This study synthesises the evidence for reproductive endocrine effects for six whole herbal medicine extracts that may be used to treat PCOS and associated oligo/amenorrhoea and hyperandrogenism. The findings were intended to add to clinicians understanding for the mechanisms of action for herbal medicine for treatment in these common conditions and reveal herbal medicines with reproductive endocrinological effects, currently demonstrated in scientific literature.
A majority of women with PCOS have insulin resistance and/or are obese. Their elevated insulin levels contribute to or cause the abnormalities seen in the hypothalamic-pituitary-ovarian axis that lead to PCOS. Hyperinsulinemia increases GnRH pulse frequency, LH over FSH dominance, increased ovarian androgen production, decreased follicular maturation, and decreased SHBG binding. Furthermore, excessive insulin, acting through its cognate receptor in the presence of component cAMP signalling, upregulates 17α-hydroxylase activity via PI3K, 17α-hydroxylase activity being responsible for synthesising androgen precursors. The combined effects of hyperinsulinemia contribute to an increased risk of PCOS.[42] Insulin resistance is a common finding among women with a normal weight as well as overweight women.[10][17][21]
One of the biggest challenges in reviewing the evidence for PCOS treatment is that many manifestations of the condition may be components of other disease processes. For example, there may be a study of medications that are useful for hirsutism, but the patient population in the study did not explicitly have PCOS. Thus, recommendations specific for treating symptoms of PCOS may be lacking. When reviewing a study of the treatment of insulin resistance in a general population, it cannot be assumed that the outcomes would mirror those in women with PCOS.

They also did an ultrasound and found multiple cysts on my ovaries, but she did not know what it was. So I never went back to that doctor she was not very bright. But when I was 18 I was reading a magazine about a girl that has pcos and the symptoms and everything, and it sounded like me. So i went to My mom's obgyn and talked to him about what I read and my history of not getting my period on my own, he did an exam, ran some blood tests, and did other ultrasound. Obesity is associated with PCOS. Obesity not only compounds the problem of insulin resistance and type 2 diabetes (see below), but also imparts cardiovascular risks. PCOS and obesity are associated with a higher risk of developing metabolic syndrome , a group of symptoms, including high blood pressure, that increase the chances of developing cardiovascular disease. In anovulatory women with PCOS defined according to the Rotterdam consensus (includes all phenotypes except the one defined by the association of hyperandrogenism with ultrasound (US) findings), CC treatment is the first choice for ovulation induction 9,15. This drug is an estrogen receptor modulator (it can act as an estrogen agonist or antagonist) and its mechanism of action is controversial but can be explained as follows. In physiological menstrual cycles, low levels of estrogen promote negative feedback in the hypothalamus and pituitary gland and inhibit the endogenous secretion of gonadotropin during the early follicular phase. When CC is administered in this phase of the cycle, it competes with estrogen for its receptors in the hypothalamus and pituitary, which will block the negative feedback mechanism. Consequently, increased levels of endogenous gonadotropins are released and the dominant follicle is recruited (follicle that has the highest number of follicle-stimulating hormone (FSH) receptors) between the sixth and ninth day of the menstrual cycle 16.

Other pharmacological treatments have attempted to lower IR. Vitamin D has been shown to decrease HOMA-IR despite a lack of change in hyperandrogenism in young, obese PCOS women[99]. Where US is available, CC treatment should be initiated between the third and fifth day of the menstrual cycle and the couple should abstain from intercourse (this is not a mandatory measure) until the tenth day of the cycle (when the presence of dominant follicles with a mean diameter of 10 mm or more is assessed via US). Sexual activity is allowed if the patient presents with monofollicular or bifollicular development. The goal of sexual abstinence until the tenth day of the cycle is to minimize the risk for multiple gestations.

Pregnancy rate in clomiphene alone group was 20.3% and 43.3% in the clomiphene plus Cimicifuga racemosa group (P < 0.01). Clinical pregnancy rate in the combination group was 36.7% versus 13.6% in the clomiphene alone group (P < 0.01). Endometrial thickness in combination group was 8.9 (±1.4) versus 7.5 (±1.3) (p < 0.001).
