There is growing evidence that mood disturbances, mostly severe depression, are common in PCOS women , in whom impaired quality of life from body image concerns cause fatigue, sleep disturbance and changes in eating habits. In addition, many PCOS patients report feeling abnormal, unfeminine, and embarrassed due to unwanted hair, often hiding their hair growth and covering their face when talking to others. Understanding how a woman feels about her body image and improving this perception are essential components of any management plan that provides overall health care to women with PCOS.
Combination birth control pills. Pills that contain estrogen and progestin decrease androgen production and regulate estrogen. Regulating your hormones can lower your risk of endometrial cancer and correct abnormal bleeding, excess hair growth and acne. Instead of pills, you might use a skin patch or vaginal ring that contains a combination of estrogen and progestin.
We conducted two searches. The first was sensitive and aimed to capture all pre-clinical studies explaining the reproductive endocrine effects of whole herbal extracts in PCOS or associated oligo/amenorrhoea and hyperandrogenism. The second search was specific and sought only clinical studies investigating herbal medicines revealed during the first search (for which a mechanism of effect had been demonstrated). We additionally searched, on a case by case basis for pre-clinical evidence for herbal medicines identified during the second search, but not included in the results of the first search. Clinical studies were excluded based on the absence of evidence for a mechanism of effect for the whole herbal extract in reproductive endocrinology associated with PCOS, oligo/amenorrhoea and hyperandrogenism. We used this approach to improve transparency and to limit confirmation bias for herbal medicines favoured by the authors in clinical practice.
A clear primary treatment for hirsutism in women with polycystic ovarian syndrome (PCOS) remains lacking. [3] However, short-term, nonpharmacologic treatments of hirsutism include shaving and the use of chemical depilatories and/or bleaching cream. [76] Plucking or waxing unwanted hair can result in folliculitis and ingrown hairs. Long-term, more permanent measures for unwanted hairs include electrolysis and laser treatment.

The differential diagnosis of PCOS is broad and includes both endocrinologic and malignant etiologies. Figure 119 provides an algorithm for the workup of select presentations. For any woman with suspected PCOS, the Endocrine Society recommends excluding pregnancy, thyroid dysfunction, hyperprolactinemia, and nonclassical congenital adrenal hyperplasia.19 Depending on presentation, conditions such as hypothalamic amenorrhea and primary ovarian insufficiency should also be excluded. In women with rapid symptom onset or significant virilization, such as deepening voice or clitoromegaly, an androgen-secreting tumor should be ruled out. Finally, Cushing syndrome or acromegaly should be excluded in patients with physical findings that suggest either condition.19 There is no need to order laboratory testing for these conditions if the patient does not have suggestive physical findings.

Other excluded studies investigated the herbal medicines included in this review examining conditions other than PCOS, oligo/amenorrhoea and hyperandrogenism. These included investigations into effectiveness for Vitex agnus-castus for pre-menstrual syndrome [92–97] and mastalgia [98, 99], Cimicifuga racemosa for menopausal symptoms [100] and Glycyrrhiza spp with Paeonia lactiflora libido in males [101].

A prospective, observational clinical trial examined the endocrine effects of Tribulus terrestris 750 mg per day, over five days in eight healthy women (aged 28–45). A significant increase in mean serum FSH concentration from 11 mIU/ml before treatment to 17.75 mIU/ml following treatment (P < 0.001) was demonstrated. Pre-treatment FSH levels returned following cessation of treatment (Table 1). Another clinical study evaluated the equivalence of Tribulus terrestris (Tribestan®) and pharmaceuticals for ovulation induction in women with oligo/anovular infertility (n = 148), [60]. During the three month follow up, ovulation rates were highest with epimestrol (74%), followed by Tribulus terrestris (60%), clomiphene (47%) and cyclofenil (24%). However, the evidence for Tribulus terrestris should be interpreted with caution due to risks for bias in clinical studies. One study was uncontrolled with a small number of healthy participants [56], the second study did not report baseline characteristics, methods for allocation to treatment groups and data were not statistically analysed [60] (Table 1).

According to Dr. Geoffrey Redmond, an endocrinologist specializing in female hormones, “Just because the ovaries are not functioning as much doesn’t mean the other abnormalities won’t still be present.” He goes on to point out that studies show male hormone levels climb fairly sharply with age.2 This could mean a worsening of symptoms such as excess hair growth as those hormones become more active. It could also mean insulin-related issues such as diabetes and cardiovascular health could become more problematic.

Because of the menstrual and hormonal irregularities, infertility is common in women with PCOS. Because of the lack of ovulation, progesterone secretion in women with PCOS is diminished, leading to long-term unopposed estrogen stimulation of the uterine lining. This situation can lead to abnormal periods, breakthrough bleeding, or prolonged uterine bleeding. Unopposed estrogen stimulation of the uterus is also a risk factor for the development of endometrial hyperplasia and cancer of the endometrium (uterine lining). However, medications can be given to induce regular periods and reduce the estrogenic stimulation of the endometrium (see below).

Because of its antiandrogenic effects, spironolactone is effective, but not FDA-approved, for this indication.22,23 A Cochrane review suggested that spironolactone is superior to finasteride.28 Combining spironolactone with oral contraceptives may be synergistic, but caution should be used in women taking drospirenone because each agent can cause hyperkalemia.2 Spironolactone is FDA pregnancy category C.
Treatment of PCOS is individualized and depends on whether or not pregnancy is being sought. Dietary modifications, weight management and regular exercise are important factors in the management of this condition in all women with PCOS, regardless of whether they are trying to conceive or not. Cosmetic treatment options to treat excess body hair, particularly facial hair, include electrolysis, laser treatment and typical medication which act at the hair follicle. There are medical and surgical options to treat the hormone abnormalities associated with PCOS. For women who are not trying to conceive, medical treatment options include oral contraceptives, progestational agents (that induce periods), and drugs that block the production or action of androgens. In some cases, surgery is performed to cauterize the cysts, which results in a decrease in male hormone levels and return of ovulation in some women. Insulin modifiers are useful in those women with high insulin levels and insulin resistance but do not benefit all women with PCOS. The safety of these medications in pregnancy has not been established.

In IVF, your egg is fertilized with your partner's sperm in a laboratory and then placed in your uterus to implant and develop. Compared to medicine alone, IVF has higher pregnancy rates and better control over your risk of having twins and triplets (by allowing your doctor to transfer a single fertilized egg into your uterus).
