Polycystic Ovarian Syndrome is a condition that affects a woman's hormonal levels. Women with PCOS produce higher than normal male hormones which cause them to skip their monthly periods and make it harder for them to get pregnant. PCOS also causes hair growth on the face and body, and baldness. So basically, women with PCOS have dense hair growth on those areas of their bodies where they're not supposed to have excess hair. But they start losing hair from their scalps, leading to baldness. PCOS can contribute to long-term health problems like diabetes and heart disorders.
PCOS is the most common endocrinopathy among reproductive-aged women in the United States, affecting approximately 7% of female patients.1 Although its exact etiology is unclear, PCOS is currently thought to emerge from a complex interaction of genetic and environmental traits. Evidence from one twin-family study indicates that there is a strong correlation between familial factors and the presence of PCOS.2
A total of 33 studies were included in this review. Eighteen pre-clinical studies reported mechanisms of effect and fifteen clinical studies corroborated pre-clinical findings, including eight randomised controlled trials, and 762 women with menstrual irregularities, hyperandrogenism and/or PCOS. Interventions included herbal extracts of Vitex agnus-castus, Cimicifuga racemosa, Tribulus terrestris, Glycyrrhiza spp., Paeonia lactiflora and Cinnamomum cassia. Endocrine outcomes included reduced luteinising hormone (LH), prolactin, fasting insulin and testosterone. There was evidence for the regulation of ovulation, improved metabolic hormone profile and improved fertility outcomes in PCOS. There was evidence for an equivalent effect of two herbal medicines and the pharmaceutical agents bromocriptine (and Vitex agnus-castus) and clomiphene citrate (and Cimicifuga racemosa). There was less robust evidence for the complementary combination of spirinolactone and Glycyrrhiza spp. for hyperandrogenism.
A secondary analysis of two randomized, double blind, placebo-controlled trials that included 182 children of mothers with PCOS reported that children exposed to metformin had higher BMI and increased prevalence of overweight/obesity at 4 years of age. The study found that at 4 years of age, the metformin group had higher weight z-score than the placebo group; difference in means 0.38 (0.07 to 0.69), p=0.017, and higher BMI z-score; difference in means 0.45 (0.11 to 0.78), p=0.010. There were also more overweight/obese children in the metformin group; 26 (32%) than in the placebo group; 14 (18%) at 4 years of age; odds ratio (95% CI): 2.17 (1.04 to 4.61), p=0.038. More studies are needed to examine this association. [94]
The risk of developing prediabetes and type 2 diabetes is increased in women with PCOS, particularly if they have a family history of diabetes. Obesity and insulin resistance, both associated with PCOS, are significant risk factor for the development of type 2 diabetes. Several studies have shown that women with PCOS have abnormal levels of LDL ("bad") cholesterol and lowered levels of HDL ("good") cholesterol in the blood. Elevated levels of blood triglycerides have also been described in women with PCOS.
The name ‘Polycystic Ovarian Syndrome’ points to the ovaries because it was long considered to be a reproductive issue. But it is now widely accepted that polycystic ovaries develop as a result of endocrine disorders characterized by a series of hormone imbalances: hyperandrogenism (specifically excess testosterone) and Insulin Resistance due to excess insulin that can trigger a cascade of other hormonal problems.4 From a systemic point of view, the continuing and/or increase of PCOS symptoms is likely due to a continuing hormonal imbalance.

