A woman should shed this lining at least four times a year, says Dunaif. One option: taking progesterone (often called a “progesterone challenge”). But the resulting bleed can be heavy for some women. Another: Take birth control pills to get the regular cycle back online. These have the added benefit of lowering male hormones, too. What’s more, there are noted benefits of taking hormonal birth control that are true across the board, like getting regular, predictable periods and having up to a 20 and 50 percent lower risk of colon (colorectal) cancer and ovarian cancer, respectively. “When any condition crosses disciplines and doesn’t have a full investment in [one of them], it often falls through the cracks. There are elements of reproduction in PCOS, but most reproductive endocrinologists mostly do in-vitro fertilization and are not necessarily interested in metabolism. Studies have attempted for years to show an advantage to metformin for ovulation induction and as an adjunct to more advanced fertility treatments. In ovulatory PCOS women metformin was associated with improved serum and follicular fluid AMH levels as well as insulin values; these changes were not seen in anovulatory PCOS women[71]. Despite the demonstration of negative effects of IR on reproductive outcome, the vast majority of evidence does not show improvement in live birth rates when metformin is used strictly for fertility[72], although treatment does improve ovulatory status[72,73].
Metformin has been the mainstay of treatment for IR and IGT in PCOS women over the past decade. Metformin is a biguanide that acts principally on the liver to inhibit hepatic gluconeogenesis. It also inhibits acetyl-CoA carboxylase activity and suppresses fatty acid production. Metformin acts on skeletal muscle to inhibit lipid production and acts peripherally on adipose tissue to stimulate glucose transport and uptake. Metformin reduces insulin levels and promotes improved insulin receptor activity[64]. Metformin may also have direct and indirect effects on the ovary with respect to insulin action and steroidogenic enzymatic activity. In the endothelium, metformin seems to improve nitric oxide vasodilatory effects. Many other mechanisms of action have been studied in both animal and human models but consistent effects are not always demonstrated with local tissue concentrations that result from therapeutic doses[65].

This therapeutic modality is also considered a second-line treatment for the infertility of women with PCOS. However, because it is an invasive method that requires general anesthesia and has a higher cost and potential complications, this technique should be used in cases of anovulatory women with CC-resistant PCOS who require laparoscopy for another reason (pelvic pain, adnexal mass, etc.). This technique can be performed using monopolar electrocautery or laser techniques, with both exhibiting a similar efficacy and the goal is between 4 and 10 punctures because a larger number may favor the development of premature ovarian failure 25,29. The mechanism of action of ovarian drilling in the treatment of infertility in women with PCOS is suggested to be based on the decreased secretion of androgens and consequent reduction of peripheral aromatization of these compounds into estrone. Furthermore, the follicular microenvironment becomes more estrogenic, which facilitates follicular growth 30. Regarding the efficacy of ovarian drilling, observational studies demonstrated that the ovulation rate was between 54 and 76% in the 6 months after the procedure and 33 and 88% in the 12 months after the procedure. During these periods, the spontaneous pregnancy rate ranged between 28 and 56% and 54 and 70%, respectively 31.
For women who don’t receive timely, appropriate care for PCOS in early adolescence, the development of symptoms such as facial-hair growth can become more challenging to treat. Brandy Cramer, 33, a program officer at The Cameron Foundation, from Midlothian, Virginia says her doctors told her she just wasn’t trying hard enough to lose weight and dismissed her when she requested they run blood tests or suggest alternatives to the birth-control pills that gave her intense migraines. Cramer grew facial hair and has only been able to remove 50 percent of it, even after expensive laser hair-removal treatment.
The use of gonadotropins for timed intercourse is associated with an ovulation rate of approximately 70%, a clinical pregnancy rate of 20% per cycle and a multiple live birth rate of 5.7% 9. Due to the cost of the treatment, the need for regular monitoring of the follicular development via ultrasound and the higher pregnancy rates with IUI, the use of gonadotropin is not routine for timed intercourse. Instead, this medication is used in IUI 26 or high-complexity treatments (IVF or ICSI) 9.
A diet patterned after the type 2 diabetes diet has been recommended for PCOS patients. [81] This diet emphasizes increased fiber; decreased refined carbohydrates, trans fats, and saturated fats; and increased omega-3 and omega-9 fatty acids. However, in some obese patients with PCOS, weight loss has improved menstrual regularity. [82] Omega-3 fatty acid supplementation has been shown to reduce liver fat content and other cardiovascular risk factors in women with PCOS, including those with hepatic steatosis, although these effects have not yet been proven to translate into a reduction in cardiometabolic events. [83]

Preclinical and clinical studies provide evidence that six herbal medicines may have beneficial effects for women with oligo/amenorrhea, hyperandrogenism and PCOS. However the quantity of pre-clinical data was limited, and the quality of clinical evidence was variable. Further pre-clinical studies are needed to explain the effects of herbal medicines not included in this review with current clinical evidence but an absence of pre-clinical data.

