Polycystic ovary syndrome (PCOS) is a complex condition that is most often diagnosed by the presence of two of the three following criteria: hyperandrogenism, ovulatory dysfunction, and polycystic ovaries. Because these findings may have multiple causes other than PCOS, a careful, targeted history and physical examination are required to ensure appropriate diagnosis and treatment. This article provides an algorithmic approach to the care of patients with suspected or known PCOS.
If he was using heroin very heavily, that may well be his starting methadone dose (or close to it.) Methadone has a long half life while heroin has a short one, so avoiding withdrawals can take a lot of methadone, initially. Methadone clinics aren't going to leave someone on a high dose though. They'll titrate up until withdrawal subsides, then taper down over time. I'm guessing he started at the clinic around when he got busted, so it's sus af to me that he'd still be taking 130mg. Or that he ever was. Maybe 100. Possibly.

#### For acne or excess hair growth, a water pill (diuretic) called spironolactone (Aldactone) may be prescribed to help reverse these problems. The use of spironolactone requires occasional monitoring of blood tests because of its potential effect on the blood potassium levels and kidney function. Eflornithine (Vaniqa) is a prescription cream that can be used to slow facial hair growth in women.Electrolysis and over-the-counter depilatory creams are other options for controlling excess hair growth.

Maybe anons theory of them using Roger's potential ssi funds for rent isn't to far off? My grandpa used to get like 1200 a month from ssi but it's probably gone up since then and if they get some sort of housing assistance usually they would only have to pay 1/3 of their income. Which would leave plenty to live off of comfortably for the month unless you are using your funds for drugs or useless amazon crap like tuna buys
The prevalence of insulin resistance in women with PCOS, as measured by impaired glucose tolerance, is substantially higher than expected compared with age-and weight-matched populations of women without PCOS.45 Although insulin resistance alone is a laboratory (not clinical) aberration, it can lead to diabetes, and it may be associated with the metabolic syndrome, thus leading to increased cardiovascular risk.2 As with diabetes, optimal treatment of PCOS requires lifestyle modifications (e.g., diet, exercise) in addition to appropriate medications.
Why treat IR in PCOS women? For many years only PCOS women with DM were treated. As the link between IGT and CAD became more apparent, many PCOS women with IGT were treated. We now understand that IR is often the first step in a progression to DM and CAD. Those who now advocate treatment for IR do so for the following reasons: reduction of insulin and androgen levels, prevention of IGT and DM, potential for improved ovulation, symptomatic improvement, prevention of MS[63]. Ultimately, secondary prevention in young women with identifiable and treatment precursor conditions is far more desirable and easier than treatment of these same women later in life with serious disease.
This is A LOT of benzodiazepines. I remember the last batch she got too, and it wasn't that long ago. She's using more than I am, and I'm using them legally for a partial spinal cord injury. The FUCK, Luna. Being dependant on heroin isn't enough for you or something? This bothers me more than the heroin, honestly. The withdrawal from benzos can actually kill you, unlike heroin. Not to mention that using multiple CNS depressants together is dangerous af. Narcan won't bring you back from that shit.
Vasoactive intestinal polypeptide (VIP) stimulates estradiol and progesterone release from ovarian granulosa cells in vitro. Very little information is available as to the role VIP plays in the control of steroid secretion during reproductive cyclicity and in ovarian pathologies involving altered steroid secretion. In this study, we determined the involvement of VIP in regulating ovarian androgen and estradiol release during estrous cyclicity and estradiol valerate (EV)-induced polycystic ovarian development in rats. Our findings show that androgen and estradiol release from ovaries obtained during different stages of rat estrous cycle mimic cyclic changes in steroid release observed in vivo with maximal release occurring during late proestrus. VIP increased androgen release from ovaries of all cycle stages except late proestrus and estradiol release from all cycle stages. Increases in VIP-induced androgen and estradiol release were maximal at early proestrus. Inclusion of saturating concentrations of androstenedione increased magnitude of VIP-induced estradiol release at diestrus and estrus but not proestrus. Magnitude of VIP-induced androgen and estradiol release tended to be greater in the ovaries from EV-treated rats with polycystic ovary compared with estrous controls. At the tissue level, ovarian VIP concentration was cycle stage dependent with highest level seen in diestrus. Maximum concentration of VIP was found in EV-treated rats. Changes in VIP were inversely related to changes in ovarian nerve growth factor, a neuropeptide involved in ovarian androgen secretion. These results strongly suggest that intraovarian VIP participates in the control of estradiol secretion during the rat estrous cycle and possibly in the maintenance of increased ovarian estradiol secretory activity of EV-treated rats.
High cholesterol and triglyceride levels increase the risk of cardiovascular disease. Getting your cholesterol and triglyceride levels in an optimal range will help protect your heart and blood vessels. Cholesterol management may include lifestyle interventions (diet and exercise) as well as medications to get your total cholesterol, LDL, HDL, and triglycerides in an optimal range.

