Reglan Active ingredient: metoclopramide $0.37 for pill Reglan is used for short-term treatment of gastroesophageal reflux disease (GERD) in certain patients who do not respond to other therapy. It is used to treat symptoms of a certain digestive problem in diabetic patients (diabetic gastroparesis). Prilosec Active ingredient: omeprazole $0.49 for pill Prilosec relieves symptoms of gastroesophageal reflux disease decreasing the amount of acid in the stomach.
Often, women with PCOS tend to put on weight easily and have difficulty losing weight. This is because women with PCOS are thrifty with calories. If they eat too much, extra insulin is needed to dispose of the extra sugar. Sadly, this tendency can also increase the risk of diabetes. This is more likely in those with a family history of adult-onset diabetes. Women with PCOS are more likely to develop high blood pressure and diabetes during pregnancy, particularly if they are overweight and have higher than normal insulin levels.
A pelvic ultrasound (transvaginal and/or pelvic/abdominal) is used to evaluate enlarged ovaries. Ultrasounds are often used to look for cysts in the ovaries and to see if the internal structures appear normal. In PCOS, the ovaries may be 1.5 to 3 times larger than normal and characteristically have more than 12 or more follicles per ovary measuring 2 to 9 mm in diameter. Often the cysts are lined up on the surface the ovaries, forming the appearance of a "pearl necklace." The follicles tend to be small and immature, thus never reaching full development. The ultrasound helps visualize these changes in more than 90% of women with PCOS, but they are also found in up to 25% of women without PCOS symptoms. Combination birth control pills — those with estrogen and Prometrium (progesterone) — are frequently prescribed to women with PCOS not looking to get pregnant. If the main concern is irregular periods and the resulting potential health risks, this is a great option. “Birth control pills are very good for protecting the lining of the uterus in women who are chronically anovulatory,” says Dunaif.
Lifestyle interventions are usually required for long term sustainable results. PCOS women who smoke have higher free androgen levels and IR as measured by HOMA-IR, QUICKI and the insulin sensitivity index following 75 g OGTT. Thus PCOS women who smoke have an additional reason to stop smoking. In more general population studies (non-PCOS) comprised mostly of middle-aged women, lifestyle intervention is more effective than metformin in preventing the progression to DM. Dietary and exercise intervention decreased the 4 year progression to DM in patients at risk (non-diabetic, elevated fasting and/or OGTT glucose) by almost 50%. Realizing the limitations of applying this population sample to young PCOS women, it still highlights the benefit of non-pharmacological treatment. PCOS women randomized to both metformin and lifestyle interventions (compared to placebo) showed improvements in HOMAIR after 4 mo. In European adolescents with PCOS who failed to achieve improvements in HOMA-IR after 6 mo of lifestyle intervention, both metformin and placebo reduced IR over 6 mo, although metformin offered no benefit over placebo. Lifestyle modification in adolescents has been successful in reducing hyperandrogenism. Modest weight loss of about 5% bodyweight has also been shown to lower hyperandrogenism which may ultimately improve IR.
There have been many studies on PCOS in the past several years; however, most are fairly small. Also, many studies examine medication effects on surrogate markers (e.g., androgen levels) rather than clinical outcomes (e.g., hirsutism). The study results are often conflicting, and in a recent systematic review, only 33 of 115 possible studies met basic inclusion criteria (e.g., randomized controlled trials), suggesting that many of the data in the literature may have methodologic flaws.1
Other pharmacological treatments have attempted to lower IR. Vitamin D has been shown to decrease HOMA-IR despite a lack of change in hyperandrogenism in young, obese PCOS women. Animal studies have demonstrated that treatment with glycyrrhizic acid affecting lipoprotein lipase activity decreases serum insulin and HOMA-IR. Although oral contraceptive pills positively affect hyperandrogenism, they have little to no effect on glucose metabolism by OGTT. Long term oral contraceptive pill use may have some limited benefit in IR but data are limited. A 6 mo course of oral contraceptive pill treatment in adolescent obese PCOS women has demonstrated some improvement in IR.

I worded that badly. What I mean is that opiates themselves don't relieve pain, but alter the perception of pain. It's possible to alter that perception of pain in other ways, which is why substances like SSRIs cause significant pain relief for some people with some conditions. Ditto with anticonvulsants. Opioids aren't the only drugs that affect norepinephrine and mU receptors, they just happen to be the best at it. If the pleasurable effects of opioids were blocked, there's still therapeutic potential in the "inert" components, like THC vs. CBD.
This review has some limitations. We used a methodological approach which was deductive and not consistent with traditional rationale for herbal selection. Our inclusion criteria for clinical studies were specific and relied upon our identification of herbal medicines with preclinical (laboratory based) evidence explaining the mechanisms of reproductive endocrinological effects in oligo/amenorrhoea, hyperandrogenism and PCOS. Clinical studies were excluded from this review due to the absence of evidence for whole herbal extracts. This was the case for Camellia sinensis (green tea) for which only one laboratory study investigated the effects of injecting epigallocatechin, a catechin found in green tea in animals . High quality clinical evidence for Camellia sinensis was not presented in this review due to the absence of pre-clinical data explaining the mechanism for effect for the whole herbal extract . Mentha spicata (spearmint) was another herbal medicine excluded from this review despite the availability of high quality clinical evidence demonstrating testosterone lowering effects in women with PCOS . We found no laboratory evidence describing the mechanism of action for Mentha spicata in hyperandrogenism. Camilla sinensis and Mentha spicata are examples of herbal medicines excluded from this review due to not meeting the inclusion criteria. Studies investigating western herbal medicines excluded from this review are provided in Tables 3, ,44 and and55.

