The clinical presentation of PCOS is variable. Patients may be asymptomatic or they may have multiple gynecologic, dermatologic, or metabolic manifestations. Patients with PCOS most commonly present with signs of hyperandrogenism and a constellation of oligomenorrhea, amenorrhea, or infertility.19,22 Workup for PCOS is sometimes prompted by an incidental finding of multiple ovarian cysts after ultrasonography.

The goal of further evaluation of suspected PCOS is twofold: to exclude other treatable conditions that can mimic PCOS and to detect and treat long-term metabolic complications. Anovulation is common after menarche, so it is reasonable to delay workup for PCOS in adolescents until they have been oligomenorrheic for at least two years.28 If an adolescent is evaluated for PCOS, it has been suggested that she meet all three of the Rotterdam criteria before being diagnosed with the condition28  (Table 119).
Treatment of polycystic ovary syndrome is individualized to treat prevailing signs/symptoms and generally focused on correcting irregular bleeding from the uterus, restoring fertility, improving the skin manifestations of androgen excess (hirsutism, acne, hair loss), and preventing diabetes and cardiovascular disease. More recently, attention has also focused on improving the quality of life in PCOS women who have body image concerns.

Patients with PCOS who are infertile but desire pregnancy should be referred to a reproductive endocrinologist for further evaluation and management of infertility. Morbidly obese women with PCOS should also be referred for pregnancy risk [2] ; metabolic surgery may be considered in morbidly obese women with PCOS, because many features of this syndrome are reversible with successful weight loss. In vitro fertilization (IVF) is reserved for women with PCOS and unsuccessful gonadotropin therapy or those with other indications for this procedure. [2]
Hirsutism is treated with a combination of approaches, including oral contraceptives with or without an antiandrogen, such as spironolactone, to lower levels or block actions of androgen on hair follicles. Oral contraceptives are often combined with antiandrogens to improve their clinical effect and to prevent pregnancy, since accidental exposure of the male fetus to antiantrogens can harm fetal development. A topical cream (eflornithine hydrochloride) also can be applied to treat facial hirsutism.
Complementary medicine (CM) use by women has increased during the past ten years [7–11] with rates of use ranging between 26% and 91% [8, 9]. One of the popular types of CM is herbal medicine [11, 12]. Herbal medicines are known to contain pharmacologically active constituents with physiological effects on female endocrinology and have been positively associated with reduced incidences of breast cancer, osteoporosis and cardiovascular disease [13–18].

Two important things to note are that the long term safety effects of use are not known (but the drug has been in use for decades already) and spironolactone IS NOT SAFE in pregnancy, so it is commonly used in conjunction with birth control pills. This use of birth control pills also protects against abnormal growth of the lining of the uterus. Some women cannot take birth control pills, in which case it is important to use other means of birth control if you are sexually active while taking spironolactone.

I've never believed that she could get together $1700 on rent ALONE. I don't know much about U.S benefits but in the UK for an under 25 year old unemployment check is just about £60 a week. Even disability allowance wouldnt support the cost of a house like that and their drug habit. Seems impossible that they'd be paying for the house without help from Roger or family members

A randomized study suggested that combined metformin/letrozole and bilateral ovarian drilling are similarly effective as second-line treatment in infertile women with clomiphene citrate–resistant PCOS. [52] In this study, 146 patients were given metformin and letrozole, and 73 underwent bilateral ovarian drilling. There was significant reduction in testosterone, fasting insulin, and ratio of fasting glucose to fasting insulin in the metformin/letrozole group. There was significant reduction in follicle-stimulating hormone (FSH), luteinizing hormone (LH), and ratio of LH to FSH in the bilateral drilling group. There was no significant difference between the patients in the 2 groups regarding cycle regularity, ovulation, pregnancy rate, and abortion rate. [52]

The homeostatic model assessment (HOMA), a more complex fasting calculation, has been compared to clamp techniques with good results. HOMA is the product of fasting glucose (mg/dL) and insulin (μU/mL) divided by a constant[45]. One major limitation of HOMA rests on the previous reflection that many young PCOS women display stimulated but not fasting metabolic abnormalities. In fact, HOMA in young PCOS patients missed 50% of IR as compared to OGTT with insulin-AUC calculations[52]. G/I ratio correlated strongly with clamp-demonstrated IR in a small study of PCOS women - interestingly, both lean and obese PCOS women had evidence of IR. Sex hormone binding globulin (SHBG) did not correlate with IR in this study[47], as has been previously postulated[53].

Insulin-sensitizing agents are indicated for most women with PCOS because they have positive effects on insulin resistance, menstrual irregularities, anovulation, hirsutism, and obesity. Of all the drugs used to treat manifestations of PCOS, metformin (Glucophage) has the most data supporting its effectiveness. Table 1 details the most common medications used to treat manifestations of PCOS.6–27
Medical therapy inhibits hair growth without eliminating hair already present. Therefore, medical therapy usually is combined with mechanical methods of hair removal, such as electrolysis or lasers. Electrolysis refers to the insertion of an electrode to destroy individual hair follicles. It is ideal for removing small areas of sparse hair of any color. Laser hair removal is a common, safe and effective cosmetic procedure that also is performed for removal of unwanted facial and body hair. Laser therapy is more expensive than electrolysis but is faster, less painful, and requires fewer sessions. Laser therapy destroys hair by targeting the pigment (melanin) in the hair follicle and is ideally suited for dark-haired, light-skinned individuals. Longer-wavelength lasers with cooling devices can be used for individuals who have red, true blond, or white hair and for dark-skinned people. With appropriate therapy, there is up to a 70% reduction in hair density in treated areas, with remaining hairs becoming thinner in diameter within 3-6 months of treatment. Most patients then require some degree of maintenance treatment, usually every 6-12 months.
Some other blood tests are suggestive but not diagnostic. The ratio of LH (Luteinizing hormone) to FSH (Follicle-stimulating hormone), when measured in international units, is elevated in women with PCOS. Common cut-offs to designate abnormally high LH/FSH ratios are 2:1[66] or 3:1[62] as tested on Day 3 of the menstrual cycle. The pattern is not very sensitive; a ratio of 2:1 or higher was present in less than 50% of women with PCOS in one study.[66] There are often low levels of sex hormone-binding globulin,[62] in particular among obese or overweight women.[citation needed]