Evidence suggests that metformin frequently, but not universally, improves ovulation rates and pregnancy rates in women with polycystic ovarian syndrome (PCOS), especially in obese women. [2, 3, 70] In addition, pretreatment with metformin has been shown to enhance the efficacy of clomiphene for inducing ovulation. [71] Consider the combination of metformin and clomiphene in older women with visceral obesity and clomiphene resistance. [2] However, this combination doesn’t significantly improve the live birth rate relative to clomiphene monotherapy. [2] Whether short-course metformin pretreatment (less than 4 weeks) is as effective as conventional long-course metformin remains uncertain. [5, 72]
A study supervised by Columbia University School of Nursing professor Nancy Reame, MSN, PhD, FAAN, and published in the Journal of Behavioral Health Services & Research, identifies the PCOS complications that may be most responsible for psychiatric problems. While weight gain and unwanted body hair can be distressing, irregular menstrual cycles is the symptom of PCOS most strongly associated with psychiatric problems, the study found.

## Alcohol will only rot your liver when taken to excess. Most people can drink without becoming addicted, while it's highly unusual for people to try heroin 1 or 2 times and not get addicted. The average person isn't going to lie, scam, steal, destroy their family and relationships, and prostitute themselves for alcohol, but heroin often turns people into thieving criminals. Not to say that alcohol isn't terrible as well, but the effect it has on an individual seems lesser than that of heroin.

This high potency blend of botanicals and antioxidants focuses on supporting the vessels of the cardiovascular system as well as cellular vitality, that can be compromised in women with PCOS and insulin resistance.* Over time, the interior lining of blood vessels are damaged when insulin levels are elevated and this damage contributes to the formation of Cardiovascular Disease. Elevated insulin also causes free radical damage and vessel injuries which are important factors in the progression of PCOS and Insulin Resistance. RejuvaPlus contains high quality antioxidants designed to support cardiovascular vessel integrity, minimize free-radical damage. An additional benefit is supporting immune function.
PCOS is associated with multiple metabolic defects, including metabolic syndrome. Twice as many women with PCOS have metabolic syndrome as in the general population, and about one-half of women with PCOS are obese.1,9 The presence of PCOS is also associated with a fourfold increase in the risk of type 2 diabetes mellitus.10 There is an increased prevalence of nonalcoholic fatty liver disease,11,12 sleep apnea,13 and dyslipidemia14 in patients with PCOS, even when controlled for body mass index. Rates of cardiovascular disease are higher in patients with PCOS, but increased cardiovascular mortality has not been consistently demonstrated.15,16 Finally, there is evidence to suggest an increased risk of mood disorders among patients with PCOS.17,18
##### Lifestyle interventions are usually required for long term sustainable results. PCOS women who smoke have higher free androgen levels and IR as measured by HOMA-IR, QUICKI and the insulin sensitivity index following 75 g OGTT[104]. Thus PCOS women who smoke have an additional reason to stop smoking. In more general population studies (non-PCOS) comprised mostly of middle-aged women, lifestyle intervention is more effective than metformin in preventing the progression to DM. Dietary and exercise intervention decreased the 4 year progression to DM in patients at risk (non-diabetic, elevated fasting and/or OGTT glucose) by almost 50%[105]. Realizing the limitations of applying this population sample to young PCOS women, it still highlights the benefit of non-pharmacological treatment. PCOS women randomized to both metformin and lifestyle interventions (compared to placebo) showed improvements in HOMAIR after 4 mo[106]. In European adolescents with PCOS who failed to achieve improvements in HOMA-IR after 6 mo of lifestyle intervention, both metformin and placebo reduced IR over 6 mo, although metformin offered no benefit over placebo[107]. Lifestyle modification in adolescents has been successful in reducing hyperandrogenism[103]. Modest weight loss of about 5% bodyweight has also been shown to lower hyperandrogenism[108] which may ultimately improve IR.

