A team approach involving care by primary care and subspecialist physicians can be helpful to address the multiple manifestations of the syndrome. Goals for treatment (e.g., treating infertility; regulating menses for endometrial protection; controlling hyperandrogenic features, including hirsutism and acne) must account for the patient's preferences because therapy selection may otherwise conflict with outcomes that the patient considers important. Metabolic complications should be addressed in every patient via a blood pressure evaluation, a lipid panel, and a two-hour oral glucose tolerance test. Patients who are overweight should be evaluated for signs and symptoms of obstructive sleep apnea. All patients should be screened for depression (Figure 119).
Fasting methods to measure IR have been advocated for many years as an adjunct to DM screening. Elevated fasting insulin levels greater than 20 μU/mL may alone indicate IR. Fasting glucose/insulin ratio (G/I) has also gained some clinical traction. A ratio < 4.5 has in general been shown to be > 90% sensitive in some populations[45] but has never been validated with clamp studies[48]. Some ethnic variation in G/I cutoff ratios may exist[49]. There has been some suggestion that G/I < 7 in very young girls may predict IR[50,51].

Many women with PCOS exhibit symptoms of insulin resistance and metabolic syndrome, risk factors that raise the risk for heart disease and other problems like diabetes and stroke. Low magnesium levels are often associated with diabetes, and some research indicates that a dietary supplement of the mineral may improve insulin sensitivity, a factor in the development of type 2 diabetes and PCOS. One study found that overweight, insulin-resistant subjects who received 300 mg of magnesium at bedtime showed a significant improvement in fasting blood glucose and insulin levels, compared to subjects who received a placebo.

Most women with PCOS have some degree of insulin resistance, weight gain, and abnormal blood lipid levels. However, insulin resistance tends to be even more pronounced in women who are obese and do not ovulate. These conditions put those with PCOS at a higher risk of developing type 2 diabetes, high blood pressure (hypertension), and cardiovascular disease.
 “We were surprised to find that menstrual abnormalities in women with PCOS was the strongest predictor for mental health issues, particularly when there are so many other symptoms—like beard growth and infertility—that can make a woman feel unfeminine,” says senior author Nancy Reame, the Mary Dickey Lindsay Professor of Disease Prevention and Health Promotion at Columbia Nursing. “The study findings suggest that we can’t treat PCOS effectively unless we pay close attention to any signs of mental distress.”
Vitamin D deficiency may play some role in the development of the metabolic syndrome, so treatment of any such deficiency is indicated.[74][75] However, a systematic review of 2015 found no evidence that vitamin D supplementation reduced or mitigated metabolic and hormonal dysregulations in PCOS.[76] As of 2012, interventions using dietary supplements to correct metabolic deficiencies in people with PCOS had been tested in small, uncontrolled and nonrandomized clinical trials; the resulting data is insufficient to recommend their use.[77]

Christina Holaday - I am so excited about the changes I have already had. I started taking the Insulite supplements as soon as they arrived. Why treat IR in PCOS women? For many years only PCOS women with DM were treated. As the link between IGT and CAD became more apparent, many PCOS women with IGT were treated. We now understand that IR is often the first step in a progression to DM and CAD. Those who now advocate treatment for IR do so for the following reasons: reduction of insulin and androgen levels, prevention of IGT and DM, potential for improved ovulation, symptomatic improvement, prevention of MS[63]. Ultimately, secondary prevention in young women with identifiable and treatment precursor conditions is far more desirable and easier than treatment of these same women later in life with serious disease.
The ultimate goal is to prevent metabolic disease. Metformin (1500 mg per day) compared to placebo in a prospective 12 wk randomized control trial decreased arterial stiffness (by peripheral pressure waveforms in the brachial artery) and endothelial function (measured by augmentation index). Metformin did not reduce HOMA-IR[82]. The study population was obese but young (mean age 30 years), demonstrating the ability to reduce CAD risk even in very young women. Metformin has reduced both carotid intimal media thickness and endothelin levels in obese PCOS women[83]. In many studies metformin has reduced both total cholesterol and LDL cholesterol levels[84-86], triglyceride levels[84] and increased HDL levels[87,88]. Animal studies have shown that acarbose given to insulin resistant rats decreased carotid intimal hyperplasia and blood flow velocities[89]. Taken as a whole, the ability of metformin (and likely other insulin sensitizing agents) to elicit an overall reduction in the risk for CAD may be easier than the ability to produce consistent measureable improvements.

