Something that really strikes me about Luna is that she's completely vacuous. There's nothing going on behind the eyes that isn't related to aesthetic. She barely even seems like a real human being. Mastodynon® additionally contains herbal extracts of Caulophyllum thalictroides, Lilium majus, Cyclamen, Ignatia and Iris. A malfunction of the body's blood sugar control system (insulin system) is frequent in women with PCOS, who often have insulin resistance and elevated blood insulin levels. It is also known that the ovaries of women with PCOS produce excess amounts of male hormones known as androgens. This excessive production of male hormones may be a result of or related to the abnormalities in insulin production.
If a woman's weight is excessive, the physician should be aggressive in championing a weight-loss program. Medications effective for weight loss (in addition to lifestyle modifications) that have been specifically studied in women with PCOS include metformin, acarbose, sibutramine, and orlistat (Xenical). Metformin is probably the first-line medication for obesity or weight reduction in patients with PCOS. Metformin results in a decrease in body mass index (BMI) of 1 to 2 kg per m2 or weight loss up to 6 lb, 10 oz to 8 lb, 13 oz (3 to 4 kg)10,46,49; acarbose results in an approximate 3 kg per m2 decrease in BMI24; sibutramine results in a decrease in BMI of 5.8 kg per m2 and weight loss of 31 lb, 11 oz (14.4 kg)26; and orlistat results in weight loss of approximately 11 lb (5 kg).50 However, a recent systematic review suggested that metformin is not effective for lowering BMI in patients with PCOS.1

For women who desire pregnancy, a medication called clomiphene (Clomid) can be used to induce ovulation (cause egg production). In addition, weight loss can normalize menstrual cycles and often increases the possibility of pregnancy in women with PCOS. Other, more aggressive, treatments for infertility (including injection of gonadotropin hormones and assisted reproductive technologies) may also be required in women who desire pregnancy and do not become pregnant on Clomid therapy.
Regardless of what reasons women have for seeking diagnosis and treatment of PCOS, it is imperative for practitioners to assess a woman’s risk for CAD. Assessment should probably be made in all PCOS patients regardless of BMI. Especially in young women or adolescents, IR may be the first identifiable risk factor. Practitioners must recognize that no universal test for IR exists and must use good clinical judgment to assess metabolic status in women. Stimulated testing with OGTT may be more sensitive than fasting measurements. Women who demonstrate IR should be counseled on lifestyle modifications. Physicians should discuss with their patients a target BMI that is realistically obtainable. It is often advisable for patients to seek nutritional assessment and counseling to help with this goal. In many individuals, consideration should be given to pharmacological treatment. Although the most commonly used medication is metformin, other medications may be appropriate first line therapy, especially in women not actively seeking pregnancy.

A study supervised by Columbia University School of Nursing professor Nancy Reame, MSN, PhD, FAAN, and published in the Journal of Behavioral Health Services & Research, identifies the PCOS complications that may be most responsible for psychiatric problems. While weight gain and unwanted body hair can be distressing, irregular menstrual cycles is the symptom of PCOS most strongly associated with psychiatric problems, the study found.


Most women with PCOS have some degree of insulin resistance, weight gain, and abnormal blood lipid levels. However, insulin resistance tends to be even more pronounced in women who are obese and do not ovulate. These conditions put those with PCOS at a higher risk of developing type 2 diabetes, high blood pressure (hypertension), and cardiovascular disease.
The principle infertility treatment initially includes preconception guidelines and the use of drugs to induce mono- or bifollicular ovulation. Other therapeutic modalities may also be employed, such as exogenous gonadotropins or laparoscopic ovarian drilling, which are considered to be second-line treatments, or in vitro fertilization (IVF), which is a third-line treatment 9. Thus, the choice of the most appropriate treatment depends on the patient's age, presence of other factors associated with infertility, experience and duration of previous treatments and the level of anxiety of the couple.

We conducted two searches. The first was sensitive and aimed to capture all pre-clinical studies explaining the reproductive endocrine effects of whole herbal extracts in PCOS or associated oligo/amenorrhoea and hyperandrogenism. The second search was specific and sought only clinical studies investigating herbal medicines revealed during the first search (for which a mechanism of effect had been demonstrated). We additionally searched, on a case by case basis for pre-clinical evidence for herbal medicines identified during the second search, but not included in the results of the first search. Clinical studies were excluded based on the absence of evidence for a mechanism of effect for the whole herbal extract in reproductive endocrinology associated with PCOS, oligo/amenorrhoea and hyperandrogenism. We used this approach to improve transparency and to limit confirmation bias for herbal medicines favoured by the authors in clinical practice.

