More research is needed to clarify the complex pathophysiology of PCOS. No single test is currently available for its diagnosis. Additionally, once diagnosis is established, the options for treatment are of limited number and effectiveness because they target only the symptoms of PCOS. Finally, patients with PCOS have higher rates of metabolic complications, such as cardiovascular disease, but their impact on mortality is not clear. Therefore, more prospective epidemiologic studies on the topic are necessary.
Combination birth control pills: Oral contraceptives (OCs) containing estrogen plus progestin help you manage PCOS symptoms by normalizing your menstrual cycle. By reducing levels of male hormones, OCs can help control excess hair growth and acne, too. You’ll usually shed the lining of your uterus every 28 days while taking an oral contraceptive, which also reduces your risk for endometrial cancer. Oral contraceptive pills are sometimes prescribed for women before they begin fertility treatment because these female hormones reduce the “male” androgen levels. But it’s important to know that they won’t help restore ovulation.19

If the patient does not present with ovulatory cycles at three months after ovarian drilling, then the procedure should be combined with CC treatment. The use of gonadotropins should be considered after 6 months of anovulatory cycles following the ovarian drilling procedure. Ovarian drilling should not be indicated as a treatment for menstrual irregularity, metabolic complications or hyperandrogenism in PCOS 29.
Three RCTs corroborate the positive fertility effects for Cimicifuga racemosa in women with PCOS, used in conjunction and when compared with the pharmaceutical Clomiphene citrate (clomiphene), [65, 68, 71] (Table 2). Results were reported for 441 women and show improved pregnancy rates when Cimicifuga racemosa was added to clomiphene during one menstrual cycle. In a study including women with PCOS (n = 147), pregnancy rates for the group receiving combined therapy (clomiphene 150 mg plus Cimicifuga racemosa 20 mg per day (Klimadynon®)) were 43.3% compared to 20.3% for women receiving only clomiphene  (Table 2). In another study using similar methodology (n = 100) pregnancy rates were 34.8% for the group treated with Cimicifuga racemosa plus clomiphene compared to 17.2% for women treated with clomiphene alone  (Table 2). Another study included women with PCOS and infertility (n = 100) compared Cimicifuga racemosa (Klimadynon®) and clomiphene over three months for hormone concentrations and pregnancy rates. Pregnancy rates were higher in the women in taking Cimicifuga racemosa compared to clomiphene, 14% and 8% respectively; however differences were not statistically significant. This study found significant effects for lowered luteinising hormone for women with PCOS receiving Cimicifuga racemosa compared to clomiphene (p = 0.007) . Findings from clinical studies concur with laboratory and animal studies; however potential risks for bias include performance and collection bias due to lack of blinding (Table 2).
PCOS is a heterogeneous disorder of uncertain cause. There is some evidence that it is a genetic disease. Such evidence includes the familial clustering of cases, greater concordance in monozygotic compared with dizygotic twins and heritability of endocrine and metabolic features of PCOS. There is some evidence that exposure to higher than typical levels of androgens in utero increases the risk of developing PCOS in later life.

The catch is that it’s not safe in pregnancy, as it can cross the placenta and harm a fetus. For that reason, doctors prescribe spironolactone along with combination estrogen-progesterone birth control pills. If a woman doesn’t want to take these, they have other options, like a progesterone-releasing intrauterine device (IUD). The benefit of using combination birth control pills and spironolactone is that they work even better together, she says.
Glucose tolerance testing (GTT) instead of fasting glucose can increase diagnosis of impaired glucose tolerance and frank diabetes among people with PCOS according to a prospective controlled trial. While fasting glucose levels may remain within normal limits, oral glucose tests revealed that up to 38% of asymptomatic women with PCOS (versus 8.5% in the general population) actually had impaired glucose tolerance, 7.5% of those with frank diabetes according to ADA guidelines.
You heard that right. But first, don’t believe what you may have heard about special diets for polycystic ovary syndrome. Some women lose more weight and feel better on a higher-protein, lower-carbohydrate plan, but the research isn’t conclusive. Remember, the Penn State researchers found a low-calorie, low-fat meal replacement diet proved successful for some women but there is no one perfect diet that will be right for everyone.8 In fact, when researchers from Australia’s University of Adelaide and Monash University reviewed five PCOS diet studies involving 137 women, they concluded that PCOS symptoms improved on any diet the women followed.9
My brother was a Heroin addict who started taking Benzos between his shooting up, like Luna, versus the other way around. He was an addict, he took benzos to keep up whatever feeling he was chasing. Thats inherently different than an anxiety or depression patient taking them for relief of symptoms. Im sure it does happen, there's always that risk but I wouldn't go out there and say that Benzo use is always a gateway.
my experiences of being arrested for heroin were in manhattan and nassau county, not the bronx but I'm sure it's the same out there. there are so many heroin arrests & arrests in general in metro NY. possession just isn't important, especially since Lurch doesn't have any open cases besides that one. they would never take something like that to trial.

130mg of methadone?!!!! That's 520mg morphine. That's an insane dosage even for someone with tolerance - I have been on high dose opioids for pain for a decade and I'm scraping 250mg morphine equivalent dose. For context: the equivalent fentanyl patch would be 250mcg per hour strong. Even people with severe pain from tumours get by on no more than 100-150mcg per hour.
We used the following definitions. PCOS according to the Rotterdam diagnostic criteria, specified by the presence of two out of three features; oligo/amenorrhoea, hyperandrogenism and polycystic ovaries on ultrasound [29, 30]. Associated endocrine features for PCOS included elevated LH , which is strongly associated with infertility (p = 0.0003)  and miscarriage  and elevated fasting glucose which is prevalent in approximately 31% of women with PCOS including normal weight women .

Couples with infrequent sexual intercourse may experience some benefit from the use of kits for ovulation monitoring (urinary luteinizing hormone excretion); however, this technique can underestimate the fertile window. The evaluation of cervical mucus throughout the menstrual cycle demonstrated similar efficacy to urinary kits for monitoring the ovulation and high rates of false positives in cycles are noted using the hCG 24. Thus, this method has not been routinely used in clinical practice, mainly when US is available.
Something that really strikes me about Luna is that she's completely vacuous. There's nothing going on behind the eyes that isn't related to aesthetic. She barely even seems like a real human being. This was true even before she got hooked on drugs and probably a big factor in why she chose to use. It doesn't seem like she was a bad person prior to heroin (though iirc in http://witchycrankypoo.tumblr.com/ someone mentioned that she was super manipulative and self-focused even before the drugs), but she's certainly always been very, very, very, very shallow.
Where PCOS is associated with overweight or obesity, successful weight loss is the most effective method of restoring normal ovulation/menstruation, but many women find it very difficult to achieve and sustain significant weight loss. A scientific review in 2013 found similar decreases in weight and body composition and improvements in pregnancy rate, menstrual regularity, ovulation, hyperandrogenism, insulin resistance, lipids, and quality of life to occur with weight loss independent of diet composition. Still, a low GI diet, in which a significant part of total carbohydrates are obtained from fruit, vegetables, and whole-grain sources, has resulted in greater menstrual regularity than a macronutrient-matched healthy diet.