Three RCTs corroborate the positive fertility effects for Cimicifuga racemosa in women with PCOS, used in conjunction and when compared with the pharmaceutical Clomiphene citrate (clomiphene), [65, 68, 71] (Table 2). Results were reported for 441 women and show improved pregnancy rates when Cimicifuga racemosa was added to clomiphene during one menstrual cycle. In a study including women with PCOS (n = 147), pregnancy rates for the group receiving combined therapy (clomiphene 150 mg plus Cimicifuga racemosa 20 mg per day (Klimadynon®)) were 43.3% compared to 20.3% for women receiving only clomiphene [65] (Table 2). In another study using similar methodology (n = 100) pregnancy rates were 34.8% for the group treated with Cimicifuga racemosa plus clomiphene compared to 17.2% for women treated with clomiphene alone [68] (Table 2). Another study included women with PCOS and infertility (n = 100) compared Cimicifuga racemosa (Klimadynon®) and clomiphene over three months for hormone concentrations and pregnancy rates. Pregnancy rates were higher in the women in taking Cimicifuga racemosa compared to clomiphene, 14% and 8% respectively; however differences were not statistically significant. This study found significant effects for lowered luteinising hormone for women with PCOS receiving Cimicifuga racemosa compared to clomiphene (p = 0.007) [67]. Findings from clinical studies concur with laboratory and animal studies; however potential risks for bias include performance and collection bias due to lack of blinding (Table 2).
Following the electronic and manual searches of bibliographies, forty six clinical studies were identified for inclusion/exclusion assessment (Figure 1). A pre-requisite for the inclusion of clinical studies was identified laboratory evidence explaining the mechanism of effect in reproductive endocrinology. Fifteen met the inclusion criteria [54–68]. Eight were randomised controlled trials (RCTs) including 762 women [61–68] (Table 2). Thirty one studies were excluded for the following reasons; investigation of isolated herbal chemicals (n = 3); inclusion of male subjects (n = 4); no pre-clinical evidence (n = 11) and conditions different to those specified (n = 13).
This month, a groundbreaking study was published in the journal Nature Medicine that might have found a treatment for this disorder. The research was lead by Dr. Paolo Giacobini at the French National Institute of Health and Medical Research. The authors’ goal was to determine if something in the environment of the womb could be causing PCOS. We have known for awhile that PCOS runs in families (so if your mother or sister has PCOS, you may be more likely to have PCOS because you share genes that are associated with the disorder) but there don’t seem to be enough carriers of these genes to explain the high prevalence of PCOS (again, 10% of women!).

This review synthesises the evidence for mechanisms of effect for herbal medicine in oligo/amenorrhoea, hyperandrogenism and PCOS. Laboratory, animal and clinical studies demonstrate that the herbal medicines Vitex agnus-castus, Cimicifuga racemosa and Tribulus terrestris initiate endocrine effects in the pituitary as measured by lowered prolactin and LH and raised FSH. Four herbal medicines, Tribulus terrestris, Glycyrrhiza spp., (alone and in combination with Paeonia lactiflora), Paeonia lactiflora (in combination with Cinnamomum cassia) and Cinnamomum cassia demonstrated morphological changes in polycystic ovaries and steroidogenesis, including reduced ovarian volume and cysts, lowered androgens, improved insulin sensitivity and increased oestradiol.
A woman should shed this lining at least four times a year, says Dunaif. One option: taking progesterone (often called a “progesterone challenge”). But the resulting bleed can be heavy for some women. Another: Take birth control pills to get the regular cycle back online. These have the added benefit of lowering male hormones, too. What’s more, there are noted benefits of taking hormonal birth control that are true across the board, like getting regular, predictable periods and having up to a 20 and 50 percent lower risk of colon (colorectal) cancer and ovarian cancer, respectively. Women with PCOS have been shown to have higher levels of advanced glycation end products (AGEs) in their blood. AGEs are compounds formed when glucose binds with proteins, and are believed to contribute to certain degenerative diseases and aging. One small study found that cutting down on dietary AGEs significantly reduced insulin levels in women with PCOS. Foods high in AGEs include animal-derived foods and processed foods. Applying high heat (grilling, searing, roasting) increases levels.
Second-line therapy, when clomiphene citrate fails to lead to pregnancy, is either exogenous gonadotropins or laparoscopic ovarian surgery. [2, 3] If gonadotropins are used, a low-dose regimen is recommended, [3] and patients must be monitored with ultrasonography and laboratory studies. [2] Note that gonadotropin therapy is expensive and is associated with an increased risk of multiple pregnancy and ovarian hyperstimulation syndrome. [2]

Your doctor or nurse will look at your skin and measure your weight and blood pressure. They’ll ask questions about your period, any symptoms you may be having, and your personal and family health history. They may do a pelvic exam and blood tests to check your hormone levels, whether you may be pregnant, and more. In some cases, your doctor or nurse may recommend getting an ultrasound to check for ovarian cysts.

