Vasoactive intestinal polypeptide (VIP) stimulates estradiol and progesterone release from ovarian granulosa cells in vitro. Very little information is available as to the role VIP plays in the control of steroid secretion during reproductive cyclicity and in ovarian pathologies involving altered steroid secretion. In this study, we determined the involvement of VIP in regulating ovarian androgen and estradiol release during estrous cyclicity and estradiol valerate (EV)-induced polycystic ovarian development in rats. Our findings show that androgen and estradiol release from ovaries obtained during different stages of rat estrous cycle mimic cyclic changes in steroid release observed in vivo with maximal release occurring during late proestrus. VIP increased androgen release from ovaries of all cycle stages except late proestrus and estradiol release from all cycle stages. Increases in VIP-induced androgen and estradiol release were maximal at early proestrus. Inclusion of saturating concentrations of androstenedione increased magnitude of VIP-induced estradiol release at diestrus and estrus but not proestrus. Magnitude of VIP-induced androgen and estradiol release tended to be greater in the ovaries from EV-treated rats with polycystic ovary compared with estrous controls. At the tissue level, ovarian VIP concentration was cycle stage dependent with highest level seen in diestrus. Maximum concentration of VIP was found in EV-treated rats. Changes in VIP were inversely related to changes in ovarian nerve growth factor, a neuropeptide involved in ovarian androgen secretion. These results strongly suggest that intraovarian VIP participates in the control of estradiol secretion during the rat estrous cycle and possibly in the maintenance of increased ovarian estradiol secretory activity of EV-treated rats.

PCOS is associated with multiple metabolic defects, including metabolic syndrome. Lifestyle interventions are usually required for long term sustainable results. PCOS women who smoke have higher free androgen levels and IR as measured by HOMA-IR, QUICKI and the insulin sensitivity index following 75 g OGTT[104]. Thus PCOS women who smoke have an additional reason to stop smoking. In more general population studies (non-PCOS) comprised mostly of middle-aged women, lifestyle intervention is more effective than metformin in preventing the progression to DM. Dietary and exercise intervention decreased the 4 year progression to DM in patients at risk (non-diabetic, elevated fasting and/or OGTT glucose) by almost 50%[105]. Realizing the limitations of applying this population sample to young PCOS women, it still highlights the benefit of non-pharmacological treatment. PCOS women randomized to both metformin and lifestyle interventions (compared to placebo) showed improvements in HOMAIR after 4 mo[106]. In European adolescents with PCOS who failed to achieve improvements in HOMA-IR after 6 mo of lifestyle intervention, both metformin and placebo reduced IR over 6 mo, although metformin offered no benefit over placebo[107]. Lifestyle modification in adolescents has been successful in reducing hyperandrogenism[103]. Modest weight loss of about 5% bodyweight has also been shown to lower hyperandrogenism[108] which may ultimately improve IR.

Differences between Cinnamomum cassia group and normal weight and ovulatory controls were not significant. (P < 0.17). No reported adverse reactions. Small pilot study, the authors report that larger studies are required to confirm findings. Small sample size may explain non-significant comparison with normal weight and ovulating women. Reproductive outcomes were unchanged in this study however the duration of the study was insufficient to demonstrate reproductive changes.

Regardless of what reasons women have for seeking diagnosis and treatment of PCOS, it is imperative for practitioners to assess a woman’s risk for CAD. Assessment should probably be made in all PCOS patients regardless of BMI. Especially in young women or adolescents, IR may be the first identifiable risk factor. There is no good evidence from this review that statins improve menstrual regularity, spontaneous ovulation rate, hirsutism or acne, either alone or in combination with the combined oral contraceptive pill. There is also no good evidence that statins have a beneficial effect on hirsutism or acne (pimples) associated with PCOS. In women with PCOS, statins are effective in reducing serum androgen levels and decreasing bad cholesterol (LDL), but statins are not effective in reducing fasting insulin or insulin resistance. There is no good evidence available on the long‐term use of statins (alone or in combination) for the management of PCOS.

Lack of ovulation (monthly release of an egg from the ovary) is the predominant cause for infertility in PCOS women, although some individuals also may have a higher rate of miscarriage. In some PCOS women who are overweight, diet combined with exercise can restore ovulation without the use of medications. Otherwise, several medications are available to restore ovulation in PCOS women. Oral clomiphene citrate stimulates FSH secretion from a woman’s own pituitary (located at the base of the skull) to induce ovarian follicle growth. Subcutaneous injections of gonadotropins with FSH and LH can induce the same events, if necessary. Both medications increase the chance of multiple births, although FSH injections are the most powerful. Other medications include metformin, an insulin sensitizer that slowly lowers insulin and androgen levels to gradually induce ovulation with a lower chance of multiple births. Medications that block estrogen production to stimulate FSH secretion (letrozol) also exist, but additional studies are needed before they can be routinely recommended for ovulation induction. Since most PCOS women ovulate following medical therapy, surgery or in-vitro fertilization (IVF) are generally reserved for PCOS women who fail to ovulate with medication or have other fertility problems.
The diagnostic workup should begin with a thorough history and physical examination. Clinicians should focus on the patient's menstrual history, any fluctuations in the patient's weight and their impact on PCOS symptoms, and cutaneous findings (e.g., terminal hair, acne, alopecia, acanthosis nigricans, skin tags).19 Patients should also be asked about factors related to common comorbidities of PCOS.