Another study, a double-blind trial by Legro et al, found that letrozole is more effective than clomiphene in the treatment of infertility in PCOS. Based on treatment periods of up to five cycles, the study, which involved 750 anovulatory women with PCOS, found that the birth rates for letrozole and clomiphene were 27.5% and 19.1%, respectively. The rate of congenital abnormalities and the risk of pregnancy loss in the letrozole and clomiphene groups were found to be comparable, although the likelihood of twin births was lower with letrozole. [53, 54]
Lifestyle measures to achieve a weight loss of 5%-10% in overweight women can help regulate ovulation and periods. Although the basic approach of nutrition is needed, it can be more challenging to lose weight and maintain weight loss with PCOS. Dr. Dunaif from Northwestern is very encouraging that even “a little bit of weight reduction and exercise can improve insulin sensitivity.”

The IUI is performed with the same dose of gonadotropins recommended for timed intercourse (combined or not with clomiphene). However, for this treatment modality, the recombinant hCG is administered for final oocyte maturation when the dominant follicle has a mean diameter of 17 to 18 mm via US examination and capacitated sperm can be injected into the uterine cavity 36 hours later. Beta hCG is measured 14 days later to confirm pregnancy 25.


Herbal medicines are complex interventions with the potential for synergistic and antagonistic interactions between compounds [25]. Effects within the body may also exhibit complexity by simultaneous interactions with various body systems, both biochemically and by altering organ function [26]. The focus of this review was studies investigating whole herbal medicine extracts with direct effects on reproductive endocrinology for the treatment of women with irregular menstruation, hyperandrogenism and PCOS. The rationale for using this methodology was to identify herbal medicines with current scientific evidence explaining specific reproductive endocrinological effects in PCOS, oligo/amenorrhoea and hyperandrogenism, to develop understanding for the direct effects of herbal medicines on reproductive endocrinology and to highlight herbal medicines for which there was current scientific evidence supporting herbal medicine selection. The purpose of this review is to inform clinical decisions in integrative settings and meet clinicians and consumers preferences for pragmatic herbal management within an holistic, individualised treatment frame [27, 28].
In vitro fertilization represents the third-line treatment for infertility in women with PCOS 9. However, if the initial assessment demonstrates a bilateral tubal occlusion and/or concentration of recovered motile sperm less than or equal to 5 million, this treatment becomes the first option along with lifestyle changes. The risk of OHSS is the main complication of the highly complexity treatment in women with PCOS. Thus, to minimize this side effect, ovarian stimulation should be initiated with low doses of gonadotropins (100 to 150 IU of FSHr) and the pituitary should be suppressed with a gonadotropin-releasing hormone (GnRH) antagonist because this method is associated with a reduced risk of OHSS compared with an agonist (29 randomized control trials (RCTs); OR 0.43; 95% CI: 0.33 to 0.57) 32. If the patient presents with clinical and ultrasound signs of OHSS, final oocyte maturation should be performed with a GnRH agonist and embryos should be frozen and transferred in a subsequent cycle 33,34. Infertile women with PCOS may present with better general oocyte and embryo quality rates; however, the clinical pregnancy and live birth rates are similar to those observed in normo-ovulatory women without PCOS 35.

Because of the menstrual and hormonal irregularities, infertility is common in women with PCOS. Because of the lack of ovulation, progesterone secretion in women with PCOS is diminished, leading to long-term unopposed estrogen stimulation of the uterine lining. This situation can lead to abnormal periods, breakthrough bleeding, or prolonged uterine bleeding. Unopposed estrogen stimulation of the uterus is also a risk factor for the development of endometrial hyperplasia and cancer of the endometrium (uterine lining). However, medications can be given to induce regular periods and reduce the estrogenic stimulation of the endometrium (see below).
Second-line therapy, when clomiphene citrate fails to lead to pregnancy, is either exogenous gonadotropins or laparoscopic ovarian surgery. [2, 3] If gonadotropins are used, a low-dose regimen is recommended, [3] and patients must be monitored with ultrasonography and laboratory studies. [2] Note that gonadotropin therapy is expensive and is associated with an increased risk of multiple pregnancy and ovarian hyperstimulation syndrome. [2]

