Another study, a double-blind trial by Legro et al, found that letrozole is more effective than clomiphene in the treatment of infertility in PCOS. Based on treatment periods of up to five cycles, the study, which involved 750 anovulatory women with PCOS, found that the birth rates for letrozole and clomiphene were 27.5% and 19.1%, respectively. The rate of congenital abnormalities and the risk of pregnancy loss in the letrozole and clomiphene groups were found to be comparable, although the likelihood of twin births was lower with letrozole. We used the following definitions. PCOS according to the Rotterdam diagnostic criteria, specified by the presence of two out of three features; oligo/amenorrhoea, hyperandrogenism and polycystic ovaries on ultrasound [29, 30]. Associated endocrine features for PCOS included elevated LH [31], which is strongly associated with infertility (p = 0.0003) [32] and miscarriage [33] and elevated fasting glucose which is prevalent in approximately 31% of women with PCOS including normal weight women [34].

PCOS women with different phenotypes have been found similarly insulin resistant in response to a 3 h 75 g OGTT[31]. Obese (compared to lean) PCOS women tend to have a higher degree of IR. Correlation between hyperandrogenism and IR is significant in many studies but not as significant as the link between insulin abnormalities and obesity[32]. PCOS women demonstrate greater variation in insulin parameters compared to controls, independent of weight[33]. Animal studies of prenatal testosterone exposure show downstream IR in early postnatal life[34]. Some human data shows a high degree of correlation between hyperandrogenism and IR[35,36] and the relationship between hyperandrogenism and IR seem to differ between PCOS and non-PCOS women[35].
The first step in diagnosing PCOS is to have a complete history and physical examination performed by a trained health care provider. A careful history and physical examination can detect whether androgen excess is causing male-pattern hair growth (hirsutism), acne or hair loss and whether ovulation is occurring normally. Physical examination also can detect high blood pressure and increased abdominal obesity as risk factors for diabetes and cardiovascular disease in individuals who are overweight.

Paeonia lactiflora in combination with Cinnamomum cassia Aqueous extract Unkei-to Paeonia lactiflora and Cinnamomum cassia combination was investigated for steroid hormonal effects on cultured human granulosa cells (obtained from women undergoing IVF). Cells were incubated with different doses for 48 hours [42] One clinical trial investigated the effects of Paeonia lactiflora and Cinnamomum cassia combination (Unkei-to) [57]. This single arm study included amenorrheic women aged 17–29 years (n = 157) with a sub group of women with hyper-functioning oligo/amenorrhoea (n = 42). Ovulation occurred in 61.3% of primary amenorrheic women and in 27.3% of secondary amenorrheic women following two months of treatment [57]. 1. Increased granulosa production of oestradiol [42].
Miscarriage is the medical term for the spontaneous loss of pregnancy from conception to 20 weeks gestation. Risk factors for a woman having a miscarriage include cigarette smoking, older maternal age, radiation exposure, previous miscarriage, maternal weight, illicit drug use, use of NSAIDs, and trauma or anatomical abnormalities to the uterus. There are five classified types of miscarriage: 1) threatened abortion; 2) incomplete abortion; 3) complete abortion; 4) missed abortion; and (5 septic abortion. While there are no specific treatments to stop a miscarriage, a woman's doctor may advise avoiding certain activities, bed rest, etc. If a woman believes she has had a miscarriage, she needs to seek prompt medical attention.

PCOS treatment is different for different people. While there is no cure for PCOS, taking medicine and losing weight can help  your symptoms. If you don’t want to become pregnant, your doctor or nurse may recommend hormonal birth control, like the hormonal IUD, birth control implant, pill, patch, ring, or shot to treat your PCOS. The pill, patch, or ring may be particularly helpful if you’re struggling with acne or want more regular periods. If you’re trying to get pregnant, drugs that treat insulin resistance may help, as well as certain fertility drugs that can help you ovulate. Losing weight can also help with ovulation and fertility.

The name ‘Polycystic Ovarian Syndrome’ points to the ovaries because it was long considered to be a reproductive issue. But it is now widely accepted that polycystic ovaries develop as a result of endocrine disorders characterized by a series of hormone imbalances: hyperandrogenism (specifically excess testosterone) and Insulin Resistance due to excess insulin that can trigger a cascade of other hormonal problems.4 From a systemic point of view, the continuing and/or increase of PCOS symptoms is likely due to a continuing hormonal imbalance.

This thread fucks me up. I feel so much pity and sadness for this poor girl. Sort of hits close to home, when I turned 18 I moved in with a boy and got really sick and hooked on drugs for 2 years. I left him and got better, never did I ever get as bad as Luna but it could have happened had I been complacent like she is. I hope she finds the courage to leave. I think there's still beauty and potential underneath all the grime and illness, I even like her art sometimes and wish she'd spend more time on her paintings instead of on drugs. We undertook two searches of the scientific literature. The first search sought pre-clinical studies which explained the reproductive endocrine effects of whole herbal extracts in oligo/amenorrhoea, hyperandrogenism and PCOS. Herbal medicines from the first search informed key words for the second search. The second search sought clinical studies, which corroborated laboratory findings. Subjects included women with PCOS, menstrual irregularities and hyperandrogenism.
In a patient not seeking pregnancy, the Endocrine Society recommends hormonal contraception (i.e., oral contraceptive, dermal patch, or vaginal ring) as the initial medication for treatment of irregular menses and hyperandrogenism manifesting as acne or hirsutism.19,30 Small studies have shown that metformin can restore regular menses in up to 50% to 70% of women with PCOS,39,40 but oral contraceptives have been shown to be superior to metformin for regulating menses and lowering androgen levels.30 There are no studies demonstrating superiority of one oral contraceptive over another in treating PCOS. Prevention of endometrial hyperplasia from chronic anovulation may be accomplished either by progesterone derivatives, progestin-containing oral contraceptives, or the levonorgestrel-releasing intrauterine system (Mirena).31,41 Patient comfort and preference should also be taken into account when treating irregular menses.
Many health experts believe that numerous variables, including genetics, might be a factor in the development of PCOS. For example, you might find your sister, mother, aunt or grandmother has PCOS. It is clear that hormone imbalance is a primary influencing factor in PCOS as well as a condition called insulin resistance. Insulin is a potent hormone which is released by the pancreas as a result of food intake, in particular carbs. Insulin conveys sugar out from the blood and moves it into other cells such as muscle, liver and even fat cells. The sugar is then changed into energy or in some cases stored as fat. Sometimes this process is defective which is called insulin resistance.
High levels of masculinizing hormones: Known as hyperandrogenism, the most common signs are acne and hirsutism (male pattern of hair growth, such as on the chin or chest), but it may produce hypermenorrhea (heavy and prolonged menstrual periods), androgenic alopecia (increased hair thinning or diffuse hair loss), or other symptoms.[17][19] Approximately three-quarters of women with PCOS (by the diagnostic criteria of NIH/NICHD 1990) have evidence of hyperandrogenemia.[20]
