### Other names for this syndrome include polycystic ovarian syndrome, polycystic ovary disease, functional ovarian hyperandrogenism, ovarian hyperthecosis, sclerocystic ovary syndrome, and Stein–Leventhal syndrome. The eponymous last option is the original name; it is now used, if at all, only for the subset of women with all the symptoms of amenorrhea with infertility, hirsutism, and enlarged polycystic ovaries.[52]

Clinical trials have shown that metformin can effectively reduce androgen levels, improve insulin sensitivity, and facilitate weight loss in patients with PCOS as early as adolescence. [55, 56, 57, 58] One study concluded that the use of metformin throughout pregnancy was associated with a 9-fold decrease in gestational diabetes in women with PCOS. [59] In addition to having the potential to reduce gestational diabetes in pregnant women with PCOS, metformin may also reduce the risk of preeclampsia in this population. [60]
Hirsutism is treated with a combination of approaches, including oral contraceptives with or without an antiandrogen, such as spironolactone, to lower levels or block actions of androgen on hair follicles. Oral contraceptives are often combined with antiandrogens to improve their clinical effect and to prevent pregnancy, since accidental exposure of the male fetus to antiantrogens can harm fetal development. A topical cream (eflornithine hydrochloride) also can be applied to treat facial hirsutism.

The goal of further evaluation of suspected PCOS is twofold: to exclude other treatable conditions that can mimic PCOS and to detect and treat long-term metabolic complications. Anovulation is common after menarche, so it is reasonable to delay workup for PCOS in adolescents until they have been oligomenorrheic for at least two years.28 If an adolescent is evaluated for PCOS, it has been suggested that she meet all three of the Rotterdam criteria before being diagnosed with the condition28  (Table 119).
Paeonia lactiflora combined with Cinnamomum cassia in a preparation called Unkei-to was investigated in an in-vitro study for ovarian production of 17-beta-oestradiol and progesterone, [42] (Table 1). Granulosa cells obtained from women undergoing IVF were examined for steroid hormone concentration following incubation with different doses over 48 hours. Oestradiol was significantly increased (p < 0.01) following exposure to doses of 0.3 ug/ml of Unkei-to. Supporting clinical evidence was found in one clinical trial of 157 infertile women aged 17–29 years, including a subgroup of 42 women with hyper-functioning (PCOS) oligo/amenorrhoea. Treatment with Unkei-to, 7.5 grams per day for eight weeks, demonstrated significant reductions of mean LH in the PCOS sub-group of 49.7% (±15.3). Ovulation was confirmed in 30 out of 42 oligo/amenorrheic women [57] (Table 1). Limitations however include findings based on sub-group comparisons without description of subgroup baseline characteristics (other than oligomenorrhoea). Although the same aqueous extract intervention was investigated in pre-clinical and clinical studies, it contained additional herbal extracts and it was irrational to attribute hormonal effects to Paeonia lactiflora and Cinnamomum cassia.

Human data regarding metformin improvement in IR in PCOS women shows mixed results and is complicated by varying methods of assessing IR. Short term (3 mo) treatment with metformin (1500 mg per day) failed to affect IR as measured by AUC-Insulin after 75-g OGTT. Metformin (1600 mg per day) in obese PCOS women treated for 6 mo failed to reduce IR as measured by QUICKI[66]. This is in contrast to similar length studies on obese PCOS women who demonstrated decreased IR as measured by HOMA-IR, QUICKI and ISI, and correlated with alterations in phosphoproteins related to IR[67]. Longer term metformin therapy (2 years, 1600 mg per day) in young, obese PCOS women reduced fasting insulin, hyperandrogenism and produced borderline reductions in HOMA-IR (P = 0.05)[68]. Metformin was compared prospectively to naltrexone and prenisolone in combination with oral contraceptive pills (OCPS). IR was unchanged despite lowered androgen levels[69]. Metformin has been compared to orlistat and pioglitazone over a 4 mo treatment course and although each treatment reduced IR as measured by HOMA-IR, metformin (1500 mg per day) had the least reduction (< 20%)[70].