One menstrual cycle. 147 women aged less than 35 years with un-explained infertility and recurrent clomiphene resistance for ovulation induction. Anovulatory participants were excluded (n = 28). Anovulation was diagnosed by serum oestradiol < 200 ng/ml and absence of a dominant ovarian follicle on day 9 of the menstrual cycle. Complete data sets available for 119 women. All women received Clomiphene citrate (clomiphene) 150 mg on menstrual cycle days 3–7. A randomised group also took Cimicifuga racemosa 20 mg per day between days 1–12. Cimicifuga racemosa described as ‘phytoestrogens’ was provided in the commercial preparation Klimadynon®, manufactured by Norica in Germany. A trigger injection (human chorionic gonadotropin, 10 000 IU) and timed intercourse was recommended when a dominant follicle > 17 mm was observed. Pregnancy rate measured as increasing serum human chorionic gonadotropin (HCG) over two days. Clinical pregnancy defined as detection of gestational sac with embryonic heart-beat. Endometrial thickness measured by ultrasound concurrent with follicle maturation monitoring. Number of days to ovulation (trigger injection) Serum concentration for FSH oestradiol and LH. Luteal progesterone measured on days 21–23 of the menstrual cycle. Miscarriage and multiple pregnancy rates. Pregnancy rate in clomiphene alone group was 20.3% and 43.3% in the clomiphene plus Cimicifuga racemosa group (P < 0.01). Clinical pregnancy rate in the combination group was 36.7% versus 13.6% in the clomiphene alone group (P < 0.01). Endometrial thickness in combination group was 8.9 (±1.4) versus 7.5 (±1.3) (p < 0.001). Days to ovulation in clomiphene alone group was 13.0 ± 1.1 and in the clomiphene plus Cimicifuga racemosa group 14.2 ± 1.3 (n.s.). Luteal progesterone peak (ng/ml) in combination group was 13.3 (±3.1) versus 9.3 (±2.0) in clomiphene alone group (p < 0.01). All other hormone measures were not significantly different No detailed current baseline criteria for other causes of infertility. Confounding factors include current male fertility status. This may have caused an imbalance between the two groups. There is no description of the distribution of excluded (anovulatory) participants between groups.

No evidence that therapeutic agents additional to V. agnus-castus in Mastodynon® affect prolactin concentration. Four withdrew for unknown reasons. All pregnant women were withdrawn from the study. 4 women had miscarriages, all in the active arm. After 2 years there were 21 more pregnancies with 2 miscarriages – evenly spread over active and placebo groups. Women with infertility were included in this study however data from women who conceived were excluded. This may have led to an underestimation of treatment effect (type 1 error).

I don't believe that Luna goes ~3 days without eating uwu~ often like she claims, but she probably doesn't eat much at all on days where she's e-begging. The junk food she shares on Instagram? Pretty sure she only eats that stuff occasionally. She probably doesn't have enough money for food a lot of the time, and binges on sugary things when they get paid. Junkies will choose drugs over food, and unless Pat's dropping off groceries EVERY WEEK I think she probably skips a lot of meals/doesn't eat that many calories a day.

Hola, actualmente estoy tomando diane diario por que son las únicas anticonceptivas que me funcionan, no puedo dejarlas porque si no padezco de acne severo e hirsutismo. Podría combinar ambas cosas? Lo que me gustaría es poder dejarme las pastillas algunos períodos, o si por ejemplo quisiera ser madre, poder dejar los métodos anticonceptivos y no tener que pasarlo mal por esos problemas, se conseguiría con estas cápsulas a largo plazo?


Polycystic ovary syndrome (PCOS) is a prevalent, complex endocrine disorder characterised by polycystic ovaries, chronic anovulation and hyperandrogenism leading to symptoms of irregular menstrual cycles, hirsutism, acne and infertility. Evidence based medical management emphasises a multidisciplinary approach for PCOS, as conventional pharmaceutical treatment addresses single symptoms, may be contra-indicated, is often associated with side effects and not effective in some cases. In addition women with PCOS have expressed a strong desire for alternative treatments. This review examines the reproductive endocrine effects in PCOS for an alternative treatment, herbal medicine. The aim of this review was to identify consistent evidence from both pre-clinical and clinical research, to add to the evidence base for herbal medicine in PCOS (and associated oligo/amenorrhoea and hyperandrogenism) and to inform herbal selection in the provision clinical care for these common conditions.

What matters most: Your goal when losing weight is to aim specifically to reduce the fat around your abdomen (belly fat). This requires boosting your metabolism like women who had PCOS did in an Italian weight loss study.7 Those who rode exercise bikes for 30 minutes, three times a week, lost more abdominal fat than those who shed pounds by just eating less.7 You won’t be successful if you only change your diet. More women in the exercise group than the diet group began ovulating even though both groups lost similar amounts of weight.7
Polycystic ovary syndrome (PCOS) is a condition associated with hormone imbalances that affects women. Though the underlying cause of PCOS is not known or well understood, it is believed that an imbalance of sex hormones and resistance to the effects of the hormone insulin are the main problems. These problems result in a characteristic group of signs, symptoms and complications such as excess facial and body hair, irregular menstrual periods, infertility, and insulin resistance.
High levels of insulin. Insulin is a hormone that controls how the food you eat is changed into energy. Insulin resistance is when the body's cells do not respond normally to insulin. As a result, your insulin blood levels become higher than normal. Many women with PCOS have insulin resistance, especially those who have overweight or obesity, have unhealthy eating habits, do not get enough physical activity, and have a family history of diabetes (usually type 2 diabetes). Over time, insulin resistance can lead to type 2 diabetes.
