Selection of herbal medicines for the management of PCOS often includes the combined prescription of Glycyrrhiza spp. and Paeonia lactiflora[72–75]. We found preliminary evidence for this combination for hyperandrogenism only, and the evidence was more robust for Glycyrrhiza spp. alone than when combined with Paeonia lactiflora. Comparatively, our findings for the combination of Peaonia lactiflora and Cinnamomum cassia demonstrated no change in androgen concentration, suggesting that the anti-androgen activity in the Glycyrrhiza spp. and Paeonia lactiflora combination more likely attributable to Glycyrrhiza spp. However our findings may be complicated by the aqueous extraction methods used in the Paeonia lactiflora and Cinnamomum cassia combination and the preclinical studies into the Glycorrhizza spp and Paeonia lactiflora combination. More research into the anti-androgen effects of the combination Glycyrrhiza spp. and Paeonia lactiflora is needed to clarify the anti-androgen mechanism particularly if this herbal combination remains cornerstone herbal management for hyperandrogenism.

Someone please correct me if I'm wrong, this prospect really excites me. One small study found that cutting down on dietary AGEs significantly reduced insulin levels in women with PCOS. Foods high in AGEs include animal-derived foods and processed foods. Applying high heat (grilling, searing, roasting) increases levels.

Very little information is available as to the role VIP plays in the control of steroid secretion during reproductive cyclicity and in ovarian pathologies involving altered steroid secretion. In this study, we determined the involvement of VIP in regulating ovarian androgen and estradiol release during estrous cyclicity and estradiol valerate (EV)-induced polycystic ovarian development in rats. Our findings show that androgen and estradiol release from ovaries obtained during different stages of rat estrous cycle mimic cyclic changes in steroid release observed in vivo with maximal release occurring during late proestrus. VIP increased androgen release from ovaries of all cycle stages except late proestrus and estradiol release from all cycle stages. Increases in VIP-induced androgen and estradiol release were maximal at early proestrus. Inclusion of saturating concentrations of androstenedione increased magnitude of VIP-induced estradiol release at diestrus and estrus but not proestrus. Magnitude of VIP-induced androgen and estradiol release tended to be greater in the ovaries from EV-treated rats with polycystic ovary compared with estrous controls. At the tissue level, ovarian VIP concentration was cycle stage dependent with highest level seen in diestrus. Maximum concentration of VIP was found in EV-treated rats. Changes in VIP were inversely related to changes in ovarian nerve growth factor, a neuropeptide involved in ovarian androgen secretion. These results strongly suggest that intraovarian VIP participates in the control of estradiol secretion during the rat estrous cycle and possibly in the maintenance of increased ovarian estradiol secretory activity of EV-treated rats.

Given the conditions associated with PCOS, the Endocrine Society, the Androgen Excess and PCOS Society, and the American College of Obstetricians and Gynecologists recommend that clinicians evaluate patients' blood pressure at every visit and lipid levels at the time of diagnosis, and screen for type 2 diabetes with a two-hour oral glucose tolerance test regardless of a patient's body mass index. Patients should have repeat diabetes screening every three to five years, or more often if other indications for screening are present.19–21 The Endocrine Society further recommends depression screening, as well as screening for symptoms of obstructive sleep apnea in overweight and obese patients with PCOS.19 However, routine screening for nonalcoholic fatty liver disease or endometrial cancer (using ultrasonography) is not recommended.19

This review has some limitations. We used a methodological approach which was deductive and not consistent with traditional rationale for herbal selection. Our inclusion criteria for clinical studies were specific and relied upon our identification of herbal medicines with preclinical (laboratory based) evidence explaining the mechanisms of reproductive endocrinological effects in oligo/amenorrhoea, hyperandrogenism and PCOS. Clinical studies were excluded from this review due to the absence of evidence for whole herbal extracts. This was the case for Camellia sinensis (green tea) for which only one laboratory study investigated the effects of injecting epigallocatechin, a catechin found in green tea in animals [76]. High quality clinical evidence for Camellia sinensis was not presented in this review due to the absence of pre-clinical data explaining the mechanism for effect for the whole herbal extract [77]. Mentha spicata (spearmint) was another herbal medicine excluded from this review despite the availability of high quality clinical evidence demonstrating testosterone lowering effects in women with PCOS [78]. We found no laboratory evidence describing the mechanism of action for Mentha spicata in hyperandrogenism. Camilla sinensis and Mentha spicata are examples of herbal medicines excluded from this review due to not meeting the inclusion criteria. Studies investigating western herbal medicines excluded from this review are provided in Tables 3, ​,44 and ​and55.
Regardless of what reasons women have for seeking diagnosis and treatment of PCOS, it is imperative for practitioners to assess a woman’s risk for CAD. Assessment should probably be made in all PCOS patients regardless of BMI. Especially in young women or adolescents, IR may be the first identifiable risk factor. Practitioners must recognize that no universal test for IR exists and must use good clinical judgment to assess metabolic status in women. Stimulated testing with OGTT may be more sensitive than fasting measurements. Women who demonstrate IR should be counseled on lifestyle modifications. Physicians should discuss with their patients a target BMI that is realistically obtainable. It is often advisable for patients to seek nutritional assessment and counseling to help with this goal. In many individuals, consideration should be given to pharmacological treatment. Although the most commonly used medication is metformin, other medications may be appropriate first line therapy, especially in women not actively seeking pregnancy.