I was diagnosed with pcos while using implanon birth control in November 2012. I was told it was che...mical, it was symptom and blood diagnosed not with cysts on my ovaries. My ob/gyn told me that implanon can cause pcos and many other terrible medical conditions. I had it removed in January 2013 and have been trying to get pregnant since, after being told I needed a hysterectomy. (I was 24). In the past year I have gotten pregnant twice, both ended with miscarriage. In January this year, I had my blood tested again and I am almost completely normal. I'll always have pcos, it's life long, but the fact that after a year of getting the implanon removed I was able to get my blood back to normal. My endocrinologist called it miraculous!

Polycystic ovary syndrome (PCOS) is a complex condition that is most often diagnosed by the presence of two of the three following criteria: hyperandrogenism, ovulatory dysfunction, and polycystic ovaries. Because these findings may have multiple causes other than PCOS, a careful, targeted history and physical examination are required to ensure appropriate diagnosis and treatment. This article provides an algorithmic approach to the care of patients with suspected or known PCOS.
A randomized study suggested that combined metformin/letrozole and bilateral ovarian drilling are similarly effective as second-line treatment in infertile women with clomiphene citrate–resistant PCOS. [52] In this study, 146 patients were given metformin and letrozole, and 73 underwent bilateral ovarian drilling. There was significant reduction in testosterone, fasting insulin, and ratio of fasting glucose to fasting insulin in the metformin/letrozole group. There was significant reduction in follicle-stimulating hormone (FSH), luteinizing hormone (LH), and ratio of LH to FSH in the bilateral drilling group. There was no significant difference between the patients in the 2 groups regarding cycle regularity, ovulation, pregnancy rate, and abortion rate. [52]

For acne or excess hair growth, a water pill (diuretic) called spironolactone (Aldactone) may be prescribed to help reverse these problems. Eflornithine (Vaniqa) is a prescription cream that can be used to slow facial hair growth in women.Electrolysis and over-the-counter depilatory creams are other options for controlling excess hair growth.

Cushing's syndrome, sometimes referred to as hypercortisolism, is a hormonal disorder caused by prolonged exposure to high levels of the hormone cortisol. Symptoms may include obesity, thinning arms and legs, a rounded face, and increased fat around the neck. Some causes of Cushing's syndrome is from taking glucocorticoid hormones such as prednisone for inflammatory diseases. Treatment for Cushing's syndrome depends on the cause.
When a woman is not menstruating or ovulating, an insufficient amount of the hormone progesterone is produced. This hormonal imbalance can lead to an overgrowth of the lining of the uterus (endometrial hyperplasia) and can increase a woman's risk of developing endometrial cancer. Women with PCOS who do ovulate and become pregnant tend to have an increased risk of complications such as miscarriage.
A surgery called ovarian drilling might make your ovaries work better when ovulation medications don't, but it's being done less often than it used to. The doctor makes a small cut in your belly and uses a tool called a laparoscope with a needle to poke your ovary and wreck a small part of it. The procedure changes your hormone levels and may make it easier for you to ovulate.

Approximately 15% of women with PCOS do not respond to the maximum dose of CC and are considered resistant to this medication. Due to the anti-estrogenic effect of this drug, endometrial proliferation may be inappropriate, which decreases the chance of embryo implantation. Moreover, this effect can also change the cervical mucus characteristics with a consequent reduction in sperm penetration 17,23. If the patient does not ovulate after the use of CC, gonadotropins for timed intercourse or ovarian drilling are the next steps to manage anovulatory infertile women with PCOS 9.

## While PCOS is not curable, symptoms are treatable with medications and changes in diet and exercise. Hormonal imbalances can be treated with birth control pills, androgen blocking medications, or medications that help the body use insulin better. Medications that help the body respond better to insulin may also be helpful. For women whose infertility problems are not resolved with lifestyle changes alone, medications that improve ovulation (fertility drugs) may be helpful.

The PCOS diagnosis is generally made through clinical signs and symptoms. The doctor will want to exclude other illnesses that have similar features, such as low thyroid hormone blood levels (hypothyroidism) or elevated levels of a milk-producing hormone (prolactin). Also, tumors of the ovary or adrenal glands can produce elevated male hormone (androgen) blood levels that cause acne or excess hair growth, thus mimicking the symptoms of PCOS.

Other tests that may be helpful but are not necessary for diagnosis include measurement of LH and follicle-stimulating hormone (FSH) levels to determine a serum ratio of LH/FSH.
