2. Oestrogen receptor binding affinity for Cimicifuga racemosa was studied using pituitary cell cultures from ovariectomised rats. This study followed a clinical study demonstrating significantly lowered LH in post-menopausal women following administration of Cimicifuga racemosa (2 mg for two months) against placebo control (n = 110) [45] 2. Increases luteal progesterone concentration [65, 67, 68]
You just found out you have polycystic ovary syndrome (PCOS). Now what? The condition, which affects five million women in the U.S. alone, can wreak havoc on your hormones and still doesn’t have a known cure. While many women successfully manage their symptoms solely with medication and hormone therapy, some prefer to supplement traditional treatments with more holistic remedies, while some opt for entirely natural alternatives.
In a patient not seeking pregnancy, the Endocrine Society recommends hormonal contraception (i.e., oral contraceptive, dermal patch, or vaginal ring) as the initial medication for treatment of irregular menses and hyperandrogenism manifesting as acne or hirsutism.19,30 Small studies have shown that metformin can restore regular menses in up to 50% to 70% of women with PCOS,39,40 but oral contraceptives have been shown to be superior to metformin for regulating menses and lowering androgen levels.30 There are no studies demonstrating superiority of one oral contraceptive over another in treating PCOS. Prevention of endometrial hyperplasia from chronic anovulation may be accomplished either by progesterone derivatives, progestin-containing oral contraceptives, or the levonorgestrel-releasing intrauterine system (Mirena).31,41 Patient comfort and preference should also be taken into account when treating irregular menses.

“Often times the cosmetic issues are huge, depending on the severity. When you summarize the typical PCOS patient as someone who is fat, has acne, and male-pattern baldness, that is definitely depressing, but that in and of itself is not enough to cause depression,” she said. “It’s the hormonal imbalances that have a real neurobiological affect on the brain and we have evidence that the excess of androgens in women is definitely linked to depression.”


Mean platelet volume (MPV) is a marker associated with adverse cardiovascular events, and women with newly diagnosed PCOS appear to have significantly elevated MPV levels. [47] Kabil Kucur et al reported that use of ethinyl estradiol/cyproterone acetate or metformin for the treatment of women with PCOS seemed to have similar beneficial effects in reducing MPV. [47]

Once a diagnosis of PCOS is confirmed, it is imperative to assess women for CAD risk factors. Despite the many reasons women seek medical care for PCOS, the greatest long term risk for these women is CAD. This is generally not viewed or even recognized as a concern by women seeking care in the first place. The link between PCOS and CAD is multi-faceted. C-reactive protein (CRP) is higher in age matched PCOS women and is linked to BMI[10] with some ethnic variation in this risk[11]. The prevalence of MS in PCOS women is as high as 40% with increased prevalence of hypertension, dyslipidemia and abnormal glucose metabolism, all before age 30[12]. PCOS women aged 20-40 already demonstrate poor vascular function measured by brachial artery vascular flow[13]. No single blood test can predict or quantify this CAD risk. Although no standard recommendation for assessment of CAD risk factors exists, measurement of glucose metabolism, blood pressure screening, lipid screening and carotid intimal media thickness measurements have been suggested[14].