For assisted reproduction cycles, metformin use prior to or during ovarian stimulation with gonadotropins in IVF/ICSI cycles is also not associated with better clinical pregnancy or live birth rates; however, metformin may reduce the risk of OHSS 38,39 and miscarriage and improve the implantation rate because metformin may act directly on the endometrium 39 and promote better reproductive outcomes (data not confirmed) in women with PCOS 40. However, as previously mentioned, the use of a GnRH antagonist combined with ovarian stimulation with gonadotropins in women with PCOS and the induction of final ovarian maturation with a GnRH agonist with subsequent embryo cryopreservation are more effective strategies to prevent OHSS regardless of metformin use 33. Thus, the routine use of metformin in cycles of ovarian stimulation for IVF in women with PCOS is not recommended except in the presence of a disorder in glucose metabolism 9.

A 2017 review concluded that while both myo-inositol and D-chiro-inositols may regulate menstrual cycles and improve ovulation, there is a lack of evidence regarding effects on the probability of pregnancy. A 2012 and 2017 review have found myo-inositol supplementation appears to be effective in improving several of the hormonal disturbances of PCOS. Myo-inositol reduces the amount of gonadotropins and the length of controlled ovarian hyperstimulation in women undergoing in vitro fertilization.A 2011 review found not enough evidence to conclude any beneficial effect from D-chiro-inositol. There is insufficient evidence to support the use of acupuncture.
Metformin has been studied specifically in adolescent PCOS women. Metformin therapy for 10 mo decreased fasting serum insulin levels in obese girls with PCOS.The positive effects of metformin in adolescents wore off within 3 mo of medication discontinuation. Metformin in obese PCOS adolescents has shown improvements in IR by clamp studies, fasting measurements and OGTT after just 3 mo of therapy[76,77]. Other studies have found non-significant trends to improved IR by HOMA and OGTT-AUC in adolescent PCOS patients. Metformin has also been shown to effectively contribute to BMI reduction in PCOS adolescents.
A malfunction of the body's blood sugar control system (insulin system) is frequent in women with PCOS, who often have insulin resistance and elevated blood insulin levels. Researchers believe that these abnormalities may be related to the development of PCOS. It is also known that the ovaries of women with PCOS produce excess amounts of male hormones known as androgens. This excessive production of male hormones may be a result of or related to the abnormalities in insulin production.

Patients with PCOS who are infertile but desire pregnancy should be referred to a reproductive endocrinologist for further evaluation and management of infertility. Morbidly obese women with PCOS should also be referred for pregnancy risk  ; metabolic surgery may be considered in morbidly obese women with PCOS, because many features of this syndrome are reversible with successful weight loss. In vitro fertilization (IVF) is reserved for women with PCOS and unsuccessful gonadotropin therapy or those with other indications for this procedure. 
From a practical standpoint, the use of aromatase inhibitors may be an option before IVF/ICSI after counseling and the consent of the couple in specific cases of women with CC-resistant PCOS without other infertility factors and for whom the high-complexity treatment is cost-prohibitive 41. The recommended dose of letrozole is 5 to 10 mg/day for 5 to 10 days.
Other tests that may be helpful but are not necessary for diagnosis include measurement of LH and follicle-stimulating hormone (FSH) levels to determine a serum ratio of LH/FSH. A ratio greater than 2 generally indicates PCOS, but there are no exact cutoff values because many different assays are used.26 The FSH level is more helpful in ruling out ovarian failure.26
When the former NFL cheerleader Natalie Nirchi stopped menstruating at age 17, she was diagnosed with polycystic ovary syndrome (PCOS), a hormone disorder affecting up to 10 percent of women of reproductive age. She didn’t initially show any of the physical symptoms, like excess hair growth, cystic acne, or obesity, but a blood test revealed that she had high levels of testosterone and an ultrasound showed cysts on her ovaries.
Insulin resistance is a condition in which the body’s cells do not respond to the effects of insulin. When the body does not respond to insulin, the level of glucose in the blood increases. This may cause more insulin to be produced as the body tries to move glucose into cells. Insulin resistance can lead to diabetes mellitus. It also is associated with acanthosis nigricans.
One study indicates that caloric intake timing can have a big impact on glucose, insulin and testosterone levels. Lowering insulin could potentially help with infertility issues. Women with PCOS who ate the majority of their daily calories at breakfast for 12 weeks significantly improved their insulin and glucose levels as well as decreased their testosterone levels by 50 percent, compared to women who consumed their largest meals at dinnertime. The effective diet consisted of a 980-calorie breakfast, a 640-calorie lunch, and a 190-calorie dinner.
Once a diagnosis of PCOS is confirmed, it is imperative to assess women for CAD risk factors. Despite the many reasons women seek medical care for PCOS, the greatest long term risk for these women is CAD. This is generally not viewed or even recognized as a concern by women seeking care in the first place. The link between PCOS and CAD is multi-faceted. C-reactive protein (CRP) is higher in age matched PCOS women and is linked to BMI with some ethnic variation in this risk. The prevalence of MS in PCOS women is as high as 40% with increased prevalence of hypertension, dyslipidemia and abnormal glucose metabolism, all before age 30. PCOS women aged 20-40 already demonstrate poor vascular function measured by brachial artery vascular flow. No single blood test can predict or quantify this CAD risk. Although no standard recommendation for assessment of CAD risk factors exists, measurement of glucose metabolism, blood pressure screening, lipid screening and carotid intimal media thickness measurements have been suggested.