Clinical equivalence for prolactin lowering effects of Vitex agnus-castus (Agnucaston® 40 mg per day) and the pharmaceutical Bromocriptine (Parlodel® 5 mg per day) was found in one study including 40 women with hyperprolactinaemia [63]. Mean concentrations for prolactin following three months treatment with Vitex agnus-castus was significantly reduced from 946 mIU/l (±173) to 529 mIU/l (±297) (p < 0.001). Comparatively, mean prolactin concentration in the Bromocriptine group was significantly reduced from 885 mIU/l (±178) to 473 mIU/l (±266) (p < 0.001) demonstrating that both treatments were effective treatment for women with hyperprolactinaemia (normal reference range 25-628 mIU/l). The mean difference in prolactin reduction of the two groups was not significant (p = 0.96) (Table 2).
Evidence for Tribulus terrestris, Glycyrrhiza spp. alone and in combination with Paeonia lactiflora and Paeonia lactiflora with Cinnamomum cassia was limited by the volume of laboratory and animal studies, with only one to two studies found for each herb or herbal combination. There was supporting clinical data, however many were small single arm, open label studies measuring endocrine effects in healthy women. Evidence for these herbal medicines is preliminary and in an emergent phase.
On June 8, 2011, the FDA notified health care professionals of its recommendations for limiting the use of the highest approved dose (80 mg) of the cholesterol-lowering medication simvastatin (Zocor) because of increased risk of muscle damage. The FDA required changes to the simvastatin label to add new contraindications (should not be used with certain medications) and dose limitations for using simvastatin with certain medications. [64]
My brother was a Heroin addict who started taking Benzos between his shooting up, like Luna, versus the other way around. He was an addict, he took benzos to keep up whatever feeling he was chasing. Thats inherently different than an anxiety or depression patient taking them for relief of symptoms. Im sure it does happen, there's always that risk but I wouldn't go out there and say that Benzo use is always a gateway.
Couples with infrequent sexual intercourse may experience some benefit from the use of kits for ovulation monitoring (urinary luteinizing hormone excretion); however, this technique can underestimate the fertile window. Lifestyle interventions are usually required for long term sustainable results. PCOS women who smoke have higher free androgen levels and IR as measured by HOMA-IR, QUICKI and the insulin sensitivity index following 75 g OGTT[104]. Thus PCOS women who smoke have an additional reason to stop smoking. In more general population studies (non-PCOS) comprised mostly of middle-aged women, lifestyle intervention is more effective than metformin in preventing the progression to DM. Dietary and exercise intervention decreased the 4 year progression to DM in patients at risk (non-diabetic, elevated fasting and/or OGTT glucose) by almost 50%[105]. Realizing the limitations of applying this population sample to young PCOS women, it still highlights the benefit of non-pharmacological treatment. PCOS women randomized to both metformin and lifestyle interventions (compared to placebo) showed improvements in HOMAIR after 4 mo[106]. In European adolescents with PCOS who failed to achieve improvements in HOMA-IR after 6 mo of lifestyle intervention, both metformin and placebo reduced IR over 6 mo, although metformin offered no benefit over placebo[107]. Lifestyle modification in adolescents has been successful in reducing hyperandrogenism[103]. Modest weight loss of about 5% bodyweight has also been shown to lower hyperandrogenism[108] which may ultimately improve IR.

According to Dr. Geoffrey Redmond, an endocrinologist specializing in female hormones, “Just because the ovaries are not functioning as much doesn’t mean the other abnormalities won’t still be present.” He goes on to point out that studies show male hormone levels climb fairly sharply with age.2 This could mean a worsening of symptoms such as excess hair growth as those hormones become more active. It could also mean insulin-related issues such as diabetes and cardiovascular health could become more problematic.

There are also experts who suggest taking more of a lifestyle treatment approach rather than medication, which some call a “Band-Aid” to symptoms. One such expert is Amy Medling, a certified health coach who is founder of PCOS Diva and author of Healing PCOS: A 21-Day Plan for Reclaiming Your Health and Life with Polycystic Ovary Syndrome. She stresses that some women don’t feel well on some of these drugs (she was one of them), so looking at other ways to manage PCOS will get them to a more balanced place. “I hear from many women who are frustrated and hopeless and feel underserved by the mainstream way of managing PCOS,” says Medling.