Side effects: This herb can cause several side effects including muscle pain, gastrointestinal issues, weight gain, headache, dizziness and vaginal spotting. Black Cohosh has also been associated with liver disease so be watchful for symptoms like dark urine, loss of appetite, yellowing of the skin or eyes and nausea which can be signs of liver complications.
Women with polycystic ovary syndrome have menstrual disorders caused by the absence of ovulation. About 20% of women will not ovulate on clomiphene citrate, the primary treatment option. These women can be treated with a surgical procedure like laparoscopic electrocautery of the ovaries or by ovulation induction with gonadotrophins or gonadotrophin releasing hormone (GnRH). In normal menstrual cycles, GnRH is released in a regular pulsatile interval. A portable pump can be used to mimic this pulse to help these women to ovulate and hopefully to get pregnant. The review of trials did not find enough evidence to show the effectiveness of pulsatile GnRH in women with polycystic ovary syndrome.
Metformin is among one of the main treatments to target insulin resistance if you have prediabetes or diabetes, including because of PCOS. The advice is controversial, but some physicians believe that PCOS always requires metformin, notes Dr. Dunaif. “There’s no reason to give every woman with PCOS metformin. It’s a good and safe drug, but there’s no point in taking it if you don’t need it,” she says. (She notes that it has such a good safety profile that even when overprescribed, it doesn’t cause any harm.) But it’s not a good way to reduce the male hormone symptoms, like excess hair growth.
Two laboratory based RCT’s examined the effects of Tribulus Terrestris in rats with polycystic ovaries induced with oestradiol valerate [46, 47] (Table 1). Both studies demonstrate significantly improved ovulation rates for animals treated with two doses of Tribulus terrestris extracts compared to controls. Although the endocrinological effects were not described in either study, laboratory findings of ovulation induction are supported by the clinical findings of elevated FSH following treatment with Tribulus terrestris[56] (Table 2).
Combined hormonal birth control pills can be used for long-term treatment in women with PCOS who do not wish to become pregnant. Combined hormonal pills contain both estrogen and progestin. These birth control pills regulate the menstrual cycle and reduce hirsutism and acne by decreasing androgen levels. They also decrease the risk of endometrial cancer.

Another animal study examined the effects of Glycyrrhiza uralensis on the morphological features of polycystic ovaries using immunohistochemistry [50] (Table 1). This study demonstrated significantly increased ovulation rates by the number of corpus luteum in polycystic ovaries compared with controls. The authors propose that the mechanism of effect for Glycyrrhiza uralensis was competitive inhibition of oestrogen at oestrogen receptor sites, limiting the production of nerve growth factor (NGF), its neurotropic effects and inhibition of sympathetic neurological involvement in the pathogenesis of polycystic ovaries.
In addition to improving reproductive and metabolic factors, the reduction in body weight may be associated with reduced incidence of complications during pregnancy and the neonatal period. Treatment of PCOS is individualized and depends on whether or not pregnancy is being sought. Dietary modifications, weight management and regular exercise are important factors in the management of this condition in all women with PCOS, regardless of whether they are trying to conceive or not. Cosmetic treatment options to treat excess body hair, particularly facial hair, include electrolysis, laser treatment and typical medication which act at the hair follicle. There are medical and surgical options to treat the hormone abnormalities associated with PCOS. For women who are not trying to conceive, medical treatment options include oral contraceptives, progestational agents (that induce periods), and drugs that block the production or action of androgens. In some cases, surgery is performed to cauterize the cysts, which results in a decrease in male hormone levels and return of ovulation in some women. Insulin modifiers are useful in those women with high insulin levels and insulin resistance but do not benefit all women with PCOS. The safety of these medications in pregnancy has not been established.
Insulin-sensitizing drugs. A particular medication, called metformin, is usually prescribed.20 Another medication, pioglitazone (Actos) may also be suggested for women with PCOS. These two medications are FDA-approved for diabetes treatment, but plenty of research shows they can be equally effective for women with PCOS, too. These insulin-sensitizing medications can help your body respond more readily to insulin, and better control your glucose levels. Both metformin and pioglitazone can reduce the insulin resistance and high insulin levels that commonly occur with PCOS, and in turn, can reduce high androgen levels.
The risk of developing prediabetes and type 2 diabetes is increased in women with PCOS, particularly if they have a family history of diabetes. Obesity and insulin resistance, both associated with PCOS, are significant risk factor for the development of type 2 diabetes. Elevated levels of blood triglycerides have also been described in women with PCOS.

