Mean platelet volume (MPV) is a marker associated with adverse cardiovascular events, and women with newly diagnosed PCOS appear to have significantly elevated MPV levels. [47] Kabil Kucur et al reported that use of ethinyl estradiol/cyproterone acetate or metformin for the treatment of women with PCOS seemed to have similar beneficial effects in reducing MPV. [47]

In anovulatory women with PCOS defined according to the Rotterdam consensus (includes all phenotypes except the one defined by the association of hyperandrogenism with ultrasound (US) findings), CC treatment is the first choice for ovulation induction 9,15. This drug is an estrogen receptor modulator (it can act as an estrogen agonist or antagonist) and its mechanism of action is controversial but can be explained as follows. The prevalence of infertility in women with PCOS varies between 70 and 80%. Notably, if a patient is resistant to this drug and/or requires the use of gonadotropins and/or presents with other causes of infertility, a tubal patency evaluation becomes mandatory prior to initiating the therapeutic treatment of infertility 8.

I thought that too at first (i've been following her since she was like 16, unfollowed for a while, then when i refollowed came back to grimy heroin luna). It's because her hair is longer and she's gotten thinner. That's really it. Everything else from her youth looks so much better. The light in her eyes, the freckles, the friendships, the cute babydoll dresses… She just looked happier…. This work attempts to give a review of ovarian innervation, the mechanism of regulation of nerve activity and the role of the sympathetic activity in ovarian pathologies affecting reproductive function. We provide a succinct outline of the findings of our group in this area. The participation of stress as an etiological factor for ovarian pathologies throughout animal models and preliminary data in patients with polycystic ovary syndrome give strong support for a participation of sympathetic nerves in the ovary function both in normal and pathological states.
Clinical trials have shown that metformin can effectively reduce androgen levels, improve insulin sensitivity, and facilitate weight loss in patients with PCOS as early as adolescence. [55, 56, 57, 58] One study concluded that the use of metformin throughout pregnancy was associated with a 9-fold decrease in gestational diabetes in women with PCOS. [59] In addition to having the potential to reduce gestational diabetes in pregnant women with PCOS, metformin may also reduce the risk of preeclampsia in this population. [60]
Positive effects for Vitex agnus-castus in oligo/amenorrhoea and infertility was demonstrated in three placebo controlled RCTs [61, 62, 64]. In a study including women with menstrual irregularity and infertility (n = 96), menstrual cyclicity was significantly improved for women treated with Vitex agnus-castus (Mastodynon® 30 drops per day for three months) compared to placebo (p = 0.023) [61] (Table 2). Another study, including women with sub fertility (n = 67), showed improved menstrual cyclicity for a sub-group of women with oligomenorrhoea following treatment with Vitex agnus-castus (Phyto-Hypophyson® 7.5 ml per day) compared to placebo, (p = 0.023) [62] (Table 2). A third study including women with hyperprolactinaemia (n = 37) demonstrated improved menstrual cyclicity by an increased average number of luteal days from 3.4 days (±5.0) to 10.5 days (±4.3) (p < 0.005) following treatment with Vitex agnus-castus (Strotan® 20 mg per day) for three months. The placebo group reported average number of days in the luteal phase was 3.4 (±5.1) at baseline and 5.5 (±5.2) at three months, which was not significant (p = 0.22) [64] (Table 2). Methodological shortcomings included not reporting baseline characteristics for subgroups and small sample sizes; however clinical outcomes demonstrated physiological effects consistent with laboratory and animal findings (Tables 1 and ​and22).

Androgen lowering effects for Glycyrrhiza spp. have been demonstrated in one laboratory study examining hormone concentration in female rats (Glycyrrhiza uralensis), [53] and corroborated in two clinical trials, one including healthy women [55] and the other including women with PCOS (Glycyrrhiza glabra) [54] (Table 1). The animal study reported significantly reduced free and total testosterone and increased oestradiol in sterilised rats and no hormonal changes in oophrectomised rats. The authors conclude that the hormonal effects occurred primarily in the ovary via enhanced aromatisation of testosterone to 17-beta oestradiol. The investigators also observed significantly increased oestradiol. There were no changes to FSH or LH in androgen sterilised or oophrectomised rats [53].

All women make testosterone, but if you have PCOS, your ovaries make a little bit more testosterone than they are supposed to. Skin cells and hair follicles can be extremely sensitive to the small increases in testosterone found in young women with PCOS.
However, in women with PCOS receiving low doses of gonadotropins for timed intercourse, metformin administration can double the clinical pregnancy rate (OR 2.25; 95% CI: 1.50 to 3.38; p<0.001; 7 trials) and the live birth rate (OR 1.94; 95% CI: 1.10 to 3.44; p=0.020; 2 trials). Moreover, this practice can reduce the cancellation rate due to ovarian hyperresponsiveness by approximately 60% (OR 0.41; 95% CI: 0.24 to 0.72; p=0.002; 7 trials), the number of days of stimulation (mean difference (MD)=-3.28; 95% CI: -6.23 to 0.32; p=0.030; 6 trials) and the dose of gonadotropins (MD=-306.62; 95% CI: -500.02 to -113.22; p=0.002; 7 trials) in low-complexity cycles. However, the use of metformin is not related to a reduction in the multiple pregnancy rate (OR 0.32; 95% CI: 0.08 to 1.23; p=0.100; 3 trials), a change in the miscarriage rate (OR 0.47; 95% CI: 0.14 to 1.54; p=0.210; 5 trials) or OHSS (OR 0.56; 95% CI: 0.26 to 1.21; p=0.140; 5 trials). Notably, no conclusive data are available on the appropriate dose and time (pre-treatment or during gonadotropin treatment) for the use of metformin during timed intercourse with gonadotropins 37.
Surgery. Surgery is also an option, usually only if the other options do not work. The outer shell (called the cortex) of ovaries is thickened in women with PCOS and thought to play a role in preventing spontaneous ovulation. Ovarian drilling is a surgery in which the doctor makes a few holes in the surface of your ovary using lasers or a fine needle heated with electricity. Surgery usually restores ovulation, but only for 6 to 8 months.