Wang et al. 2008 [66] Double blinded, placebo controlled randomised trial (pilot). Eight weeks. 15 overweight women with oligo/amenorrhoea and polycystic ovaries on ultrasound. Mean body mass index 28.8 ± 1.3 kg/m2. Mean age 31.1 ± 2.0 years Cinnamomum cassia extract 333 mg (Integrity Nutraceuticals International Sarasota, Florida) or placebo. One tablet three times per day. Primary outcomes: Insulin resistance and sensitivity. Secondary outcomes oestradiol and testosterone concentration. Body mass index (BMI). Before and after treatment comparisons between randomised groups plus comparison between treatment group and normal ovulatory, normal weight women. Adverse events. Improved insulin sensitivity (QUICKI) in the treatment group. 0.35 to 0.38, (7.7%) p < 0.03. Insulin resistance (HOMO-IR) significantly reduced in treatment group 2.57 to 1.43 (44.5%) p < 0.03. Controls no change insulin sensitivity or insulin resistance. No change in either group for BMI, testosterone and oestradiol. Differences between Cinnamomum cassia group and normal weight and ovulatory controls were not significant. (P < 0.17). No reported adverse reactions. Small pilot study, the authors report that larger studies are required to confirm findings. Small sample size may explain non-significant comparison with normal weight and ovulating women. Reproductive outcomes were unchanged in this study however the duration of the study was insufficient to demonstrate reproductive changes.
Teens with PCOS may have slightly different signs and symptoms. Irregular cycles are common in normal girls in the first years after periods start (menarche). Therefore, signs of ovulation problems for girls include irregular periods more than 2 years after menarche, absence of cycles for more than 3 months, or never having a first period by the time puberty is completed. Also, heavy or overly frequent periods may be a sign of problems related to PCOS. Adolescents do not require an ultrasound for the evaluation of PCOS, since large ovaries with many follicles develop as part of normal puberty.

The principle infertility treatment initially includes preconception guidelines and the use of drugs to induce mono- or bifollicular ovulation. Other therapeutic modalities may also be employed, such as exogenous gonadotropins or laparoscopic ovarian drilling, which are considered to be second-line treatments, or in vitro fertilization (IVF), which is a third-line treatment 9. Thus, the choice of the most appropriate treatment depends on the patient's age, presence of other factors associated with infertility, experience and duration of previous treatments and the level of anxiety of the couple.

Regardless of what reasons women have for seeking diagnosis and treatment of PCOS, it is imperative for practitioners to assess a woman’s risk for CAD. Assessment should probably be made in all PCOS patients regardless of BMI. Especially in young women or adolescents, IR may be the first identifiable risk factor. Practitioners must recognize that no universal test for IR exists and must use good clinical judgment to assess metabolic status in women. Stimulated testing with OGTT may be more sensitive than fasting measurements. Women who demonstrate IR should be counseled on lifestyle modifications. Physicians should discuss with their patients a target BMI that is realistically obtainable. It is often advisable for patients to seek nutritional assessment and counseling to help with this goal. In many individuals, consideration should be given to pharmacological treatment. Although the most commonly used medication is metformin, other medications may be appropriate first line therapy, especially in women not actively seeking pregnancy.
Certain lifestyle changes, such as diet and exercise, are considered first-line treatment for adolescent girls and women with polycystic ovarian syndrome (PCOS). [41] Pharmacologic treatments are reserved for so-called metabolic derangements, such as anovulation, hirsutism, and menstrual irregularities. Medications for such conditions include oral contraceptives, metformin, prednisone, leuprolide, clomiphene, and spironolactone.
To be quite precise (Gastroenterofag here): opioids do slow the metabolism and make you crave sugary crap. Weight gain versus loss depends on whether the dose is high enough to actually be too fucked up most of the day to eat (that's why virtually all thin heroin users who go on methadone or subutex end up ballooning: they're no longer taking astronomical doses of opioids that make them too zonked out to eat, as the cross tolerance allows us to give them about 40% less of the equianalgesic dose). Sage for medfagging.
I worded that badly. What I mean is that opiates themselves don't relieve pain, but alter the perception of pain. It's possible to alter that perception of pain in other ways, which is why substances like SSRIs cause significant pain relief for some people with some conditions. Ditto with anticonvulsants. Opioids aren't the only drugs that affect norepinephrine and mU receptors, they just happen to be the best at it. If the pleasurable effects of opioids were blocked, there's still therapeutic potential in the "inert" components, like THC vs. CBD.

