Quantitative insulin sensitivity check index (QUICKI) was developed to improve the sensitivity of fasting measurements. QUICKI is calculated as: 1/[log(insulin fasting) + log(glucose fasting)] and has been well correlated to clamp measurements in obese and non-obese patients[15]. QUICKI also demonstrates correlation with HOMA-IR[53]. QUICKI research calculations in young PCOS women are often identical to age matched women with DM[54].

Hirsutism is a bothersome hyperandrogenic manifestation of PCOS that may require at least six months of treatment before improvement begins. According to a 2015 Cochrane review, the most effective first-line therapy for mild hirsutism is oral contraceptives.32 Spironolactone, 100 mg daily, and flutamide, 250 mg twice daily, are safe for patient use, but the evidence for their effectiveness is minimal.32 Other therapies include eflornithine (Vaniqa), electrolysis, or light-based therapies such as lasers and intense pulsed light. Any of these can be used as monotherapy in mild cases or as adjunctive therapy in more severe cases.33

In the case of Polycystic Ovarian Syndrome, excess insulin causes the ovaries to produce excess testosterone, which can prevent ovulation and result in infertility. High insulin levels can also increase the conversion of testosterone into estrogen, which affects weight gain and the formation of ovarian cysts.5 Excess glucose is stored in fat cells that continue to pump out excess estrogen, further destabilizing the hormone system. Metformin has been shown to be useful in the management of irregular periods, ovulation induction, weight loss, as well as the prevention of type 2 diabetes and gestational diabetes mellitus in women with PCOS.
However, in women with PCOS receiving low doses of gonadotropins for timed intercourse, metformin administration can double the clinical pregnancy rate (OR 2.25; 95% CI: 1.50 to 3.38; p<0.001; 7 trials) and the live birth rate (OR 1.94; 95% CI: 1.10 to 3.44; p=0.020; 2 trials). Moreover, this practice can reduce the cancellation rate due to ovarian hyperresponsiveness by approximately 60% (OR 0.41; 95% CI: 0.24 to 0.72; p=0.002; 7 trials), the number of days of stimulation (mean difference (MD)=-3.28; 95% CI: -6.23 to 0.32; p=0.030; 6 trials) and the dose of gonadotropins (MD=-306.62; 95% CI: -500.02 to -113.22; p=0.002; 7 trials) in low-complexity cycles. However, the use of metformin is not related to a reduction in the multiple pregnancy rate (OR 0.32; 95% CI: 0.08 to 1.23; p=0.100; 3 trials), a change in the miscarriage rate (OR 0.47; 95% CI: 0.14 to 1.54; p=0.210; 5 trials) or OHSS (OR 0.56; 95% CI: 0.26 to 1.21; p=0.140; 5 trials). OGTT with 75-g glucose and hourly glucose and insulin measurements has been compared to clamp techniques. However these data show that 1 and 2 h levels are often needed to diagnose IR and stress the potential for false negative results with fasting measurements alone. Combination birth control pills — those with estrogen and Prometrium (progesterone) — are frequently prescribed to women with PCOS not looking to get pregnant. If the main concern is irregular periods and the resulting potential health risks, this is a great option. “Birth control pills are very good for protecting the lining of the uterus in women who are chronically anovulatory,” says Dunaif.
The ultimate goal is to prevent metabolic disease. Metformin (1500 mg per day) compared to placebo in a prospective 12 wk randomized control trial decreased arterial stiffness (by peripheral pressure waveforms in the brachial artery) and endothelial function (measured by augmentation index). Metformin did not reduce HOMA-IR[82]. The study population was obese but young (mean age 30 years), demonstrating the ability to reduce CAD risk even in very young women. Metformin has reduced both carotid intimal media thickness and endothelin levels in obese PCOS women[83]. In many studies metformin has reduced both total cholesterol and LDL cholesterol levels[84-86], triglyceride levels[84] and increased HDL levels[87,88]. Animal studies have shown that acarbose given to insulin resistant rats decreased carotid intimal hyperplasia and blood flow velocities[89]. Taken as a whole, the ability of metformin (and likely other insulin sensitizing agents) to elicit an overall reduction in the risk for CAD may be easier than the ability to produce consistent measureable improvements.
Any of the above symptoms and signs may be absent in PCOS, with the exception of irregular or no menstrual periods. All women with PCOS will have irregular or no menstrual periods. Metformin has been studied specifically in adolescent PCOS women. Metformin therapy for 10 mo decreased fasting serum insulin levels in obese girls with PCOS[74].The positive effects of metformin in adolescents wore off within 3 mo of medication discontinuation[75]. Metformin in obese PCOS adolescents has shown improvements in IR by clamp studies, fasting measurements and OGTT after just 3 mo of therapy[76,77]. Other studies have found non-significant trends to improved IR by HOMA and OGTT-AUC in adolescent PCOS patients[78]. Metformin has also been shown to effectively contribute to BMI reduction in PCOS adolescents[79].
PCOS treatment is different for different people. While there is no cure for PCOS, taking medicine and losing weight can help  your symptoms. If you don’t want to become pregnant, your doctor or nurse may recommend hormonal birth control, like the hormonal IUD, birth control implant, pill, patch, ring, or shot to treat your PCOS. A newer insulin resistance medication class, the thiazolidinediones (glitazones), have shown equivalent efficacy to metformin, but metformin has a more favorable side effect profile.[79][80] The United Kingdom's National Institute for Health and Clinical Excellence recommended in 2004 that women with PCOS and a body mass index above 25 be given metformin when other therapy has failed to produce results.[81][82] Metformin may not be effective in every type of PCOS, and therefore there is some disagreement about whether it should be used as a general first line therapy.[83] The use of statins in the management of underlying metabolic syndrome remains unclear.[84]