Anecdotally, oral contraceptives are among the most common agents used to treat menstrual irregularities in women with PCOS. However, there are few studies examining their effect on menstrual cycles in women with PCOS. Cyproterone acetate plus ethinyl estradiol has been extensively studied, but it is not available in the United States. Studies suggest that the following agents may improve menstrual irregularities (e.g., oligomenorrhea): spironolactone (in an open-label study),22 acarbose,24 rosigli-tazone,32 and metformin.10,11,32,47,51 Metformin is probably the best choice because it may improve insulin resistance in addition to improving menstrual irregularities.

Because of its antiandrogenic effects, spironolactone is effective, but not FDA-approved, for this indication.22,23 A Cochrane review suggested that spironolactone is superior to finasteride.28 Combining spironolactone with oral contraceptives may be synergistic, but caution should be used in women taking drospirenone because each agent can cause hyperkalemia.2 Spironolactone is FDA pregnancy category C.

Clinical equivalence for prolactin lowering effects of Vitex agnus-castus (Agnucaston® 40 mg per day) and the pharmaceutical Bromocriptine (Parlodel® 5 mg per day) was found in one study including 40 women with hyperprolactinaemia [63]. Mean concentrations for prolactin following three months treatment with Vitex agnus-castus was significantly reduced from 946 mIU/l (±173) to 529 mIU/l (±297) (p < 0.001). Comparatively, mean prolactin concentration in the Bromocriptine group was significantly reduced from 885 mIU/l (±178) to 473 mIU/l (±266) (p < 0.001) demonstrating that both treatments were effective treatment for women with hyperprolactinaemia (normal reference range 25-628 mIU/l). The mean difference in prolactin reduction of the two groups was not significant (p = 0.96) (Table 2).

Pre-clinical and clinical evidence was found for Vitex agnus-castus for lowered prolactin, improved menstrual regularity and treatment of infertility. Vitex agnus-castus contains a variety of compounds which bind to dopamine type 2 (DA-2) receptors in the brain; reduce cyclic adenosine mono phosphate (cAMP) and lowered prolactin secretion (Table 1). This was demonstrated in studies using recombinant DA-2 receptor proteins, and basal and stimulated rat pituitary cell cultures [38–41]. Prolactin lowering effects were found in normal and ovariectomised rats [49]. Additional agonistic opiate effects were observed in studies using human opiate receptors cell cultures [70].

One warning: Retinoids can cause birth defects and cannot be used if you are already pregnant or are planning to become pregnant.

Just fyi, when a cat stops grooming itself, it's a sign that it's about to die. Or at least something is seriously wrong, physically or psychologically. Elderly cats typically stop grooming themselves when they're about to die. I'm a vet tech, and have seen cats in their early 20's that still groom themselves. They may need a bit of help with their back ends due to arthritis, but a well cared for cat of any age will at least try.
This is a really big deal, because GnRH antagonist drugs are pretty common and used to treat some cancer. They are also part of the treatment for women with PCOS who are stimulating their ovaries for IVF and egg-freezing. This study suggests that we might be able to stop PCOS before it develops by treating pregnant women with this drug, and we can potentially use it to treat women who currently have PCOS. It is important to note that there are two commonly recognized types of PCOS, a lean phenotype and an obese phenotype. The authors explain that their mouse model most closely resemble the lean PCOS phenotype, so this treatment might not be able to help all women with PCOS. However, it is still an important step forward.

Women with polycystic ovary syndrome have menstrual disorders caused by the absence of ovulation. About 20% of women will not ovulate on clomiphene citrate, the primary treatment option. These women can be treated with a surgical procedure like laparoscopic electrocautery of the ovaries or by ovulation induction with gonadotrophins or gonadotrophin releasing hormone (GnRH). In normal menstrual cycles, GnRH is released in a regular pulsatile interval. A portable pump can be used to mimic this pulse to help these women to ovulate and hopefully to get pregnant. The review of trials did not find enough evidence to show the effectiveness of pulsatile GnRH in women with polycystic ovary syndrome.