Contrary to the implication of “polycystic,” some women with the condition don’t have any cysts. A diagnosis requires only two of the following three criteria to be met: elevated levels of male sex hormones (which can cause excess hair growth, acne, and baldness), irregular or absent periods, and/or at least 12 follicular cysts on one or both ovaries.
Oh FUCK no. Someone please let the woman know; it's a matter of saving an animal from a lifetime of misery and despair. Siamese cats are much harder to properly keep than the average housecat and needs a household where they'll be stimulated and properly trained so they don't get aggressive. There's no way two lazy dirty junkies could take care of one.

Cinnamon cassia One animal study compared the effectiveness of Cinnamomum cassia with metformin against controls in rats with PCOS. Hormone concentration was measured at 15 and 30 days [48] One pilot RCT demonstrated positive effects for metabolic parameter’s (HOMO and QUICKI) for Cinnamomum cassia in overweight women with PCOS [66] 1. Equivalence for metformin for reduced testosterone in PCOS [48]

Some have tried to utilize ultrasound to detect IR. Of note, normoglycemic women often have the phenotypic criteria for polycystic ovaries on ultrasound[54], consistent with other data in young adolescents showing that polycystic ovaries by ultrasound appearance often does not correlate with either anovulatory menstrual cycles or metabolic abnormalities[55]. Therefore ultrasound is too non-specific to use with any reliability in measuring IR.
A neck lump or nodule is the most common symptom of thyroid cancer. You may feel a lump, notice one side of your neck appears to be different, or your doctor may find it during a routine examination. If the tumor is large, it may cause neck or facial pain, shortness of breath, difficulty swallowing, cough unrelated to a cold, hoarseness or voice change.
MRI (or magnetic resonance imaging) scan is a radiology technique which uses magnetism, radio waves, and a computer to produce images of body structures. MRI scanning is painless and does not involve X-ray radiation. Patients with heart pacemakers, metal implants, or metal chips or clips in or around the eyes cannot be scanned with MRI because of the effect of the magnet.

#### But mostly if food requires more preparation than sticking a spoon into ice cream containers or tearing open a bag of chips, Luna can't prepare it. So I can see her using rice she could use in a meal because she is starving every other day because ain't no way is that girl gonna cook rice. She'd probably nod off in the middle and burn down the house so it is probably a net good she's about to give Pat a toe-cheesy sock full of possibly contaminated rice.

It's trendy to be ~body posi~ and have massive self confidence even if you're conventionally ugly right now. Especially if you're not conventionally attractive, actually. It's the in thing with the group she wants to be part of. I don't see how you guys don't get that she's working that angle hard. She wants to be seen a martyr and a hero for defying unconventional beauty standards and seeing worth in herself when society doesn't.

Polycystic Ovarian Syndrome is a condition that affects a woman's hormonal levels. Women with PCOS produce higher than normal male hormones which cause them to skip their monthly periods and make it harder for them to get pregnant. PCOS also causes hair growth on the face and body, and baldness. So basically, women with PCOS have dense hair growth on those areas of their bodies where they're not supposed to have excess hair. But they start losing hair from their scalps, leading to baldness. PCOS can contribute to long-term health problems like diabetes and heart disorders.

