I was diagnosed with pcos while using implanon birth control in November 2012. I was told it was che...mical, it was symptom and blood diagnosed not with cysts on my ovaries. My ob/gyn told me that implanon can cause pcos and many other terrible medical conditions. I had it removed in January 2013 and have been trying to get pregnant since, after being told I needed a hysterectomy. (I was 24). In the past year I have gotten pregnant twice, both ended with miscarriage. In January this year, I had my blood tested again and I am almost completely normal. I'll always have pcos, it's life long, but the fact that after a year of getting the implanon removed I was able to get my blood back to normal. My endocrinologist called it miraculous!

Antiandrogens, such as spironolactone, are effective for hirsutism. [79] Spironolactone (50-100 mg twice daily) is an effective primary therapy for hirsutism. Because of the potential teratogenic effects of spironolactone, patients require an effective form of contraception (eg, an oral contraceptive). Adverse effects of spironolactone include gastrointestinal discomfort and irregular menstrual bleeding, which can be managed by adding an oral contraceptive.
Clinical equivalence for prolactin lowering effects of Vitex agnus-castus (Agnucaston® 40 mg per day) and the pharmaceutical Bromocriptine (Parlodel® 5 mg per day) was found in one study including 40 women with hyperprolactinaemia [63]. Mean concentrations for prolactin following three months treatment with Vitex agnus-castus was significantly reduced from 946 mIU/l (±173) to 529 mIU/l (±297) (p < 0.001). Comparatively, mean prolactin concentration in the Bromocriptine group was significantly reduced from 885 mIU/l (±178) to 473 mIU/l (±266) (p < 0.001) demonstrating that both treatments were effective treatment for women with hyperprolactinaemia (normal reference range 25-628 mIU/l). The mean difference in prolactin reduction of the two groups was not significant (p = 0.96) (Table 2).

Other laboratory tests can be helpful in making the diagnosis of PCOS. Serum levels of male hormones ( DHEA and testosterone ) may be elevated. However, levels of testosterone that are highly elevated are common with PCOS and call for additional evaluation. However, in women with PCOS receiving low doses of gonadotropins for timed intercourse, metformin administration can double the clinical pregnancy rate (OR 2.25; 95% CI: 1.50 to 3.38; p<0.001; 7 trials) and the live birth rate (OR 1.94; 95% CI: 1.10 to 3.44; p=0.020; 2 trials). Vasoactive intestinal polypeptide (VIP) stimulates estradiol and progesterone release from ovarian granulosa cells in vitro. Very little information is available as to the role VIP plays in the control of steroid secretion during reproductive cyclicity and in ovarian pathologies involving altered steroid secretion. In this study, we determined the involvement of VIP in regulating ovarian androgen and estradiol release during estrous cyclicity and estradiol valerate (EV)-induced polycystic ovarian development in rats. Our findings show that androgen and estradiol release from ovaries obtained during different stages of rat estrous cycle mimic cyclic changes in steroid release observed in vivo with maximal release occurring during late proestrus. VIP increased androgen release from ovaries of all cycle stages except late proestrus and estradiol release from all cycle stages. Increases in VIP-induced androgen and estradiol release were maximal at early proestrus. Inclusion of saturating concentrations of androstenedione increased magnitude of VIP-induced estradiol release at diestrus and estrus but not proestrus. Magnitude of VIP-induced androgen and estradiol release tended to be greater in the ovaries from EV-treated rats with polycystic ovary compared with estrous controls. At the tissue level, ovarian VIP concentration was cycle stage dependent with highest level seen in diestrus. Maximum concentration of VIP was found in EV-treated rats. Changes in VIP were inversely related to changes in ovarian nerve growth factor, a neuropeptide involved in ovarian androgen secretion. These results strongly suggest that intraovarian VIP participates in the control of estradiol secretion during the rat estrous cycle and possibly in the maintenance of increased ovarian estradiol secretory activity of EV-treated rats.

There have been many studies on PCOS in the past several years; however, most are fairly small. Also, many studies examine medication effects on surrogate markers (e.g., androgen levels) rather than clinical outcomes (e.g., hirsutism). Our laboratory search included investigations into the effects of herbal medicine using computer models, cell cultures, animals with PCOS induced with oestradiol valerate and androgens and sterilised and ovariectomised rats. We excluded laboratory studies which commenced using isolated chemicals not directly extracted from crude herbal medicines and studies examining androgen effects in male animals.

Our second search for clinical trials was performed without language restriction and included randomised controlled trials, non-randomised, open label and single arm clinical trials. We included clinical studies investigating commercially available herbal extracts and investigations that compared the effectiveness of herbal medicine with pharmaceuticals. We excluded clinical studies investigating herbal medicines with unrelated outcomes (including pre-menstrual syndrome, endometriosis and mastalgia) and clinical studies examining the effectiveness of complex herbal formulas for PCOS and associated oligo/amenorrhoea and hyperandrogenism, without demonstration of a mechanism of effect for the whole complex formula. We compared data from laboratory and animal studies with the outcomes of clinical trials. Clinical studies were assessed for risks of bias at study and outcome levels with risks summarised, tabulated (Tables 1 and ​and2)2) and presented in contextual narrative.

The diagnostic workup should begin with a thorough history and physical examination. Metformin, an antidiabetic drug, improves insulin resistance and decreases hyperinsulinemia in patients with PCOS. Why treat IR in PCOS women? For many years only PCOS women with DM were treated. As the link between IGT and CAD became more apparent, many PCOS women with IGT were treated. Those who now advocate treatment for IR do so for the following reasons: reduction of insulin and androgen levels, prevention of IGT and DM, potential for improved ovulation, symptomatic improvement, prevention of MS[63]. Ultimately, secondary prevention in young women with identifiable and treatment precursor conditions is far more desirable and easier than treatment of these same women later in life with serious disease.