Polycystic ovarian syndrome is classified as a disorder which encompasses a group of symptoms that collectively indicate a distinct health issue. This means that the symptoms associated with PCOS will vary from woman to woman. Your PCOS experience is probably very different from another woman with this condition but that does not mean you are alone! Three RCTs corroborate the positive fertility effects for Cimicifuga racemosa in women with PCOS, used in conjunction and when compared with the pharmaceutical Clomiphene citrate (clomiphene), [65, 68, 71] (Table 2). Results were reported for 441 women and show improved pregnancy rates when Cimicifuga racemosa was added to clomiphene during one menstrual cycle. In a study including women with PCOS (n = 147), pregnancy rates for the group receiving combined therapy (clomiphene 150 mg plus Cimicifuga racemosa 20 mg per day (Klimadynon®)) were 43.3% compared to 20.3% for women receiving only clomiphene [65] (Table 2). In another study using similar methodology (n = 100) pregnancy rates were 34.8% for the group treated with Cimicifuga racemosa plus clomiphene compared to 17.2% for women treated with clomiphene alone [68] (Table 2). Another study included women with PCOS and infertility (n = 100) compared Cimicifuga racemosa (Klimadynon®) and clomiphene over three months for hormone concentrations and pregnancy rates. Pregnancy rates were higher in the women in taking Cimicifuga racemosa compared to clomiphene, 14% and 8% respectively; however differences were not statistically significant. This study found significant effects for lowered luteinising hormone for women with PCOS receiving Cimicifuga racemosa compared to clomiphene (p = 0.007) [67]. Findings from clinical studies concur with laboratory and animal studies; however potential risks for bias include performance and collection bias due to lack of blinding (Table 2).
Many women with PCOS exhibit symptoms of insulin resistance and metabolic syndrome, risk factors that raise the risk for heart disease and other problems like diabetes and stroke. Low magnesium levels are often associated with diabetes, and some research indicates that a dietary supplement of the mineral may improve insulin sensitivity, a factor in the development of type 2 diabetes and PCOS. One study found that overweight, insulin-resistant subjects who received 300 mg of magnesium at bedtime showed a significant improvement in fasting blood glucose and insulin levels, compared to subjects who received a placebo.
Approximately 15% of women with PCOS do not respond to the maximum dose of CC and are considered resistant to this medication. Due to the anti-estrogenic effect of this drug, endometrial proliferation may be inappropriate, which decreases the chance of embryo implantation. Moreover, this effect can also change the cervical mucus characteristics with a consequent reduction in sperm penetration 17,23. If the patient does not ovulate after the use of CC, gonadotropins for timed intercourse or ovarian drilling are the next steps to manage anovulatory infertile women with PCOS 9.
The effectiveness of aromatase inhibitors in the treatment of PCOS remains controversial. A meta-analysis investigated 78 studies on the use of these medications in the infertility treatment of women with PCOS. Of these studies, 13 RCTs met the inclusion criteria. Six studies compared the use of letrozole versus CC and found that letrozole presented with a higher ovulation rate/patient (OR 2.90; 95% CI: 1.72- 4.88; p<0.0001); however, no significant differences in the rate of ovulation per cycle or better pregnancy, live birth, multiple pregnancy or miscarriages rates were noted. Letrozole also did not obtain better results regarding clinical pregnancy or live birth rates compared with placebo or CC + metformin in women with CC-resistant PCOS. The results of the comparison of the effects of letrozole and anastrozole on ovulation and pregnancy rates in women with CC-resistant PCOS are controversial 41.
Patients with PCOS who are infertile but desire pregnancy should be referred to a reproductive endocrinologist for further evaluation and management of infertility. Morbidly obese women with PCOS should also be referred for pregnancy risk [2] ; metabolic surgery may be considered in morbidly obese women with PCOS, because many features of this syndrome are reversible with successful weight loss. In vitro fertilization (IVF) is reserved for women with PCOS and unsuccessful gonadotropin therapy or those with other indications for this procedure. [2]
Polycystic ovary syndrome (PCOS) is a prevalent, complex endocrine disorder characterised by polycystic ovaries, chronic anovulation and hyperandrogenism leading to symptoms of irregular menstrual cycles, hirsutism, acne and infertility. Evidence based medical management emphasises a multidisciplinary approach for PCOS, as conventional pharmaceutical treatment addresses single symptoms, may be contra-indicated, is often associated with side effects and not effective in some cases. In addition women with PCOS have expressed a strong desire for alternative treatments. This review examines the reproductive endocrine effects in PCOS for an alternative treatment, herbal medicine. The aim of this review was to identify consistent evidence from both pre-clinical and clinical research, to add to the evidence base for herbal medicine in PCOS (and associated oligo/amenorrhoea and hyperandrogenism) and to inform herbal selection in the provision clinical care for these common conditions.

