulates estradiol and progesterone release from ovarian granulosa cells in vitro. Very little information is available as to the role VIP plays in the control of steroid secretion during reproductive cyclicity and in ovarian pathologies involving altered steroid secretion. In this study, we determined the involvement of VIP in regulating ovarian androgen and estradiol release during estrous cyclicity and estradiol valerate (EV)-induced polycystic ovarian development in rats. Our findings show that androgen and estradiol release from ovaries obtained during different stages of rat estrous cycle mimic cyclic changes in steroid release observed in vivo with maximal release occurring during late proestrus. VIP increased androgen release from ovaries of all cycle stages except late proestrus and estradiol release from all cycle stages. Increases in VIP-induced androgen and estradiol release were maximal at early proestrus. Inclusion of saturating concentrations of androstenedione increased magnitude of VIP-induced estradiol release at diestrus and estrus but not proestrus. Magnitude of VIP-induced androgen and estradiol release tended to be greater in the ovaries from EV-treated rats with polycystic ovary compared with estrous controls. At the tissue level, ovarian VIP concentration was cycle stage dependent with highest level seen in diestrus. Maximum concentration of VIP was found in EV-treated rats. Changes in VIP were inversely related to changes in ovarian nerve growth factor, a neuropeptide involved in ovarian androgen secretion. These results strongly suggest that intraovarian VIP participates in the control of estradiol secretion during the rat estrous cycle and possibly in the maintenance of increased ovarian estradiol secretory activity of EV-treated rats.

Results of a second study by the same research team offer doctors a clear strategy to help you lose weight.5 According to this study at Penn State Health in Philadelphia, patients who worked with their physician to follow a limited fat, low calorie diet (~1200 calories) by relying on low-calorie prepared meals (eg, SmartOnes, Lean Cuisine or Healthy Choice) and an appetite suppressant with meals and making lifestyle changes plus added fruits and vegetables.4

New evidence suggests that using medications that lower insulin levels in the blood may be effective in restoring menstruation and reducing some of the health risks associated with PCOS. Lowering insulin levels also helps to reduce the production of testosterone, thus diminishing many of the symptoms associated with excess testosterone: hair growth on the body, alopecia (scalp hair loss), acne and possibly cardiovascular risk.


As many as 70% of PCOS women are insulin resistant and 10% have DM[20-22]. In PCOS women with normal glucose metabolism initially, the rate of conversion to abnormal glucose metabolism can be 25% over just three years[23]. More alarming, insulin abnormalities are highly prevalent in adolescents with PCOS[24]. Almost 20% of young Thai women with PCOS actually have DM[25]. Overall, normal glucose levels on an OGTT do not predict IR and IR, despite normal glucose levels, is correlated with CRP, dyslipidemia and other CAD risk factors[26]. Therefore, glucose levels alone lack the sensitivity to predict metabolic risk in PCOS patients. Precursor states of insulin abnormalities likely predict long term CAD risk well before glucose abnormalities. IR can be just as severe in diabetics and non-diabetics[27], stressing the seriousness of this metabolic impairment as a precursor and not a separate disease. Animal models have shown that IR alone damages myocardial cells, providing direct evidence of end organ disease[28]. Human data link HOMA-IR to left ventricular dysfunction[29]. Abnormal glucose metabolism short of IGT and DM still deserves attention, identification and treatment[30].

PCOS women demonstrate greater variation in insulin parameters compared to controls, independent of weight[33]. Animal studies of prenatal testosterone exposure show downstream IR in early postnatal life[34]. Some human data shows a high degree of correlation between hyperandrogenism and IR[35,36] and the relationship between hyperandrogenism and IR seem to differ between PCOS and non-PCOS women[35].
Patients with polycystic ovarian syndrome (PCOS) who have impaired glucose tolerance should start a comprehensive program of diet and exercise to reduce their risk of developing diabetes mellitus. While there are no specific treatments to stop a miscarriage, a woman's doctor may advise avoiding certain activities, bed rest, etc. If a woman believes she has had a miscarriage, she needs to seek prompt medical attention.