I am physically dependent but I also function–I can work, I can manage my life. The thing that upsets me is that these days, I would never have been given benzos as the first med for my illness, I would have been given something that's less physically addictive and potentially dangerous. In the long term I believe benzos aren't good for you at all, and I'm certainly long term.

Oligomenorrhoea was defined as menstrual cycle length that extended beyond 35 days (day one being the first day of menses). Amenorrhoea was defined as no menstrual period for three to six months or more [19]. This review was focussed on hypothalamic, pituitary and ovarian causes of menstrual irregularity with associated elevated gonadotropins including LH and prolactin and arrested folliculogenesis typically observed in polycystic ovaries. Hyperprolactinaemia is usually considered a unique cause for oligo/amenorrhoea; however in the present case it was included due to the potential co-existence for elevated prolactin, LH and PCOS, [32, 35].

The routine use of OGTT is advocated by some in all PCOS women[15]. In teenagers, abnormalities in glucose metabolism manifest prior to dyslipidemia, suggesting that assessment of glucose metabolism is even more important in younger women[16]. DM is diagnosed by an 8 h fasting plasma glucose ≥ 126 mg/dL, 2 h glucose value ≥ 200 mg/dL after oral glucose tolerance test (OGTT) or random glucose ≥ 200 mg/dL with symptoms of DM confirmed by either fasting plasma glucose or OGTT. Hemoglobin AIC > 6.5% may also be issued to diagnose DM[17]. Impaired glucose tolerance (IGT) is defined by a 2 h cutoff of 140-200 mg/dL on OGTT[18].The prevalence of IGT in obese adolescents is surprisingly as high as 15%[19].
Blood tests then can be performed to determine whether the ovaries are functioning normally or producing excess amounts of androgen. An ovarian ultrasound also can be done to measure the size of the ovaries and determine whether they have a polycystic appearance. At the same time, other conditions resembling PCOS, including pituitary, thyroid and adrenal abnormalities and other rare tumors, can be identified. Fasting blood levels of glucose, triglyceride and cholesterol also can be measured, along with another glucose determination obtained 2 hours after ingesting a glucose solution (2-hour oral glucose tolerance test), to predict the risk for developing diabetes and cardiovascular disease.

Once a diagnosis of PCOS is confirmed, it is imperative to assess women for CAD risk factors. Despite the many reasons women seek medical care for PCOS, the greatest long term risk for these women is CAD. This is generally not viewed or even recognized as a concern by women seeking care in the first place. The link between PCOS and CAD is multi-faceted. C-reactive protein (CRP) is higher in age matched PCOS women and is linked to BMI[10] with some ethnic variation in this risk[11]. The prevalence of MS in PCOS women is as high as 40% with increased prevalence of hypertension, dyslipidemia and abnormal glucose metabolism, all before age 30[12]. PCOS women aged 20-40 already demonstrate poor vascular function measured by brachial artery vascular flow[13]. No single blood test can predict or quantify this CAD risk. Although no standard recommendation for assessment of CAD risk factors exists, measurement of glucose metabolism, blood pressure screening, lipid screening and carotid intimal media thickness measurements have been suggested[14].

Teens with PCOS may have slightly different signs and symptoms. Irregular cycles are common in normal girls in the first years after periods start (menarche). Therefore, signs of ovulation problems for girls include irregular periods more than 2 years after menarche, absence of cycles for more than 3 months, or never having a first period by the time puberty is completed. Also, heavy or overly frequent periods may be a sign of problems related to PCOS. Adolescents do not require an ultrasound for the evaluation of PCOS, since large ovaries with many follicles develop as part of normal puberty.

