Treatment of PCOS depends partially on the woman's stage of life. For younger women who desire birth control, the birth control pill, especially those with low androgenic (male hormone-like) side effects can cause regular periods and prevent the risk of uterine cancer. Another option is intermittent therapy with the hormone progesterone. Progesterone therapy will induce menstrual periods and reduce the risk of uterine cancer, but will not provide contraceptive protection.

This study synthesises the evidence for reproductive endocrine effects for six whole herbal medicine extracts that may be used to treat PCOS and associated oligo/amenorrhoea and hyperandrogenism. The findings were intended to add to clinicians understanding for the mechanisms of action for herbal medicine for treatment in these common conditions and reveal herbal medicines with reproductive endocrinological effects, currently demonstrated in scientific literature.
More research is needed to clarify the complex pathophysiology of PCOS. No single test is currently available for its diagnosis. Additionally, once diagnosis is established, the options for treatment are of limited number and effectiveness because they target only the symptoms of PCOS. Finally, patients with PCOS have higher rates of metabolic complications, such as cardiovascular disease, but their impact on mortality is not clear. Therefore, more prospective epidemiologic studies on the topic are necessary.

Herbal extract Phyto-Hypophyson® by Steril-Pharma GmbH Herrsching, Germany; contains Vitex agnus-castus plus Chelledonium majus and Silybum marianum (St Mary’s thistle) in homeopathic form. Additional herbal extracts have reported activity in hepatic function. There are no reports for direct reproductive effects. 150 drops per day (7.5 ml per day). Primary outcome for participants with amenorrhoea: at least one spontaneous menses. Oligomenorrhoeic subgroup - clinical outcomes were significantly improved in the treatment arm at 82% compared to 45% in placebo arm P = 0.021. When the amenorrheic group were included in analysis, differences were not significant p = 0.19. Diagnosis for anovulatory amenorrhoea is not well described. Non-statistically significant take home baby rates were complicated by insufficient sample size. 366 patients are required to have a 95% chance, as significant at the 5% level, an increase in take home baby rates from 6% in the placebo group to 18% in the experimental group. The authors conclude that this preparation may be useful if given 3–6 months, yet they only tested for 3 months.

Medications to induce fertility when trying to conceive include the ovulation inducer clomiphene or pulsatile leuprorelin. In practice, CC treatment can initiate the menstrual cycle as early as the second day. Classically, this drug treatment has been initiated between the third and fifth day of the menstrual cycle and maintained for 5 days. Ovulation typically occurs seven days after the last CC tablet is taken. Seven days after the probable date of ovulation, follicular rupture can be confirmed by progesterone levels greater than 3 ng/dL (evaluated only at the beginning of the treatment to verify the response to CC when US is unavailable) and pregnancy can be confirmed by measuring the blood beta fraction of human chorionic gonadotropin (βhCG) 7 days after the progesterone measurement. The couple should maintain their usual frequency of sexual intercourse, including during the fertile period. This protocol is ideal for primary healthcare centers with limited subsidiary resources.
Side effects: Red clover has been associated with headaches, nausea, vaginal bleeding, muscle ache and rash. Do not take this herb if you have conditions that are worsened by estrogen exposure such as endometriosis, breast cancer, ovarian cancer or if you are pregnant or breastfeeding. An animal study compared the effectiveness of Cinnamomum cassia and the pharmaceutical Metformin on hormone concentration in rats with PCOS [48] (Table 1). Both interventions demonstrated significant improvements compared to controls at 15 days for measures of testosterone ng/ml (control 0.747 ± 0.039; metformin 0.647 ± 0.027; Cinnamomum cassia 0.625 ± 0.029); LH ng/ml (control 7.641 ± 0.267; metformin 6.873 ± 0.214; Cinnamomum cassia 6.891 ± 0.221) and insulin resistance (HOMA-IR) (control 10.018 ± 0.217; metformin 7.067 ± 0.184 Cinnamomum cassia 8.772 ± 0.196) (p < 0.05) [48]. The metabolic effects for Cinnamomum cassia were further demonstrated in overweight women with oligo/amenorrhoea and PCOS in a placebo controlled RCT [66] (Table 2). However, although the RCT had low risks for bias, it was a pilot study primarily investigating feasibility. Outcomes were promising for metabolic profile in PCOS however the sample size was small and the authors recommended further studies.