Although there isn’t a single cure for PCOS, lifestyle and nutritional changes can significantly improve the hormonal imbalances that are so prevalent in PCOS. Weight loss and exercise will reduce the levels of insulin and targeted nutritional supplements can help balance out the systemic biochemistry. A healthy lifestyle is the best medicine and through a comprehensive and consistent improvement in diet and activity, PCOS symptoms can be effectively managed or diminished, and some can be made to disappear completely.
Women with polycystic ovary syndrome (PCOS) may suffer from irregular periods, excessive hair growth (hirsutism) and acne (pimples). High levels of serum androgens (male hormone) are one of the main features of PCOS. There is no good evidence from this review that statins improve menstrual regularity, spontaneous ovulation rate, hirsutism or acne, either alone or in combination with the combined oral contraceptive pill. There is also no good evidence that statins have a beneficial effect on hirsutism or acne (pimples) associated with PCOS. In women with PCOS, statins are effective in reducing serum androgen levels and decreasing bad cholesterol (LDL), but statins are not effective in reducing fasting insulin or insulin resistance. There is no good evidence available on the long‐term use of statins (alone or in combination) for the management of PCOS.
I don't mean to break any hearts, but if something happens to Lurch, the chances of Tuna fixing herself are still near zero. She's addicted to the life, they fight plenty and she has made mention before of hooking customers up, etc. It's very rare for a junkie couple to have one person lifting all the weight. On the off chance he does go away, she'll latch on to someone else and get back into the same role (holding / palming / whatever)
Lack of ovulation (monthly release of an egg from the ovary) is the predominant cause for infertility in PCOS women, although some individuals also may have a higher rate of miscarriage. In some PCOS women who are overweight, diet combined with exercise can restore ovulation without the use of medications. Otherwise, several medications are available to restore ovulation in PCOS women. Oral clomiphene citrate stimulates FSH secretion from a woman’s own pituitary (located at the base of the skull) to induce ovarian follicle growth. Subcutaneous injections of gonadotropins with FSH and LH can induce the same events, if necessary. Both medications increase the chance of multiple births, although FSH injections are the most powerful. Other medications include metformin, an insulin sensitizer that slowly lowers insulin and androgen levels to gradually induce ovulation with a lower chance of multiple births. Medications that block estrogen production to stimulate FSH secretion (letrozol) also exist, but additional studies are needed before they can be routinely recommended for ovulation induction. Since most PCOS women ovulate following medical therapy, surgery or in-vitro fertilization (IVF) are generally reserved for PCOS women who fail to ovulate with medication or have other fertility problems.
The diagnostic workup should begin with a thorough history and physical examination. Clinicians should focus on the patient's menstrual history, any fluctuations in the patient's weight and their impact on PCOS symptoms, and cutaneous findings (e.g., terminal hair, acne, alopecia, acanthosis nigricans, skin tags).19 Patients should also be asked about factors related to common comorbidities of PCOS.

Anecdotally, oral contraceptives are among the most common agents used to treat menstrual irregularities in women with PCOS. However, there are few studies examining their effect on menstrual cycles in women with PCOS. Cyproterone acetate plus ethinyl estradiol has been extensively studied, but it is not available in the United States. Studies suggest that the following agents may improve menstrual irregularities (e.g., oligomenorrhea): spironolactone (in an open-label study),22 acarbose,24 rosigli-tazone,32 and metformin.10,11,32,47,51 Metformin is probably the best choice because it may improve insulin resistance in addition to improving menstrual irregularities.

Because of its antiandrogenic effects, spironolactone is effective, but not FDA-approved, for this indication.22,23 A Cochrane review suggested that spironolactone is superior to finasteride.28 Combining spironolactone with oral contraceptives may be synergistic, but caution should be used in women taking drospirenone because each agent can cause hyperkalemia.2 Spironolactone is FDA pregnancy category C.

