Obese women with PCOS may have an increased risk of congenital anomalies (heart and neural tube defects), gestational diabetes mellitus [odds ratio (OR) 2.94; 95% confidence interval (CI): 1.70-5.08], hypertensive disorders during pregnancy (OR 3.67; 95% CI: 1.98-6.81) [mainly preeclampsia (OR 3.47; 95% CI: 1.95-6.17)], miscarriages, preterm births (OR 1.75; 95% CI: 1.16-2.62), the need for intensive unit care (OR 2.31; 95% CI: 1.25-4.26), increased perinatal mortality (OR 3.07; 95% CI: 1.03-9.21) 11,12 and Caesarean delivery (OR 1.74; 95% CI: 1.38-2.11) 12. The risk for preterm births and preeclampsia appears to be associated with maternal hyperandrogenism 13.
Metformin has been the mainstay of treatment for IR and IGT in PCOS women over the past decade. Metformin is a biguanide that acts principally on the liver to inhibit hepatic gluconeogenesis. It also inhibits acetyl-CoA carboxylase activity and suppresses fatty acid production. Metformin acts on skeletal muscle to inhibit lipid production and acts peripherally on adipose tissue to stimulate glucose transport and uptake. Metformin reduces insulin levels and promotes improved insulin receptor activity[64]. Metformin may also have direct and indirect effects on the ovary with respect to insulin action and steroidogenic enzymatic activity. In the endothelium, metformin seems to improve nitric oxide vasodilatory effects. Many other mechanisms of action have been studied in both animal and human models but consistent effects are not always demonstrated with local tissue concentrations that result from therapeutic doses[65].
Most women with PCOS have some degree of insulin resistance, weight gain, and abnormal blood lipid levels. However, insulin resistance tends to be even more pronounced in women who are obese and do not ovulate. These conditions put those with PCOS at a higher risk of developing type 2 diabetes, high blood pressure (hypertension), and cardiovascular disease.
Following the electronic and manual searches of bibliographies, forty six clinical studies were identified for inclusion/exclusion assessment (Figure¬†1). A pre-requisite for the inclusion of clinical studies was identified laboratory evidence explaining the mechanism of effect in reproductive endocrinology. Fifteen met the inclusion criteria [54‚Äď68]. Eight were randomised controlled trials (RCTs) including 762 women [61‚Äď68] (Table¬†2). Thirty one studies were excluded for the following reasons; investigation of isolated herbal chemicals (n = 3); inclusion of male subjects (n = 4); no pre-clinical evidence (n = 11) and conditions different to those specified (n = 13).

A long-term study suggested that metformin continued to improve the metabolic profile of women with PCOS over a 36-month treatment course, particularly improving circulating high-density lipoprotein cholesterol (HDL-C), diastolic blood pressure, and body mass index (BMI). [61] However, data are insufficient as yet to recommend metformin to all women with PCOS.

Selection of herbal medicines for the management of PCOS often includes the combined prescription of Glycyrrhiza spp. and Paeonia lactiflora[72‚Äď75]. We found preliminary evidence for this combination for hyperandrogenism only, and the evidence was more robust for Glycyrrhiza spp. alone than when combined with Paeonia lactiflora. Comparatively, our findings for the combination of Peaonia lactiflora and Cinnamomum cassia demonstrated no change in androgen concentration, suggesting that the anti-androgen activity in the Glycyrrhiza spp. and Paeonia lactiflora combination more likely attributable to Glycyrrhiza spp. However our findings may be complicated by the aqueous extraction methods used in the Paeonia lactiflora and Cinnamomum cassia combination and the preclinical studies into the Glycorrhizza spp and Paeonia lactiflora combination. More research into the anti-androgen effects of the combination Glycyrrhiza spp. and Paeonia lactiflora is needed to clarify the anti-androgen mechanism particularly if this herbal combination remains cornerstone herbal management for hyperandrogenism.

Herbal extract Phyto-Hypophyson¬ģ by Steril-Pharma GmbH Herrsching, Germany; contains Vitex agnus-castus plus Chelledonium majus and Silybum marianum (St Mary‚Äôs thistle) in homeopathic form. Additional herbal extracts have reported activity in hepatic function. There are no reports for direct reproductive effects. 150 drops per day (7.5¬†ml per day). Primary outcome for participants with amenorrhoea: at least one spontaneous menses. Oligomenorrhoeic subgroup - clinical outcomes were significantly improved in the treatment arm at 82% compared to 45% in placebo arm P = 0.021. When the amenorrheic group were included in analysis, differences were not significant p = 0.19. Diagnosis for anovulatory amenorrhoea is not well described. Non-statistically significant take home baby rates were complicated by insufficient sample size. 366 patients are required to have a 95% chance, as significant at the 5% level, an increase in take home baby rates from 6% in the placebo group to 18% in the experimental group. The authors conclude that this preparation may be useful if given 3‚Äď6 months, yet they only tested for 3¬†months.

According to Dr. Geoffrey Redmond, an endocrinologist specializing in female hormones, ‚ÄúJust because the ovaries are not functioning as much doesn‚Äôt mean the other abnormalities won‚Äôt still be present.‚ÄĚ He goes on to point out that studies show male hormone levels climb fairly sharply with age.2 This could mean a worsening of symptoms such as excess hair growth as those hormones become more active. Anecdotally, oral contraceptives are among the most common agents used to treat menstrual irregularities in women with PCOS. Antiandrogens, such as spironolactone, are effective for hirsutism. [79] Spironolactone (50-100 mg twice daily) is an effective primary therapy for hirsutism. Because of the potential teratogenic effects of spironolactone, patients require an effective form of contraception (eg, an oral contraceptive). Adverse effects of spironolactone include gastrointestinal discomfort and irregular menstrual bleeding, which can be managed by adding an oral contraceptive.

Oral contraceptives (birth control pills) contain a combination of hormones (estrogen and progesterone). Used properly, oral contraceptives can ensure that women with PCOS menstruate every four weeks. This, in turn, lowers the risk of endometrial cancer, which is higher in women with PCOS because they do not menstruate regularly and properly shed the endometrial lining.