Acne is common in the general population and in patients with PCOS. Hormonal contraceptives are first-line medications for treating acne associated with PCOS and can be used in conjunction with standard topical acne therapy (e.g., retinoids, antibiotics, benzoyl peroxide) or as monotherapy.19,34 Antiandrogens, spironolactone being the most common, can be added as second-line medications.19,34

Other insulin sensitizing agents have been advocated and studied for the treatment of IR in PCOS, principally thiazolinediones. Thiazolinediones stimulate gene transcription that alters lipid and glucose metabolism, decreases lipolysis and decreases fat deposition[90]. Thiazolinediones decrease fatty acid release, suppress gluconeogenesis and reduce tumor necrosis factor α disruption of insulin activity[64]. Pioglitazone and rosiglitazone have decreased IR (measured by clamp studies) in PCOS women[90-93]. Glitazones have also decreased IR by OGTT AUC-Insulin in PCOS women[91,93,94]. In patients with DM, thiazolinediones reduce central adiposity[95], a trait commonly shared with PCOS women. Pioglitazone by way of IR and adiponectin levels also has improved menstrual regularity in PCOS women[96,97]. Adverse outcomes have been seen in pregnant animals with limited to no human data. Therefore, as a class, thiazolinediones are not considered first line therapy for PCOS women seeking pregnancy. Rosiglitazone has even been found to decrease pro-inflammatory markers in human granulosa cells cultured following in vitro fertilization oocyte retrieval, thus showing additional target tissue for therapy[98]. However, these effects have not been adequately studied and have no current practical application.

Just fyi, when a cat stops grooming itself, it's a sign that it's about to die. Or at least something is seriously wrong, physically or psychologically. Elderly cats typically stop grooming themselves when they're about to die. I'm a vet tech, and have seen cats in their early 20's that still groom themselves. They may need a bit of help with their back ends due to arthritis, but a well cared for cat of any age will at least try.

On February 28, 2012, the FDA approved important safety label changes for the class of cholesterol-lowering drugs known as statins, including removal of routine monitoring of liver enzymes. Information about the potential for generally nonserious and reversible cognitive side effects and reports of increased blood glucose and glycosylated hemoglobin (HbA1c) levels was added to the statin labels. In addition, extensive contraindication and dose-limitation updates were added to the lovastatin label in situations when this drug is taken with certain medications that can increase the risk for myopathy. [63]
A long-term study suggested that metformin continued to improve the metabolic profile of women with PCOS over a 36-month treatment course, particularly improving circulating high-density lipoprotein cholesterol (HDL-C), diastolic blood pressure, and body mass index (BMI). [61] However, data are insufficient as yet to recommend metformin to all women with PCOS.
 PCOS seems to be inherited. Female relatives or children of patients with PCOS are at increased risk for having PCOS. Environmental risk factors, including low birth weight, rapid weight gain in infancy, early pubic hair and puberty development, childhood obesity, excess adult weight, and unhealthy lifestyle, are also important and may interact with genes to lead to PCOS (called epigenetics).

Your doctor may offer different medicines that can treat symptoms such as irregular periods, acne, excess hair, and elevated blood sugar. Fertility treatments are available to help women get pregnant. Losing as little as 5% excess weight can help women ovulate more regularly and lessen other PCOS symptoms. The ideal way to do this is through nutrition and exercise.