Hirsutism is a bothersome hyperandrogenic manifestation of PCOS that may require at least six months of treatment before improvement begins. According to a 2015 Cochrane review, the most effective first-line therapy for mild hirsutism is oral contraceptives.32 Spironolactone, 100 mg daily, and flutamide, 250 mg twice daily, are safe for patient use, but the evidence for their effectiveness is minimal.32 Other therapies include eflornithine (Vaniqa), electrolysis, or light-based therapies such as lasers and intense pulsed light. Any of these can be used as monotherapy in mild cases or as adjunctive therapy in more severe cases.33

One laboratory study and two clinical investigations provided evidence for the two herb combination, Glycyrrhiza uralensis and Paeonia lactiflora[53, 58, 59] (Table 1). An animal study found significant reductions in free and total testosterone following exposure to the combination [53] (Table 1). These findings were supported in two open label clinical trials including women with PCOS (n = 34) [59] and women with hyperandrogenism (n = 8) [58]. Both trials examined the effects on androgens for the aqueous extract TJ-68 (equal parts Glycyrrhiza uralensis and Paeonia lactiflora), 75 grams per day for 24 weeks and 5–10 grams per day for 2–8 weeks respectively. In the trial including women with PCOS, mean serum testosterone was significantly reduced from 137.1 ng/dL (±27.6) to 85.3 ng/dL (±38), p < 0.001 at four weeks of treatment [59]. Similar effects were observed in the women with oligomenorrhoea and hyperandrogenism which showed serum testosterone reduced from 50-160 ng/dL prior to treatment to less than 50 ng/dL [58]. However statistical significance was not reached due to the small sample size despite positive outcomes in seven out of eight participants (Table 1).

Although it has been known for many years that the ovary is innervated by catecholaminergic nerve fibers and much experimental evidence has strengthened the notion that catecholamines are physiologically involved in the control of ovarian function, scarce evidence has been presented as to the role of sympathetic activity in ovarian pathologies that affect reproductive function. The purpose of this article is to provide a succinct overview of the findings in this area and discuss them relative to the pathology of polycystic ovary syndrome, the most common ovarian pathology in women during their reproductive years.

No evidence that therapeutic agents additional to V. agnus-castus in Mastodynon® affect prolactin concentration. Four withdrew for unknown reasons. All pregnant women were withdrawn from the study. 4 women had miscarriages, all in the active arm. After 2 years there were 21 more pregnancies with 2 miscarriages – evenly spread over active and placebo groups. Women with infertility were included in this study however data from women who conceived were excluded. This may have led to an underestimation of treatment effect (type 1 error).
The pathogenesis of PCOS has been linked to altered luteinizing hormone (LH) action, insulin resistance, and a possible predisposition to hyperandrogenism.3–7 One theory maintains that underlying insulin resistance exacerbates hyperandrogenism by suppressing synthesis of sex hormone–binding globulin and increasing adrenal and ovarian synthesis of androgens, thereby increasing androgen levels. These androgens then lead to irregular menses and physical manifestations of hyperandrogenism.8

Other names for this syndrome include polycystic ovarian syndrome, polycystic ovary disease, functional ovarian hyperandrogenism, ovarian hyperthecosis, sclerocystic ovary syndrome, and Stein–Leventhal syndrome. The eponymous last option is the original name; it is now used, if at all, only for the subset of women with all the symptoms of amenorrhea with infertility, hirsutism, and enlarged polycystic ovaries.[52]

The genetic component appears to be inherited in an autosomal dominant fashion with high genetic penetrance but variable expressivity in females; this means that each child has a 50% chance of inheriting the predisposing genetic variant(s) from a parent, and, if a daughter receives the variant(s), the daughter will have the disease to some extent.[24][26][27][28] The genetic variant(s) can be inherited from either the father or the mother, and can be passed along to both sons (who may be asymptomatic carriers or may have symptoms such as early baldness and/or excessive hair) and daughters, who will show signs of PCOS.[26][28] The phenotype appears to manifest itself at least partially via heightened androgen levels secreted by ovarian follicle theca cells from women with the allele.[27] The exact gene affected has not yet been identified.[7][24][29] In rare instances, single-gene mutations can give rise to the phenotype of the syndrome.[30] Current understanding of the pathogenesis of the syndrome suggests, however, that it is a complex multigenic disorder.[31]
