Some have tried to utilize ultrasound to detect IR. Of note, normoglycemic women often have the phenotypic criteria for polycystic ovaries on ultrasound[54], consistent with other data in young adolescents showing that polycystic ovaries by ultrasound appearance often does not correlate with either anovulatory menstrual cycles or metabolic abnormalities[55]. Therefore ultrasound is too non-specific to use with any reliability in measuring IR.
A team approach involving care by primary care and subspecialist physicians can be helpful to address the multiple manifestations of the syndrome. Goals for treatment (e.g., treating infertility; regulating menses for endometrial protection; controlling hyperandrogenic features, including hirsutism and acne) must account for the patient's preferences because therapy selection may otherwise conflict with outcomes that the patient considers important. Metabolic complications should be addressed in every patient via a blood pressure evaluation, a lipid panel, and a two-hour oral glucose tolerance test. Patients who are overweight should be evaluated for signs and symptoms of obstructive sleep apnea. All patients should be screened for depression (Figure 119).
Treatment of PCOS is individualized and depends on whether or not pregnancy is being sought. Dietary modifications, weight management and regular exercise are important factors in the management of this condition in all women with PCOS, regardless of whether they are trying to conceive or not. Cosmetic treatment options to treat excess body hair, particularly facial hair, include electrolysis, laser treatment and typical medication which act at the hair follicle. There are medical and surgical options to treat the hormone abnormalities associated with PCOS. For women who are not trying to conceive, medical treatment options include oral contraceptives, progestational agents (that induce periods), and drugs that block the production or action of androgens. In some cases, surgery is performed to cauterize the cysts, which results in a decrease in male hormone levels and return of ovulation in some women. Insulin modifiers are useful in those women with high insulin levels and insulin resistance but do not benefit all women with PCOS. The safety of these medications in pregnancy has not been established.
The unfortunate truth is that there is no cure for PCOS. But because the disorder can have a profound effect on your health and well-being, treatment is a must. There is one important thing to know about your options: Because the cause of PCOS is still unknown, appropriate therapies are based on a woman’s individual concerns. Is she bothered most about excess hair growth? Does she want to have a baby? Does she need to get her periods back on track?
Polycystic ovary syndrome (PCOS) is a condition in which the ovaries produce an abnormal amount of androgens, male sex hormones that are usually present in women in small amounts. The name polycystic ovary syndrome describes the numerous small cysts (fluid-filled sacs) that form in the ovaries. However, some women with this disorder do not have cysts, while some women without the disorder do develop cysts.
Polycystic ovarian syndrome is classified as a disorder which encompasses a group of symptoms that collectively indicate a distinct health issue. This means that the symptoms associated with PCOS will vary from woman to woman. Your PCOS experience is probably very different from another woman with this condition but that does not mean you are alone! PCOS is a condition which can affect between 5 – 10% of all women and is the most common cause of infertility.8 You may be wondering how you ended up in this group of women but unfortunately the precise reason polycystic ovarian syndrome develops isn’t completely clear. 8 Having said that, many experts believe insulin as well as hormone imbalances have a strong influence on PCOS and the severity of its symptoms.

Insulin-sensitizing agents, including metformin,31 acarbose (Precose),24 and rosiglitazone (Avandia),20 may be used to treat hirsutism in women with PCOS. Spironolactone22 and rosiglitazone32 have been shown to be more effective than metformin, based on Ferriman-Gallwey hirsutism scores. A Cochrane review suggested that metformin is as effective as oral contraceptives for treating hirsutism in women with PCOS,33 but in contrast, a recent systematic review suggested that metformin is not effective.1 Topical eflornithine cream is FDA-approved for management of unwanted facial hair, but there are no published data regarding its use specifically in women with PCOS. Sibutramine (Meridia), which is approved for obesity management, can also improve hirsutism.26
Treatments for hirsutism in women with PCOS are similar to those in women without PCOS, such as patients with idiopathic hirsutism. There are many nonpharmacologic treatment options, including electrolysis, waxing, bleaching, plucking, depilatory creams (a form of hair removal that dissolves the hair), thermolysis (use of heat), and laser therapy. Several medications have been studied for the treatment of hirsutism in women with PCOS. First-line agents include spironolactone (Aldactone)22,23,28–30 and metformin,13,16,20,22,31–33 as well as eflornithine (Vaniqa) for facial hirsutism.9

Where US is available, CC treatment should be initiated between the third and fifth day of the menstrual cycle and the couple should abstain from intercourse (this is not a mandatory measure) until the tenth day of the cycle (when the presence of dominant follicles with a mean diameter of 10 mm or more is assessed via US). Sexual activity is allowed if the patient presents with monofollicular or bifollicular development. The goal of sexual abstinence until the tenth day of the cycle is to minimize the risk for multiple gestations.
Other tests that may be helpful but are not necessary for diagnosis include measurement of LH and follicle-stimulating hormone (FSH) levels to determine a serum ratio of LH/FSH. A ratio greater than 2 generally indicates PCOS, but there are no exact cutoff values because many different assays are used.26 The FSH level is more helpful in ruling out ovarian failure.26

This is an important distinction; given her longtime love for drugs, tendency to abuse everything she gets, and unwillingness to go sober even when she's not on H, there's a good chance that Luna has 'polysubstance dependence' and is chasing the feeling of being 'not sober' by abusing her meds. We undertook two searches of the scientific literature. The first search sought pre-clinical studies which explained the reproductive endocrine effects of whole herbal extracts in oligo/amenorrhoea, hyperandrogenism and PCOS. Herbal medicines from the first search informed key words for the second search. The second search sought clinical studies, which corroborated laboratory findings. Methadone is said to not get people "high," but I beg to differ. It doesn't give any of the euphoria that heroin does, nor the rush, but it definitely has a body high to it. Fasting methods to measure IR have been advocated for many years as an adjunct to DM screening. Elevated fasting insulin levels greater than 20 μU/mL may alone indicate IR. Fasting glucose/insulin ratio (G/I) has also gained some clinical traction. A ratio < 4.5 has in general been shown to be > 90% sensitive in some populations[45] but has never been validated with clamp studies[48]. Some ethnic variation in G/I cutoff ratios may exist[49]. There has been some suggestion that G/I < 7 in very young girls may predict IR[50,51].

Medications for PCOS include oral contraceptives and metformin. The oral contraceptives increase sex hormone binding globulin production, which increases binding of free testosterone. This reduces the symptoms of hirsutism caused by high testosterone and regulates return to normal menstrual periods. Metformin is a medication commonly used in type 2 diabetes mellitus to reduce insulin resistance, and is used off label (in the UK, US, AU and EU) to treat insulin resistance seen in PCOS. In many cases, metformin also supports ovarian function and return to normal ovulation.[74][78] Spironolactone can be used for its antiandrogenic effects, and the topical cream eflornithine can be used to reduce facial hair. A newer insulin resistance medication class, the thiazolidinediones (glitazones), have shown equivalent efficacy to metformin, but metformin has a more favorable side effect profile.[79][80] The United Kingdom's National Institute for Health and Clinical Excellence recommended in 2004 that women with PCOS and a body mass index above 25 be given metformin when other therapy has failed to produce results.[81][82] Metformin may not be effective in every type of PCOS, and therefore there is some disagreement about whether it should be used as a general first line therapy.[83] The use of statins in the management of underlying metabolic syndrome remains unclear.[84]