Ask your health care provider about a weight loss plan if you are overweight. If you’re overweight, losing weight may lessen some of the symptoms of PCOS. Talk to your health care provider or nutritionist about healthy ways to lose weight such as exercising more and following a nutrition plan that helps manage insulin levels. Healthy eating can also keep your heart healthy and lower your risk of developing diabetes.

If the patient does not present with ovulatory cycles at three months after ovarian drilling, then the procedure should be combined with CC treatment. The use of gonadotropins should be considered after 6 months of anovulatory cycles following the ovarian drilling procedure. Ovarian drilling should not be indicated as a treatment for menstrual irregularity, metabolic complications or hyperandrogenism in PCOS 29.
Our second search for clinical trials was performed without language restriction and included randomised controlled trials, non-randomised, open label and single arm clinical trials. We included clinical studies investigating commercially available herbal extracts and investigations that compared the effectiveness of herbal medicine with pharmaceuticals. We excluded clinical studies investigating herbal medicines with unrelated outcomes (including pre-menstrual syndrome, endometriosis and mastalgia) and clinical studies examining the effectiveness of complex herbal formulas for PCOS and associated oligo/amenorrhoea and hyperandrogenism, without demonstration of a mechanism of effect for the whole complex formula. We compared data from laboratory and animal studies with the outcomes of clinical trials. Clinical studies were assessed for risks of bias at study and outcome levels with risks summarised, tabulated (Tables 1 and ​and2)2) and presented in contextual narrative.

Acne is common in the general population and in patients with PCOS. Hormonal contraceptives are first-line medications for treating acne associated with PCOS and can be used in conjunction with standard topical acne therapy (e.g., retinoids, antibiotics, benzoyl peroxide) or as monotherapy.19,34 Antiandrogens, spironolactone being the most common, can be added as second-line medications.19,34
Paeonia lactiflora in combination with Cinnamomum cassia Aqueous extract Unkei-to Paeonia lactiflora and Cinnamomum cassia combination was investigated for steroid hormonal effects on cultured human granulosa cells (obtained from women undergoing IVF). Cells were incubated with different doses for 48 hours [42] One clinical trial investigated the effects of Paeonia lactiflora and Cinnamomum cassia combination (Unkei-to) [57]. This single arm study included amenorrheic women aged 17–29 years (n = 157) with a sub group of women with hyper-functioning oligo/amenorrhoea (n = 42). Ovulation occurred in 61.3% of primary amenorrheic women and in 27.3% of secondary amenorrheic women following two months of treatment [57]. 1. Increased granulosa production of oestradiol [42].
The advantages of CC use are low cost, oral administration, few side effects (flushing, headache, visual disturbances and abdominal discomfort), the induction of monofollicular development in most cases 16 and a low rate of multiple gestations (2 to 13%) 17. The initial dose is 50 mg/day for five days (starting between the second and fifth day of the menstrual cycle) and may be increased to 150 mg/day 17,18; however, doses greater than 100 mg/day usually do not offer additional benefits (may be useful in obese women) 18. The ovulation rate may reach 75 to 80% 19 with a conception rate of 22% per cycle 20 and a cumulative pregnancy rate between 60 and 70% in six cycles 9. There is no evidence that the administration of human chorionic gonadotropin (hCG) in the mid-cycle increases ovulation rates (OR 0.99; 95% CI: 0.36-2.77) or clinical pregnancy (OR 1.02; 95% CI: 0.56-1.89) 21,22. CC treatment should be limited to six ovulatory cycles and US monitoring is not mandatory (it is recommended only in the first ovulatory cycle to adjust the dose based on the ovarian follicular growth and development and for endometrial assessment) 17,18. Additional cycles of ovulation induction with CC (maximum of twelve cycles) may be individually evaluated based on the cost-effectiveness and age of women and after discussion with the couple 9. The incidence of ovarian hyperstimulation syndrome (OHSS; increased capillary permeability with consequent third-space fluid sequestration and hemoconcentration) associated with the use of CC is low, approximately 1 to 6% 17,23.

