Other tests that may be helpful but are not necessary for diagnosis include measurement of LH and follicle-stimulating hormone (FSH) levels to determine a serum ratio of LH/FSH. A ratio greater than 2 generally indicates PCOS, but there are no exact cutoff values because many different assays are used.26 The FSH level is more helpful in ruling out ovarian failure.26

The prevalence of infertility in women with PCOS varies between 70 and 80%. According to the American Society for Reproductive Medicine, the evaluation of infertility in women with PCOS or other causes of subfertility should start after six months of attempting pregnancy without success if the couple has regular sexual intercourse (2 to 3 times/week) without using contraceptive methods 7. To optimize the efficacy of the treatment of infertile women with PCOS, evaluations of tubal patency (hysterosalpingography or laparoscopy with chromotubation) and semen analysis (spermogram) are mandatory before deciding on treatment. However, tubal patency evaluation may not be necessary prior to initiating clomiphene citrate (CC) treatment. Notably, if a patient is resistant to this drug and/or requires the use of gonadotropins and/or presents with other causes of infertility, a tubal patency evaluation becomes mandatory prior to initiating the therapeutic treatment of infertility 8.

We already have partial opiate agonists (like Suboxone) that people have been on for years. From what I've read about a vaccine like this, opioids could still be administered with the intended effect of substantial pain relief. With no unwanted side effects (high). Someone please correct me if I'm wrong, this prospect really excites me. Something like this could lead to a whole new class of painkilling drugs for people with chronic pain, allowing them to function without being high at all times.
Patients with PCOS who are infertile but desire pregnancy should be referred to a reproductive endocrinologist for further evaluation and management of infertility. Morbidly obese women with PCOS should also be referred for pregnancy risk  ; metabolic surgery may be considered in morbidly obese women with PCOS, because many features of this syndrome are reversible with successful weight loss. In vitro fertilization (IVF) is reserved for women with PCOS and unsuccessful gonadotropin therapy or those with other indications for this procedure.

An animal study compared the effectiveness of Cinnamomum cassia and the pharmaceutical Metformin on hormone concentration in rats with PCOS  (Table 1). Both interventions demonstrated significant improvements compared to controls at 15 days for measures of testosterone ng/ml (control 0.747 ± 0.039; metformin 0.647 ± 0.027; Cinnamomum cassia 0.625 ± 0.029); LH ng/ml (control 7.641 ± 0.267; metformin 6.873 ± 0.214; Cinnamomum cassia 6.891 ± 0.221) and insulin resistance (HOMA-IR) (control 10.018 ± 0.217; metformin 7.067 ± 0.184 Cinnamomum cassia 8.772 ± 0.196) (p < 0.05) . The metabolic effects for Cinnamomum cassia were further demonstrated in overweight women with oligo/amenorrhoea and PCOS in a placebo controlled RCT  (Table 2). However, although the RCT had low risks for bias, it was a pilot study primarily investigating feasibility. Outcomes were promising for metabolic profile in PCOS however the sample size was small and the authors recommended further studies.
A 2-hour oral glucose tolerance test can be easily performed to assess the risk of diabetes in a PCOS woman. If necessary, prevention of diabetes may require lifestyle management with weight loss, diet and exercise, and possibly the addition of insulin-sensitizing agents such as metformin, or other medications. Fasting lipid measurements in the blood also can be performed to predict the risk for future cardiovascular disease and may likewise require lifestyle management, along with oral statins and other medications to correct increased triglyceride and low-density lipoprotein (LDL)-cholesterol levels and/or decreased high-density lipoprotein cholesterol (HDL-C) levels.

The condition, which affects five million women in the U.S. alone, can wreak havoc on your hormones and still doesn’t have a known cure. While many women successfully manage their symptoms solely with medication and hormone therapy, some prefer to supplement traditional treatments with more holistic remedies, while some opt for entirely natural alternatives.
In PCOS, both ovaries tend to be enlarged, as much as three times their normal size. Eggs that do not mature fully are not released during ovulation and the immature eggs remain in the ovary as pearl-sized, fluid filled sacs. Over the course of time, many cysts may develop into what looks like a string of beads when viewed through ultrasound imaging. In as many as 90% of women with PCOS, an ultrasound of the ovaries will reveal cysts.
A prospective, observational clinical trial examined the endocrine effects of Tribulus terrestris 750 mg per day, over five days in eight healthy women (aged 28–45). A significant increase in mean serum FSH concentration from 11 mIU/ml before treatment to 17.75 mIU/ml following treatment (P < 0.001) was demonstrated. Pre-treatment FSH levels returned following cessation of treatment (Table 1). Another clinical study evaluated the equivalence of Tribulus terrestris (Tribestan®) and pharmaceuticals for ovulation induction in women with oligo/anovular infertility (n = 148), . During the three month follow up, ovulation rates were highest with epimestrol (74%), followed by Tribulus terrestris (60%), clomiphene (47%) and cyclofenil (24%). However, the evidence for Tribulus terrestris should be interpreted with caution due to risks for bias in clinical studies. One study was uncontrolled with a small number of healthy participants , the second study did not report baseline characteristics, methods for allocation to treatment groups and data were not statistically analysed  (Table 1).
For some, lifestyle changes may be all you need to control the symptoms of your PCOS. But for many other women, medications may be necessary to help control harder to manage symptoms, such as fertility, and major risk factors that arise with polycystic ovary syndrome, specifically insulin resistance that leads to diabetes and high blood cholesterol that may end up developing into heart disease.
