I smoke indoors and as long as you keep the window open and vacuum slightly more often there's no noticeable grime. You should clean your walls with cold water once a year or so to stop build-up of random crap regardless of whether you smoke. Tuna's grime has built up over years of neglect, smoking probably hasn't helped but it's mainly because she's nasty.

The pituitary gland is responsible for the release of luteinising hormone which can reduce the level of estrogen and androgen levels while raising progesterone levels.

Shahin [68] Non-blinded randomised controlled trial. Women with PCOS and infertility, n = 194. All participants received pharmaceutical ovulation induction (Clomiphene citrate 150 mg on days 3–7 of cycle); trigger injection (HCG 10000 IU Pregnyl), timed intercourse and progesterone support (oral micronized progesterone). A randomly selected group additional took Cimicifuga racemosa 120 mg per day (Klimadynon®) Primary outcomes pregnancy rates. Secondary outcomes: Pregnancy rates were 33 out of 192 cycles (17.2%) for the clomiphene alone group and 71 out of 204 cycles (34.8%) for the clomiphene plus Cimicifuga racemosa group. Non-blinding compromised the internal validity of the findings in this study. Confounding variables include variations in participant’s and clinicians attitudes and may have led to differences which were unaccounted for between the two groups. However the outcomes are objective with a statistically powered sample size.
Mid luteal (day 20) serum progesterone concentration before and after; treatment arm 2.46 (±0.70) to 9.69 (±6.34), p < 0.001. Placebo 1.99 (±0.65) to 2.34 (±0.59) p = 0.08. No description of the distribution of drop-outs or missing data. This suggests the potential imbalance between intervention and control and a possible over-exaggeration for treatment effect.

Studies have attempted for years to show an advantage to metformin for ovulation induction and as an adjunct to more advanced fertility treatments. In ovulatory PCOS women metformin was associated with improved serum and follicular fluid AMH levels as well as insulin values; these changes were not seen in anovulatory PCOS women[71]. Despite the demonstration of negative effects of IR on reproductive outcome, the vast majority of evidence does not show improvement in live birth rates when metformin is used strictly for fertility[72], although treatment does improve ovulatory status[72,73].
Ethanol extracts 1. One study examined the oestrogenic effects of Tribulus terrestris on uterine and vaginal tissue of ovariectomised rats [51]. 1. Healthy women n = 8 early menstrual cycle (follicular phase) Pre and post serum hormone concentration for FSH, LH testosterone and oestradiol at 8 am and 12 pm. Intervention consisted of Tribulus Terrestris 250 mg per day over five days. Results showed significant increase in FSH and rise in LH (not significant), an increase in oestradiol and no change in testosterone concentration [56] 1. Ovulation induction in polycystic ovaries [46, 47].

Serum (blood) levels of androgens (hormones associated with male development), including androstenedione and testosterone may be elevated.[17] Dehydroepiandrosterone sulfate levels above 700–800 µg/dL are highly suggestive of adrenal dysfunction because DHEA-S is made exclusively by the adrenal glands.[61][62] The free testosterone level is thought to be the best measure,[62][63] with ~60% of PCOS patients demonstrating supranormal levels.[20] The Free androgen index (FAI) of the ratio of testosterone to sex hormone-binding globulin (SHBG) is high[17][62] and is meant to be a predictor of free testosterone, but is a poor parameter for this and is no better than testosterone alone as a marker for PCOS,[64] possibly because FAI is correlated with the degree of obesity.[65]

PCOS is a multifaceted syndrome that affects multiple organ systems with significant metabolic and reproductive manifestations. Treatment should be individualized based on the patient's presentation and desire for pregnancy (Figure 219,29–35). Devices and medications used to treat manifestations of PCOS, and their associated adverse effects, are described in Table 2.19,29–33,36
Some other blood tests are suggestive but not diagnostic. The ratio of LH (Luteinizing hormone) to FSH (Follicle-stimulating hormone), when measured in international units, is elevated in women with PCOS. Common cut-offs to designate abnormally high LH/FSH ratios are 2:1[66] or 3:1[62] as tested on Day 3 of the menstrual cycle. The pattern is not very sensitive; a ratio of 2:1 or higher was present in less than 50% of women with PCOS in one study.[66] There are often low levels of sex hormone-binding globulin,[62] in particular among obese or overweight women.[citation needed]

Endometrial thickness in the clomiphene alone group was 8.5 mm (±1.9) compared to 12.9 (±2.3) in the clomiphene plus Cimicifuga racemosa group (p < 0.001). The miscarriage rate per pregnancy for the clomiphene alone group was 5 out of 33 (15.2%) and 6 out of 71 (8.5%) in the clomiphene plus Cimicifuga racemosa group.
Side effects: There is no serious side effect associated with Gymnema as long as it is only used for less than about 2 years. This is not a long-term herbal solution to PCOS. It can affect blood sugar so people with diabetes should monitor their levels carefully when taking Gymnema. This herb is also not recommended for breastfeeding or pregnant women even though no complications have been documented.