“If a woman has fewer than eight menstrual periods a year on a chronic basis, she probably has a 50 to 80 percent chance of having polycystic ovary syndrome based on that single observation,” said John Nestler, the chair of the department of internal medicine at Virginia Commonwealth University. “But if she has infrequent menstruation and she has elevated levels of androgens such as testosterone in the blood, than she has a greater than 90 percent chance of having the condition.”
A long-term study suggested that metformin continued to improve the metabolic profile of women with PCOS over a 36-month treatment course, particularly improving circulating high-density lipoprotein cholesterol (HDL-C), diastolic blood pressure, and body mass index (BMI). [61] However, data are insufficient as yet to recommend metformin to all women with PCOS.
The effectiveness of aromatase inhibitors in the treatment of PCOS remains controversial. A meta-analysis investigated 78 studies on the use of these medications in the infertility treatment of women with PCOS. Of these studies, 13 RCTs met the inclusion criteria. Six studies compared the use of letrozole versus CC and found that letrozole presented with a higher ovulation rate/patient (OR 2.90; 95% CI: 1.72- 4.88; p<0.0001); however, no significant differences in the rate of ovulation per cycle or better pregnancy, live birth, multiple pregnancy or miscarriages rates were noted. Letrozole also did not obtain better results regarding clinical pregnancy or live birth rates compared with placebo or CC + metformin in women with CC-resistant PCOS. The results of the comparison of the effects of letrozole and anastrozole on ovulation and pregnancy rates in women with CC-resistant PCOS are controversial 41.
Polycystic ovary syndrome (PCOS) is an endocrine disorder that affects about 5 to 10 percent of women during their childbearing years. Typically, the ovaries contain multiple small cysts, which are often slightly enlarged. About 1 in 5 women have polycystic ovaries appearing on ultrasound scans but do not have the other features of this syndrome. These cysts do not require surgical removal. Polycystic ovaries make more male hormones (androgens) than do normal ovaries. The exact cause for PCOS is not known, but sometimes it is hereditary.

Anecdotally, oral contraceptives are among the most common agents used to treat menstrual irregularities in women with PCOS. However, there are few studies examining their effect on menstrual cycles in women with PCOS. Cyproterone acetate plus ethinyl estradiol has been extensively studied, but it is not available in the United States. Studies suggest that the following agents may improve menstrual irregularities (e.g., oligomenorrhea): spironolactone (in an open-label study),22 acarbose,24 rosigli-tazone,32 and metformin.10,11,32,47,51 Metformin is probably the best choice because it may improve insulin resistance in addition to improving menstrual irregularities.

The study evaluated psychological symptoms in 126 women diagnosed with PCOS. Participants completed surveys using a standard tool for evaluating mental health, the Brief Symptom Inventory (BSI), and their responses were compared with those of adult women in the general population and of adult women undergoing outpatient psychiatric care. While small, and not a randomized controlled trial, the study offers insight into the psychiatric manifestations of different PCOS symptoms, Reame says. Body hair and menstrual problems most strongly predicted anxiety, while obesity was most strongly associated with hostility, the study found.

The clinical presentation of PCOS is variable. Patients may be asymptomatic or they may have multiple gynecologic, dermatologic, or metabolic manifestations. Patients with PCOS most commonly present with signs of hyperandrogenism and a constellation of oligomenorrhea, amenorrhea, or infertility.19,22 Workup for PCOS is sometimes prompted by an incidental finding of multiple ovarian cysts after ultrasonography.
There are also experts who suggest taking more of a lifestyle treatment approach rather than medication, which some call a “Band-Aid” to symptoms. One such expert is Amy Medling, a certified health coach who is founder of PCOS Diva and author of Healing PCOS: A 21-Day Plan for Reclaiming Your Health and Life with Polycystic Ovary Syndrome.