Clinical equivalence for prolactin lowering effects of Vitex agnus-castus (Agnucaston® 40 mg per day) and the pharmaceutical Bromocriptine (Parlodel® 5 mg per day) was found in one study including 40 women with hyperprolactinaemia [63]. Mean concentrations for prolactin following three months treatment with Vitex agnus-castus was significantly reduced from 946 mIU/l (±173) to 529 mIU/l (±297) (p < 0.001). Comparatively, mean prolactin concentration in the Bromocriptine group was significantly reduced from 885 mIU/l (±178) to 473 mIU/l (±266) (p < 0.001) demonstrating that both treatments were effective treatment for women with hyperprolactinaemia (normal reference range 25-628 mIU/l). The mean difference in prolactin reduction of the two groups was not significant (p = 0.96) (Table 2).
3. Serum hormones during follicular phase oestradiol, LH and FSH. Luteal progesterone measured day 21–23 of the cycle. Serum LH was 8.0 (±0.9) in the clomiphene group and 5.7 (±0.9) in the clomiphene plus Cimicifuga racemosa group (p < 0.001) and oestradiol was 228.3 (±30.2) in the clomiphene alone group and 299.5 (±38.9) \in the clomiphene plus Cimicifuga racemosa group (p = 0.01)
Pre-clinical and clinical evidence was found for Vitex agnus-castus for lowered prolactin, improved menstrual regularity and treatment of infertility. Vitex agnus-castus contains a variety of compounds which bind to dopamine type 2 (DA-2) receptors in the brain; reduce cyclic adenosine mono phosphate (cAMP) and lowered prolactin secretion (Table 1). This was demonstrated in studies using recombinant DA-2 receptor proteins, and basal and stimulated rat pituitary cell cultures [38–41]. Prolactin lowering effects were found in normal and ovariectomised rats [49]. Additional agonistic opiate effects were observed in studies using human opiate receptors cell cultures [70].

### Acne: Birth-control pills, anti-androgen drugs and insulin-sensitizing drugs, all mentioned above, can bring the severe acne of PCOS under control by reducing the high levels of male hormones that trigger bad break-outs in PCOS. In addition, your family doctor or dermatologist may recommend additional acne medications to unclog pores, control skin bacteria and soothe inflammation. These may include retinoids, antibiotics, and products to help unclog pores. One warning: Retinoids can cause birth defects and cannot be used if you are already pregnant or are planning to become pregnant.

The definitive cause of PCOS is unknown, but researchers have found a strong link to insulin resistance, a genetic condition often associated with diabetes, in which the muscle, fat, and liver cells do not respond properly to insulin and thus cannot easily absorb glucose (sugar) from the bloodstream. As a result, the body produces higher and higher levels of insulin to help glucose enter the cells.
she's only playing dress up as a heroin addict because she knows she always has a safety net. those are the kids that go the hardest, the ones that are so privileged they think nothing really bad is ever going to happen to them. but she's already ruined her appearance and job opportunities, so i don't know where this weird superiority complex for being "poor" comes from? even if her daddy does save her she can't just get a top notch face lift like courtney love. no one cares. she's already so fucked and doesn't know it.

Just fyi, when a cat stops grooming itself, it's a sign that it's about to die. Or at least something is seriously wrong, physically or psychologically. Elderly cats typically stop grooming themselves when they're about to die. I'm a vet tech, and have seen cats in their early 20's that still groom themselves. They may need a bit of help with their back ends due to arthritis, but a well cared for cat of any age will at least try.
This is a really big deal, because GnRH antagonist drugs are pretty common and used to treat some cancer. They are also part of the treatment for women with PCOS who are stimulating their ovaries for IVF and egg-freezing. This study suggests that we might be able to stop PCOS before it develops by treating pregnant women with this drug, and we can potentially use it to treat women who currently have PCOS. It is important to note that there are two commonly recognized types of PCOS, a lean phenotype and an obese phenotype. The authors explain that their mouse model most closely resemble the lean PCOS phenotype, so this treatment might not be able to help all women with PCOS. However, it is still an important step forward.

Women with polycystic ovary syndrome have menstrual disorders caused by the absence of ovulation. About 20% of women will not ovulate on clomiphene citrate, the primary treatment option. These women can be treated with a surgical procedure like laparoscopic electrocautery of the ovaries or by ovulation induction with gonadotrophins or gonadotrophin releasing hormone (GnRH). In normal menstrual cycles, GnRH is released in a regular pulsatile interval. A portable pump can be used to mimic this pulse to help these women to ovulate and hopefully to get pregnant. The review of trials did not find enough evidence to show the effectiveness of pulsatile GnRH in women with polycystic ovary syndrome.