A stroll around the block with your dog (spouse/significant other, neighbor, or exercise buddy), your favorite exercise class at the gym, following a video at home, or peddling on an exercise bike while you watch TV…whatever you choose to get you moving will be good.14 Aim to fit in at least 30 minutes, five times a week – the basic recommendations from the US Centers for Disease Control and Prevention.15
The differential diagnosis of PCOS is broad and includes both endocrinologic and malignant etiologies. Figure 119 provides an algorithm for the workup of select presentations. For any woman with suspected PCOS, the Endocrine Society recommends excluding pregnancy, thyroid dysfunction, hyperprolactinemia, and nonclassical congenital adrenal hyperplasia.19 Depending on presentation, conditions such as hypothalamic amenorrhea and primary ovarian insufficiency should also be excluded. In women with rapid symptom onset or significant virilization, such as deepening voice or clitoromegaly, an androgen-secreting tumor should be ruled out. Finally, Cushing syndrome or acromegaly should be excluded in patients with physical findings that suggest either condition.19 There is no need to order laboratory testing for these conditions if the patient does not have suggestive physical findings.
Removing or slowing the growth of excess hair.  Shaving, bleaching, plucking, waxing, and applying over-the-counter hair-removal creams are effective, albeit temporarily methods to get rid of unwanted excess hair. For more permanent results, you might try laser hair removal or electrolysis but these are expensive, require repeated treatments, and are not guaranteed to be successful.
A surgery called ovarian drilling might make your ovaries work better when ovulation medications don't, but it's being done less often than it used to. The doctor makes a small cut in your belly and uses a tool called a laparoscope with a needle to poke your ovary and wreck a small part of it. The procedure changes your hormone levels and may make it easier for you to ovulate.

The use of gonadotropins for timed intercourse is associated with an ovulation rate of approximately 70%, a clinical pregnancy rate of 20% per cycle and a multiple live birth rate of 5.7% 9. Due to the cost of the treatment, the need for regular monitoring of the follicular development via ultrasound and the higher pregnancy rates with IUI, the use of gonadotropin is not routine for timed intercourse. Instead, this medication is used in IUI 26 or high-complexity treatments (IVF or ICSI) 9.
Metformin has been studied specifically in adolescent PCOS women. Metformin therapy for 10 mo decreased fasting serum insulin levels in obese girls with PCOS[74].The positive effects of metformin in adolescents wore off within 3 mo of medication discontinuation[75]. Metformin in obese PCOS adolescents has shown improvements in IR by clamp studies, fasting measurements and OGTT after just 3 mo of therapy[76,77]. Other studies have found non-significant trends to improved IR by HOMA and OGTT-AUC in adolescent PCOS patients[78]. Metformin has also been shown to effectively contribute to BMI reduction in PCOS adolescents[79].
Second-line therapy, when clomiphene citrate fails to lead to pregnancy, is either exogenous gonadotropins or laparoscopic ovarian surgery. [2, 3] If gonadotropins are used, a low-dose regimen is recommended, [3] and patients must be monitored with ultrasonography and laboratory studies. [2] Note that gonadotropin therapy is expensive and is associated with an increased risk of multiple pregnancy and ovarian hyperstimulation syndrome. [2]

Women with polycystic ovary syndrome (PCOS) may suffer from irregular periods, excessive hair growth (hirsutism) and acne (pimples). High levels of serum androgens (male hormone) are one of the main features of PCOS. There is no good evidence from this review that statins improve menstrual regularity, spontaneous ovulation rate, hirsutism or acne, either alone or in combination with the combined oral contraceptive pill. There is also no good evidence that statins have a beneficial effect on hirsutism or acne (pimples) associated with PCOS. In women with PCOS, statins are effective in reducing serum androgen levels and decreasing bad cholesterol (LDL), but statins are not effective in reducing fasting insulin or insulin resistance. There is no good evidence available on the long‐term use of statins (alone or in combination) for the management of PCOS.
