A long-term study suggested that metformin continued to improve the metabolic profile of women with PCOS over a 36-month treatment course, particularly improving circulating high-density lipoprotein cholesterol (HDL-C), diastolic blood pressure, and body mass index (BMI). [61] However, data are insufficient as yet to recommend metformin to all women with PCOS.
Anecdotally, oral contraceptives are among the most common agents used to treat menstrual irregularities in women with PCOS. However, there are few studies examining their effect on menstrual cycles in women with PCOS. Cyproterone acetate plus ethinyl estradiol has been extensively studied, but it is not available in the United States. Studies suggest that the following agents may improve menstrual irregularities (e.g., oligomenorrhea): spironolactone (in an open-label study),22 acarbose,24 rosigli-tazone,32 and metformin.10,11,32,47,51 Metformin is probably the best choice because it may improve insulin resistance in addition to improving menstrual irregularities.

Clinical investigations found no adverse effects for the six herbal medicines included in this review (Table 2). Medical therapy inhibits hair growth without eliminating hair already present. Therefore, medical therapy usually is combined with mechanical methods of hair removal, such as electrolysis or lasers. Electrolysis refers to the insertion of an electrode to destroy individual hair follicles. It is ideal for removing small areas of sparse hair of any color. Laser hair removal is a common, safe and effective cosmetic procedure that also is performed for removal of unwanted facial and body hair. Laser therapy is more expensive than electrolysis but is faster, less painful, and requires fewer sessions. Laser therapy destroys hair by targeting the pigment (melanin) in the hair follicle and is ideally suited for dark-haired, light-skinned individuals. Longer-wavelength lasers with cooling devices can be used for individuals who have red, true blond, or white hair and for dark-skinned people. With appropriate therapy, there is up to a 70% reduction in hair density in treated areas, with remaining hairs becoming thinner in diameter within 3-6 months of treatment. Most patients then require some degree of maintenance treatment, usually every 6-12 months.
Complementary medicine (CM) use by women has increased during the past ten years [7–11] with rates of use ranging between 26% and 91% [8, 9]. One of the popular types of CM is herbal medicine [11, 12]. Herbal medicines are known to contain pharmacologically active constituents with physiological effects on female endocrinology and have been positively associated with reduced incidences of breast cancer, osteoporosis and cardiovascular disease [13–18].
Clinical trials have shown that metformin can effectively reduce androgen levels, improve insulin sensitivity, and facilitate weight loss in patients with PCOS as early as adolescence. [55, 56, 57, 58] One study concluded that the use of metformin throughout pregnancy was associated with a 9-fold decrease in gestational diabetes in women with PCOS. [59] In addition to having the potential to reduce gestational diabetes in pregnant women with PCOS, metformin may also reduce the risk of preeclampsia in this population. [60]
Getting three hours of exercise a week is enough to improve insulin sensitivity in women with PCOS, especially if you have been inactive up until now. Exercise can help you lose more weight, but more importantly, it can also help you lose the fat around your abdomen – that’s what contributes directly to insulin resistance, which fuels PCOS symptoms – even if you don’t lose any weight. That’s what happened in a 2017 small Monash University study.16  Women in this study added interval training (ie, short bursts of high-intensity exercise followed by regular or moderate intensity intervals) for part of their workout.16

If a regular menstrual cycle is not desired, then therapy for an irregular cycle is not necessarily required. Most experts say that, if a menstrual bleed occurs at least every three months, then the endometrium (womb lining) is being shed sufficiently often to prevent an increased risk of endometrial abnormalities or cancer.[93] If menstruation occurs less often or not at all, some form of progestogen replacement is recommended.[92] An alternative is oral progestogen taken at intervals (e.g., every three months) to induce a predictable menstrual bleeding.[medical citation needed]

Regular menstruation is important for the prevention of endometrial cancer. Women with PCOS are three times more likely to have endometrial cancer than women without. When a woman isn’t menstruating on a frequent basis, the lining of the uterus (endometrium) can begin to grow excessively and undergo atypical cell changes resulting in a precancerous condition called endometrial hyperplasia. If left untreated, this can develop into full endometrial cancer. Hormonal birth-control pills are often prescribed to help women with PCOS shed their endometrium more regularly, an important measure for preventing the overgrowth of cells in the uterus.
The first search revealed ten herbal medicines with a demonstrated mechanism of reproductive endocrinological effect for the whole herbal extract in PCOS, oligo/amenorrhoea and hyperandrogenism. She's not like many of the other cows in that she doesn't try to hide her cellulite or mismatching eye sockets or her flab. She knows she has these features and doesn't seem to care, she mentions them herself, and she might even like how they fit into her style. Irregular uterine bleeding from PCOS is usually due to lack of ovulation. Under this circumstance, the fragile lining of the uterus (endometrium) becomes thickened from excess estrogen and is not corrected by the monthly production of progesterone from the ovary that normally follows ovulation. Consequently, oral contraceptives (which contain progesterone-like substances), progesterone alone, or possibly a progestin-releasing intrauterine device, can be used to reverse the actions of estrogen on the uterine lining. Rarely, abnormal uterine bleeding after childbearing that does not respond to medical therapy may require surgical therapy.

Excess insulin. Insulin is the hormone produced in the pancreas that allows cells to use sugar, your body's primary energy supply. If your cells become resistant to the action of insulin, then your blood sugar levels can rise and your body might produce more insulin. Excess insulin might increase androgen production, causing difficulty with ovulation.