Improved insulin sensitivity (QUICKI) in the treatment group. 0.35 to 0.38, (7.7%) p < 0.03. Insulin resistance (HOMO-IR) significantly reduced in treatment group 2.57 to 1.43 (44.5%) p < 0.03. Controls no change insulin sensitivity or insulin resistance. No change in either group for BMI, testosterone and oestradiol. Differences between Cinnamomum cassia group and normal weight and ovulatory controls were not significant. (P < 0.17). No reported adverse reactions. Small pilot study, the authors report that larger studies are required to confirm findings. Small sample size may explain non-significant comparison with normal weight and ovulating women. Reproductive outcomes were unchanged in this study however the duration of the study was insufficient to demonstrate reproductive changes.

Treatments for hirsutism in women with PCOS are similar to those in women without PCOS, such as patients with idiopathic hirsutism. The combination of these drugs may raise the blood levels of statins and increase the risk for myopathy. Rhabdomyolysis, the most serious form of myopathy, can cause kidney damage and lead to kidney failure, which is life threatening. [62]
The ultimate goal is to prevent metabolic disease. Metformin (1500 mg per day) compared to placebo in a prospective 12 wk randomized control trial decreased arterial stiffness (by peripheral pressure waveforms in the brachial artery) and endothelial function (measured by augmentation index). Metformin did not reduce HOMA-IR[82]. The study population was obese but young (mean age 30 years), demonstrating the ability to reduce CAD risk even in very young women. Metformin has reduced both carotid intimal media thickness and endothelin levels in obese PCOS women[83]. In many studies metformin has reduced both total cholesterol and LDL cholesterol levels[84-86], triglyceride levels[84] and increased HDL levels[87,88]. Animal studies have shown that acarbose given to insulin resistant rats decreased carotid intimal hyperplasia and blood flow velocities[89]. Taken as a whole, the ability of metformin (and likely other insulin sensitizing agents) to elicit an overall reduction in the risk for CAD may be easier than the ability to produce consistent measureable improvements.

The prevalence of infertility in women with PCOS varies between 70 and 80%. According to the American Society for Reproductive Medicine, the evaluation of infertility in women with PCOS or other causes of subfertility should start after six months of attempting pregnancy without success if the couple has regular sexual intercourse (2 to 3 times/week) without using contraceptive methods 7. To optimize the efficacy of the treatment of infertile women with PCOS, evaluations of tubal patency (hysterosalpingography or laparoscopy with chromotubation) and semen analysis (spermogram) are mandatory before deciding on treatment. However, tubal patency evaluation may not be necessary prior to initiating clomiphene citrate (CC) treatment. Notably, if a patient is resistant to this drug and/or requires the use of gonadotropins and/or presents with other causes of infertility, a tubal patency evaluation becomes mandatory prior to initiating the therapeutic treatment of infertility 8.

Hyperprolactinaemia is usually considered a unique cause for oligo/amenorrhoea; however in the present case it was included due to the potential co-existence for elevated prolactin, LH and PCOS, [32, 35].
Clomiphene (Clomid) is an oral medication that is the most common treatment used to induce ovulation. The use of both metformin and clomiphene has about the same fertility results as clomiphene use alone. A benefit is that metformin may help reduce the risk for ovarian hyperstimulation syndrome (see also "What is ovarian hyperstimulation syndrome (OHSS) during assisted reproductive technology (ART) fertility treatments. Other treatments to stimulate ovulation include another oral medication called letrozole (Femara) and gonadotropins which are hormones that are given by injection. In vitro fertilization (IVF) and in vitro maturation (IVM) may be other fertility treatment options.

Lasers, or a very thin, heated needle, are used to pierce several holes in the surface of your ovary, which improves the chances of successful ovulation for about six to eight months. This is specifically effective for women with PCOS since the outer surface of the ovaries can become extra thick, interfering with the natural process of ovulation.
