What matters most: Your goal when losing weight is to aim specifically to reduce the fat around your abdomen (belly fat). Two laboratory based RCT’s examined the effects of Tribulus Terrestris in rats with polycystic ovaries induced with oestradiol valerate [46, 47] (Table 1). Both studies demonstrate significantly improved ovulation rates for animals treated with two doses of Tribulus terrestris extracts compared to controls. Although the endocrinological effects were not described in either study, laboratory findings of ovulation induction are supported by the clinical findings of elevated FSH following treatment with Tribulus terrestris[56] (Table 2).

Metformin therapy for 10 mo decreased fasting serum insulin levels in obese girls with PCOS[74].The positive effects of metformin in adolescents wore off within 3 mo of medication discontinuation[75]. Other studies have found non-significant trends to improved IR by HOMA and OGTT-AUC in adolescent PCOS patients[78]. Metformin has also been shown to effectively contribute to BMI reduction in PCOS adolescents[79].
Side effects: Red clover has been associated with headaches, nausea, vaginal bleeding, muscle ache and rash. Do not take this herb if you have conditions that are worsened by estrogen exposure such as endometriosis, breast cancer, ovarian cancer or if you are pregnant or breastfeeding. Red clover might also increase the chance of bleeding so avoid this herb if you have any kind of bleeding disorder.
Obesity is associated with PCOS. Obesity not only compounds the problem of insulin resistance and type 2 diabetes (see below), but also imparts cardiovascular risks. It has also been shown that levels of C-reactive protein (CRP), a biochemical marker that can predict the risk of developing cardiovascular disease, are elevated in women with PCOS. Reducing the medical risks from PCOS-associated obesity is important.
Insulin-sensitizing drugs. A particular medication, called metformin, is usually prescribed.20 Another medication, pioglitazone (Actos) may also be suggested for women with PCOS. These two medications are FDA-approved for diabetes treatment, but plenty of research shows they can be equally effective for women with PCOS, too. These insulin-sensitizing medications can help your body respond more readily to insulin, and better control your glucose levels. Both metformin and pioglitazone can reduce the insulin resistance and high insulin levels that commonly occur with PCOS, and in turn, can reduce high androgen levels.
This month, a groundbreaking study was published in the journal Nature Medicine that might have found a treatment for this disorder. The authors’ goal was to determine if something in the environment of the womb could be causing PCOS. Therefore, glucose levels alone lack the sensitivity to predict metabolic risk in PCOS patients. Precursor states of insulin abnormalities likely predict long term CAD risk well before glucose abnormalities. IR can be just as severe in diabetics and non-diabetics[27], stressing the seriousness of this metabolic impairment as a precursor and not a separate disease. Animal models have shown that IR alone damages myocardial cells, providing direct evidence of end organ disease[28]. Human data link HOMA-IR to left ventricular dysfunction[29]. Abnormal glucose metabolism short of IGT and DM still deserves attention, identification and treatment[30].

You heard that right. But first, don’t believe what you may have heard about special diets for polycystic ovary syndrome. Some women lose more weight and feel better on a higher-protein, lower-carbohydrate plan, but the research isn’t conclusive. Remember, the Penn State researchers found a low-calorie, low-fat meal replacement diet proved successful for some women but there is no one perfect diet that will be right for everyone.8 In fact, when researchers from Australia’s University of Adelaide and Monash University reviewed five PCOS diet studies involving 137 women, they concluded that PCOS symptoms improved on any diet the women followed.9

PCOS women with different phenotypes have been found similarly insulin resistant in response to a 3 h 75 g OGTT[31]. Obese (compared to lean) PCOS women tend to have a higher degree of IR. Correlation between hyperandrogenism and IR is significant in many studies but not as significant as the link between insulin abnormalities and obesity[32]. PCOS women demonstrate greater variation in insulin parameters compared to controls, independent of weight[33]. Animal studies of prenatal testosterone exposure show downstream IR in early postnatal life[34].
