The use of gonadotropins should be considered after 6 months of anovulatory cycles following the ovarian drilling procedure. Ascertain that kidney and liver function are normal and that the patient does not have advanced congestive heart failure before starting metformin therapy. The usual starting dose is 500 mg given orally twice a day. Because common adverse effects are nausea, vomiting, and diarrhea, metformin should be taken with meals. Patients who develop these adverse effects can be instructed to decrease the dosage to once a day for a week and then gradually increase the dosage. Also, inform patients that there is a high likelihood that they will have ovulatory cycles while taking metformin. The US Food and Drug Administration (FDA) has not approved metformin for this indication.


Another study, a double-blind trial by Legro et al, found that letrozole is more effective than clomiphene in the treatment of infertility in PCOS. Based on treatment periods of up to five cycles, the study, which involved 750 anovulatory women with PCOS, found that the birth rates for letrozole and clomiphene were 27.5% and 19.1%, respectively. Modest weight loss of 2-5% of total body weight can help restore ovulatory menstrual periods in obese patients with PCOS. A decrease of 500-1000 calories daily, along with 150 minutes of exercise per week, can cause ovulation.

The contraceptive not only inhibits ovarian androgen production but also increases sex hormone-binding globulin (SHBG) production. The first step in diagnosing PCOS is to have a complete history and physical examination performed by a trained health care provider. A careful history and physical examination can detect whether androgen excess is causing male-pattern hair growth (hirsutism), acne or hair loss and whether ovulation is occurring normally. We undertook two searches of the scientific literature. The first search sought pre-clinical studies which explained the reproductive endocrine effects of whole herbal extracts in oligo/amenorrhoea, hyperandrogenism and PCOS. Herbal medicines from the first search informed key words for the second search. The second search sought clinical studies, which corroborated laboratory findings. Subjects included women with PCOS, menstrual irregularities and hyperandrogenism.
In contrast, another recent meta-analysis reviewed 26 studies that evaluated the use of letrozole in women with PCOS. The use of letrozole in cycles for timed intercourse was associated with higher live birth (nine studies; OR 1.63; 95% CI: 1.31 to 2.03; n=1783; I2=3%) and clinical pregnancy rates (fourteen studies; OR 1.32; 95% CI: 1.09 to 1.60; n=2066; I2=25%) compared with CC treatment; however, this evidence was poor. Studies comparing the use of letrozole versus ovarian drilling revealed no differences in live birth, clinical pregnancy or OHSS rates. The administration of letrozole for 5 or 10 days at a dose of 5 or 7.5 mg/day displayed similar clinical pregnancy rates 42. A recent study found that the use of letrozole was associated with higher live birth rates and ovulation among 750 infertile women with polycystic ovary syndrome compared with clomiphene 43.
Insulin-sensitizing agents, including metformin,31 acarbose (Precose),24 and rosiglitazone (Avandia),20 may be used to treat hirsutism in women with PCOS. Spironolactone22 and rosiglitazone32 have been shown to be more effective than metformin, based on Ferriman-Gallwey hirsutism scores. A Cochrane review suggested that metformin is as effective as oral contraceptives for treating hirsutism in women with PCOS,33 but in contrast, a recent systematic review suggested that metformin is not effective.1 Topical eflornithine cream is FDA-approved for management of unwanted facial hair, but there are no published data regarding its use specifically in women with PCOS. Sibutramine (Meridia), which is approved for obesity management, can also improve hirsutism.26

The placebo group reported average number of days in the luteal phase was 3.4 (±5.1) at baseline and 5.5 (±5.2) at three months, which was not significant (p = 0.22) [64] (Table 2). Methodological shortcomings included not reporting baseline characteristics for subgroups and small sample sizes; however clinical outcomes demonstrated physiological effects consistent with laboratory and animal findings (Tables 1 and ​and22).

No evidence that therapeutic agents additional to V. agnus-castus in Mastodynon® affect prolactin concentration. Four withdrew for unknown reasons. All pregnant women were withdrawn from the study. 4 women had miscarriages, all in the active arm. After 2 years there were 21 more pregnancies with 2 miscarriages – evenly spread over active and placebo groups. Women with infertility were included in this study however data from women who conceived were excluded. This may have led to an underestimation of treatment effect (type 1 error).

OGTT with 75-g glucose and hourly glucose and insulin measurements has been compared to clamp techniques. Insulin sensitivity calculated by mathematical transformation of measurements has shown good correlation with glucose disposal using clamp techniques[48]. Although the OGTT is easy to perform, these calculations are more complex and make this particular calculation less desirable for clinical use. However these data show that 1 and 2 h levels are often needed to diagnose IR and stress the potential for false negative results with fasting measurements alone.