It's good to know I'm not alone with having pcos. I found out when I was 18 years old. When I was gr...owing up and in puberty I never had my period. I remember girls around me complaining about there's in high school. But mine never came. I went to a obgyn when I was 16 she gave me meds to get it started, it only worked when I took the med every 3 months. They also did an ultrasound and found multiple cysts on my ovaries, but she did not know what it was. So I never went back to that doctor she was not very bright. But when I was 18 I was reading a magazine about a girl that has pcos and the symptoms and everything, and it sounded like me. So i went to My mom's obgyn and talked to him about what I read and my history of not getting my period on my own, he did an exam, ran some blood tests, and did other ultrasound. Come to find out I have pcos and was put on birth control and metformin. Which I'm still taking and it's been 11 years since I found out I have pcos. See More
PCOS is associated with multiple metabolic defects, including metabolic syndrome. Twice as many women with PCOS have metabolic syndrome as in the general population, and about one-half of women with PCOS are obese.1,9 The presence of PCOS is also associated with a fourfold increase in the risk of type 2 diabetes mellitus.10 There is an increased prevalence of nonalcoholic fatty liver disease,11,12 sleep apnea,13 and dyslipidemia14 in patients with PCOS, even when controlled for body mass index. Rates of cardiovascular disease are higher in patients with PCOS, but increased cardiovascular mortality has not been consistently demonstrated.15,16 Finally, there is evidence to suggest an increased risk of mood disorders among patients with PCOS.17,18
The diagnostic workup should begin with a thorough history and physical examination. Clinicians should focus on the patient's menstrual history, any fluctuations in the patient's weight and their impact on PCOS symptoms, and cutaneous findings (e.g., terminal hair, acne, alopecia, acanthosis nigricans, skin tags).19 Patients should also be asked about factors related to common comorbidities of PCOS.
Vasoactive intestinal polypeptide (VIP) stimulates estradiol and progesterone release from ovarian granulosa cells in vitro. Very little information is available as to the role VIP plays in the control of steroid secretion during reproductive cyclicity and in ovarian pathologies involving altered steroid secretion. In this study, we determined the involvement of VIP in regulating ovarian androgen and estradiol release during estrous cyclicity and estradiol valerate (EV)-induced polycystic ovarian development in rats. Our findings show that androgen and estradiol release from ovaries obtained during different stages of rat estrous cycle mimic cyclic changes in steroid release observed in vivo with maximal release occurring during late proestrus. VIP increased androgen release from ovaries of all cycle stages except late proestrus and estradiol release from all cycle stages. Increases in VIP-induced androgen and estradiol release were maximal at early proestrus. Inclusion of saturating concentrations of androstenedione increased magnitude of VIP-induced estradiol release at diestrus and estrus but not proestrus. Magnitude of VIP-induced androgen and estradiol release tended to be greater in the ovaries from EV-treated rats with polycystic ovary compared with estrous controls. At the tissue level, ovarian VIP concentration was cycle stage dependent with highest level seen in diestrus. Maximum concentration of VIP was found in EV-treated rats. Changes in VIP were inversely related to changes in ovarian nerve growth factor, a neuropeptide involved in ovarian androgen secretion. These results strongly suggest that intraovarian VIP participates in the control of estradiol secretion during the rat estrous cycle and possibly in the maintenance of increased ovarian estradiol secretory activity of EV-treated rats.

The ultimate goal is to prevent metabolic disease. Metformin (1500 mg per day) compared to placebo in a prospective 12 wk randomized control trial decreased arterial stiffness (by peripheral pressure waveforms in the brachial artery) and endothelial function (measured by augmentation index). Metformin did not reduce HOMA-IR[82]. The study population was obese but young (mean age 30 years), demonstrating the ability to reduce CAD risk even in very young women. Metformin has reduced both carotid intimal media thickness and endothelin levels in obese PCOS women[83]. In many studies metformin has reduced both total cholesterol and LDL cholesterol levels[84-86], triglyceride levels[84] and increased HDL levels[87,88]. Animal studies have shown that acarbose given to insulin resistant rats decreased carotid intimal hyperplasia and blood flow velocities[89]. Taken as a whole, the ability of metformin (and likely other insulin sensitizing agents) to elicit an overall reduction in the risk for CAD may be easier than the ability to produce consistent measureable improvements.

After 35 days of graft, ovaries and kidney parenchyme of the host male did not present signs of rejection. Ovaries contained primordial, primary follicles, secondary follicles and few tertiary follicles with morphological features similar to ovaries of control females of 35 days of age. Healthy primary and secondary follicles of experimental groups had frequency distribution and size similar to control ovaries but tertiary follicles were scarce in control as well as in grafts where they were mainly atretic. PCNA, marker of proliferation, was immuno detected in granulosa cells of growing follicles and the marker of apoptosis, Caspase 3 active, was evident mainly in secondary follicles. Immunoreactivity for steroidogenic proteins, StAR, 3-βHSD and aromatase detected in the follicular wall cells and the decreased serum levels of FSH without important changes in testosterone in bilateral orchidectomized males that received ovarian graft, and testosterone decreased without changes in FSH levels in unilateral orchidectomized males (UO) with ovarian graft, all together suggest the effect of steroid hormones produced by the ovary. In conclusion, the experimental model of autologous transplant presents evidence of early ovary development under the kidney capsule and its functional integration to the endocrine axis of the host male.

The quality of evidence, as determined by the volume of pre-clinical studies and the methodological quality of clinical trials, was highest for the herbal medicines Vitex agnus-castus, Cimicifuga racemosa and Cinnamomum cassia, for which there were laboratory and/or animal studies demonstrating endocrine mechanisms of action consistent with clinical outcomes shown in RCT’s with low risks for bias.