Mammalian ovary development undergoes important changes during the perinatal period, moment when follicles are assembled and start to develop in a process not well known, involving endocrine and paracrine factors. In order to investigate the effect of two different hormonal environments on the early development of the ovary, we used an autologous transplant model in which Syrian hamster fetal ovaries were grafted under the kidney capsule of males hosts previously unilaterally or bilaterally orchidectomized. After 35 days of graft, ovaries and kidney parenchyme of the host male did not present signs of rejection. Ovaries contained primordial, primary follicles, secondary follicles and few tertiary follicles with morphological features similar to ovaries of control females of 35 days of age. Healthy primary and secondary follicles of experimental groups had frequency distribution and size similar to control ovaries but tertiary follicles were scarce in control as well as in grafts where they were mainly atretic. PCNA, marker of proliferation, was immuno detected in granulosa cells of growing follicles and the marker of apoptosis, Caspase 3 active, was evident mainly in secondary follicles. Immunoreactivity for steroidogenic proteins, StAR, 3-βHSD and aromatase detected in the follicular wall cells and the decreased serum levels of FSH without important changes in testosterone in bilateral orchidectomized males that received ovarian graft, and testosterone decreased without changes in FSH levels in unilateral orchidectomized males (UO) with ovarian graft, all together suggest the effect of steroid hormones produced by the ovary. In conclusion, the experimental model of autologous transplant presents evidence of early ovary development under the kidney capsule and its functional integration to the endocrine axis of the host male.
Herbal medicines are complex interventions with the potential for synergistic and antagonistic interactions between compounds [25]. Effects within the body may also exhibit complexity by simultaneous interactions with various body systems, both biochemically and by altering organ function [26]. The focus of this review was studies investigating whole herbal medicine extracts with direct effects on reproductive endocrinology for the treatment of women with irregular menstruation, hyperandrogenism and PCOS. The rationale for using this methodology was to identify herbal medicines with current scientific evidence explaining specific reproductive endocrinological effects in PCOS, oligo/amenorrhoea and hyperandrogenism, to develop understanding for the direct effects of herbal medicines on reproductive endocrinology and to highlight herbal medicines for which there was current scientific evidence supporting herbal medicine selection. The purpose of this review is to inform clinical decisions in integrative settings and meet clinicians and consumers preferences for pragmatic herbal management within an holistic, individualised treatment frame [27, 28].
In addition to improving reproductive and metabolic factors, the reduction in body weight may be associated with reduced incidence of complications during pregnancy and the neonatal period. In this context, lifestyle change should be the first choice for weight loss because medications to reduce weight could have side effects and bariatric surgery may be associated with preterm and small for gestational age births 14.
In PCOS, both ovaries tend to be enlarged, as much as three times their normal size. Eggs that do not mature fully are not released during ovulation and the immature eggs remain in the ovary as pearl-sized, fluid filled sacs. Over the course of time, many cysts may develop into what looks like a string of beads when viewed through ultrasound imaging. In as many as 90% of women with PCOS, an ultrasound of the ovaries will reveal cysts.

Saw Palmetto (Serenoa repens, Sabal serrulata) This herb has anti estrogenic effects and also has been found to decrease the testosterone levels in the blood. Both effects are very positive for women with PCOS .9 The herb has properties that can block the process of testosterone turning into DHT (dihydrotestosterone, a by-product of testosterone) which in turn lowers male hormones in the body.
If he was using heroin very heavily, that may well be his starting methadone dose (or close to it.) Methadone has a long half life while heroin has a short one, so avoiding withdrawals can take a lot of methadone, initially. Methadone clinics aren't going to leave someone on a high dose though. Women with PCOS have a normal uterus and healthy eggs. Many women with PCOS have trouble getting pregnant, but some women have no trouble at all. Polycystic ovary syndrome is the most common endocrinopathy among reproductive-aged women in the United States, affecting approximately 7% of female patients. Although the pathophysiology of the syndrome is complex and there is no single defect from which it is known to result, it is hypothesized that insulin resistance is a key factor. Metabolic syndrome is twice as common in patients with polycystic ovary syndrome compared with the general population, and patients with polycystic ovary syndrome are four times more likely than the general population to develop type 2 diabetes mellitus. Patient presentation is variable, ranging from asymptomatic to having multiple gynecologic, dermatologic, or metabolic manifestations. Guidelines from the Endocrine Society recommend using the Rotterdam criteria for diagnosis, which mandate the presence of two of the following three findings—hyperandrogenism, ovulatory dysfunction, and polycystic ovaries—plus the exclusion of other diagnoses that could result in hyperandrogenism or ovulatory dysfunction. It is reasonable to delay evaluation for polycystic ovary syndrome in adolescent patients until two years after menarche. For this age group, it is also recommended that all three Rotterdam criteria be met before the diagnosis is made. Patients who have marked virilization or rapid onset of symptoms require immediate evaluation for a potential androgen-secreting tumor. Treatment of polycystic ovary syndrome is individualized based on the patient's presentation and desire for pregnancy. For patients who are overweight, weight loss is recommended. Clomiphene and letrozole are first-line medications for infertility. Metformin is the first-line medication for metabolic manifestations, such as hyperglycemia. Hormonal contraceptives are first-line therapy for irregular menses and dermatologic manifestations.
For women who don’t receive timely, appropriate care for PCOS in early adolescence, the development of symptoms such as facial-hair growth can become more challenging to treat. Brandy Cramer, 33, a program officer at The Cameron Foundation, from Midlothian, Virginia says her doctors told her she just wasn’t trying hard enough to lose weight and dismissed her when she requested they run blood tests or suggest alternatives to the birth-control pills that gave her intense migraines. Cramer grew facial hair and has only been able to remove 50 percent of it, even after expensive laser hair-removal treatment.

Treatments for hirsutism in women with PCOS are similar to those in women without PCOS, such as patients with idiopathic hirsutism. There are many nonpharmacologic treatment options, including electrolysis, waxing, bleaching, plucking, depilatory creams (a form of hair removal that dissolves the hair), thermolysis (use of heat), and laser therapy. Several medications have been studied for the treatment of hirsutism in women with PCOS. First-line agents include spironolactone (Aldactone)22,23,28–30 and metformin,13,16,20,22,31–33 as well as eflornithine (Vaniqa) for facial hirsutism.9
PCOS is a heterogeneous disorder of uncertain cause.[23][24] There is some evidence that it is a genetic disease. Such evidence includes the familial clustering of cases, greater concordance in monozygotic compared with dizygotic twins and heritability of endocrine and metabolic features of PCOS.[7][23][24] There is some evidence that exposure to higher than typical levels of androgens in utero increases the risk of developing PCOS in later life.[25]