There is growing evidence that mood disturbances, mostly severe depression, are common in PCOS women , in whom impaired quality of life from body image concerns cause fatigue, sleep disturbance and changes in eating habits. In addition, many PCOS patients report feeling abnormal, unfeminine, and embarrassed due to unwanted hair, often hiding their hair growth and covering their face when talking to others. Understanding how a woman feels about her body image and improving this perception are essential components of any management plan that provides overall health care to women with PCOS.
The routine use of OGTT is advocated by some in all PCOS women[15]. In teenagers, abnormalities in glucose metabolism manifest prior to dyslipidemia, suggesting that assessment of glucose metabolism is even more important in younger women[16]. DM is diagnosed by an 8 h fasting plasma glucose ≥ 126 mg/dL, 2 h glucose value ≥ 200 mg/dL after oral glucose tolerance test (OGTT) or random glucose ≥ 200 mg/dL with symptoms of DM confirmed by either fasting plasma glucose or OGTT. Hemoglobin AIC > 6.5% may also be issued to diagnose DM[17]. Impaired glucose tolerance (IGT) is defined by a 2 h cutoff of 140-200 mg/dL on OGTT[18].The prevalence of IGT in obese adolescents is surprisingly as high as 15%[19].

Given the conditions associated with PCOS, the Endocrine Society, the Androgen Excess and PCOS Society, and the American College of Obstetricians and Gynecologists recommend that clinicians evaluate patients' blood pressure at every visit and lipid levels at the time of diagnosis, and screen for type 2 diabetes with a two-hour oral glucose tolerance test regardless of a patient's body mass index. Patients should have repeat diabetes screening every three to five years, or more often if other indications for screening are present.19–21 The Endocrine Society further recommends depression screening, as well as screening for symptoms of obstructive sleep apnea in overweight and obese patients with PCOS.19 However, routine screening for nonalcoholic fatty liver disease or endometrial cancer (using ultrasonography) is not recommended.19

The risk of developing prediabetes and type 2 diabetes is increased in women with PCOS, particularly if they have a family history of diabetes. The second-line pharmacological treatment of infertility in anovulatory women with PCOS includes the use of gonadotropins [recombinant follicle-stimulating hormone (FSHr) or human menopausal gonadotropin (HMG)] for timed intercourse or intrauterine insemination (IUI) 9. Due to the higher cost of this therapeutic modality, an evaluation of the tubal patency is recommended prior to initiating the ovarian stimulation with gonadotropins if this procedure was not performed prior to initiating CC treatment. If the fallopian tube is opened and the sperm concentration is suitable for in vivo fertilization, the ovarian stimulation begins with low doses of gonadotropins (37.5 to 75 IU/day or every other day) to achieve monofollicular growth and reduce the risk of complications (OHSS and multiple gestation) 25. US monitoring of the follicular growth (follicular diameter measurement) is mandatory in this case and the endogenous secretion of gonadotropins does not need to be inhibited with gonadotropin-releasing hormone analogues (GnRH-a) during the timed intercourse cycles. The administration of hCG (used to simulate the endogenous peak of luteinizing hormone for final oocyte maturation and ovulation triggering) is unnecessary because it does not increase the probability of conception during ovulation induction cycles for timed intercourse 21. It is important to note that if gonadotropin is chosen as the treatment option, the IUI has a higher likelihood of successful pregnancy compared with timed intercourse in patients with subfertility 26.
Just fyi, when a cat stops grooming itself, it's a sign that it's about to die. Or at least something is seriously wrong, physically or psychologically. Elderly cats typically stop grooming themselves when they're about to die. I'm a vet tech, and have seen cats in their early 20's that still groom themselves. They may need a bit of help with their back ends due to arthritis, but a well cared for cat of any age will at least try.

Lifestyle measures to achieve a weight loss of 5%-10% in overweight women can help regulate ovulation and periods. Although the basic approach of nutrition is needed, it can be more challenging to lose weight and maintain weight loss with PCOS. Dr. Dunaif from Northwestern is very encouraging that even “a little bit of weight reduction and exercise can improve insulin sensitivity.”

i would suggest researching a bit online. but from my experience working in treatment centers yes, meth has foul chemicals. when they first come in and are detoxing from it addicts smell like chemical and ammonia. there are so many foul and unnatural things in it it reeks havoc on the human body. i know she mentioned crack but i don't think we have any real proof luna has ever used meth.

Evidence suggests that metformin frequently, but not universally, improves ovulation rates and pregnancy rates in women with polycystic ovarian syndrome (PCOS), especially in obese women. [2, 3, 70] In addition, pretreatment with metformin has been shown to enhance the efficacy of clomiphene for inducing ovulation. [71] Consider the combination of metformin and clomiphene in older women with visceral obesity and clomiphene resistance. [2] However, this combination doesn’t significantly improve the live birth rate relative to clomiphene monotherapy. [2] Whether short-course metformin pretreatment (less than 4 weeks) is as effective as conventional long-course metformin remains uncertain. [5, 72]

PCOS is the most common hormone disorder in women and one of the leading causes of infertility, affecting roughly 5-10% of women of childbearing age. The condition can affect girls as young as 11. Because the signs and symptoms often seem unrelated and the diagnostic criteria are not yet clear, most women are not diagnosed until they are in their 20's or 30's. It appears to run in families. Women with sisters who have PCOS are twice as likely to have the condition.

Two important things to note are that the long term safety effects of use are not known (but the drug has been in use for decades already) and spironolactone IS NOT SAFE in pregnancy, so it is commonly used in conjunction with birth control pills. This use of birth control pills also protects against abnormal growth of the lining of the uterus. Some women cannot take birth control pills, in which case it is important to use other means of birth control if you are sexually active while taking spironolactone.

The mechanism of action of ovarian drilling in the treatment of infertility in women with PCOS is suggested to be based on the decreased secretion of androgens and consequent reduction of peripheral aromatization of these compounds into estrone. Furthermore, the follicular microenvironment becomes more estrogenic, which facilitates follicular growth 30. Regarding the efficacy of ovarian drilling, observational studies demonstrated that the ovulation rate was between 54 and 76% in the 6 months after the procedure and 33 and 88% in the 12 months after the procedure. During these periods, the spontaneous pregnancy rate ranged between 28 and 56% and 54 and 70%, respectively 31.

Lifestyle interventions are usually required for long term sustainable results. PCOS women who smoke have higher free androgen levels and IR as measured by HOMA-IR, QUICKI and the insulin sensitivity index following 75 g OGTT[104]. Thus PCOS women who smoke have an additional reason to stop smoking. In more general population studies (non-PCOS) comprised mostly of middle-aged women, lifestyle intervention is more effective than metformin in preventing the progression to DM. Dietary and exercise intervention decreased the 4 year progression to DM in patients at risk (non-diabetic, elevated fasting and/or OGTT glucose) by almost 50%[105]. Realizing the limitations of applying this population sample to young PCOS women, it still highlights the benefit of non-pharmacological treatment. PCOS women randomized to both metformin and lifestyle interventions (compared to placebo) showed improvements in HOMAIR after 4 mo[106]. In European adolescents with PCOS who failed to achieve improvements in HOMA-IR after 6 mo of lifestyle intervention, both metformin and placebo reduced IR over 6 mo, although metformin offered no benefit over placebo[107]. Lifestyle modification in adolescents has been successful in reducing hyperandrogenism[103]. Modest weight loss of about 5% bodyweight has also been shown to lower hyperandrogenism[108] which may ultimately improve IR.