Women who do not wish to become pregnant can be effectively treated for hirsutism with oral contraceptives. [77] Oral contraceptives slow hair growth in 60-100% of women with hyperandrogenemia. Therapy can be started with a preparation that has a low dose of estrogen and a nonandrogenic progestin. Preparations that have norgestrel and levonorgestrel should be avoided because of their androgenic activity. There is also a risk of thrombotic events in obese women who use oral contraceptives; therefore, the proper precautions should be exercised to prevent such events. Oral contraceptives containing cyproterone acetate are also very effective in the treatment of more severe hirsutism [78] ; however, this combination of agents has not been approved by the FDA for use in the United States.

In patients with polycystic ovarian syndrome (PCOS) who are obese, endocrine-metabolic parameters markedly improve after 4-12 weeks of dietary restriction. Their sex hormone–binding globulin (SHBG) levels rise, and free testosterone levels fall by 2-fold. [66] Serum insulin and insulin-like growth factor-1 (IGF-1) levels also decrease. In patients with PCOS who are obese, weight loss is associated with a reduction of hirsutism and a return of ovulatory cycles in 30% of women, thereby improving pregnancy rates, as well as improving glucose tolerance and lipid levels. [12, 3]
We undertook two searches of the scientific literature. The first search sought pre-clinical studies which explained the reproductive endocrine effects of whole herbal extracts in oligo/amenorrhoea, hyperandrogenism and PCOS. Herbal medicines from the first search informed key words for the second search. The second search sought clinical studies, which corroborated laboratory findings. Subjects included women with PCOS, menstrual irregularities and hyperandrogenism.

No evidence that therapeutic agents additional to V. agnus-castus in Mastodynon® affect prolactin concentration. Four withdrew for unknown reasons. All pregnant women were withdrawn from the study. 4 women had miscarriages, all in the active arm. After 2 years there were 21 more pregnancies with 2 miscarriages – evenly spread over active and placebo groups. Women with infertility were included in this study however data from women who conceived were excluded. This may have led to an underestimation of treatment effect (type 1 error).

A meta-analysis published online in June in the journal Human Reproductive Update demonstrated that when metformin is combined with lifestyle modifications such as diet and exercise, it has been shown to help women with PCOS lose more body fat, achieve lower blood sugar, and improve menstruation better than lifestyle modification alone.

Progestin therapy. Taking progestin for 10 to 14 days every one to two months can regulate your periods and protect against endometrial cancer. Progestin therapy doesn't improve androgen levels and won't prevent pregnancy. The progestin-only minipill or progestin-containing intrauterine device is a better choice if you also wish to avoid pregnancy.

Chicken and egg: was Luna an asshole who started drugs and became even more of an asshole, or did she start drugs that turned into an asshole? A lot of what's wrong with her has little to do with her addiction - e.g. her desperate attempts at focusing everything into a mad scramble for sympathy (some bum died, so she wrote a story that made it look as if he had been her closest friend so people tell her they're sorry etc.) derive from her being an asshole, not from her addiction. Anyone who consciously wants to become a junkie for an aesthetic is already way deeply fucked imho.
Preclinical and clinical studies provide preliminary evidence that six herbal medicines may have beneficial effects for women with oligo/amenorrhea, hyperandrogenism and PCOS. The quality of the evidence is variable and strongest for Vitex agnus-castus and Cimicifuga racemosa in the management of oligo/amenorrhea and infertility associated with PCOS; and Cinnamomum cassia for improving metabolic hormones in PCOS. Evidence for Tribulus terrestris, Glycyrrhiza spp. alone and in combination with Paeonia lactiflora and Paeonia lactiflora combined with Cinnamon cassia is promising but in an emergent phase. Further investigations into the mechanisms of effect for herbal extracts are needed to complete our understanding of the reproductive endocrinological effects for herbal medicine for these common conditions.

oh ok thanks I didn't see that. wow he got that fast! maybe the bronx is faster at processing them. and yeah you'd think so. but especially with public defenders, they get paid by NY state, not the defendant obvi. the more hours they spend on a case, the more they get paid. also, the courts go on vacation usually for summer and Christmas/New Year's. Lurch got arrested in May but wasn't seen til July, prob due to summer vacation(s). one of my cases was still going on during the winter and I went to court in November and then didn't have to go again until mid January due to the holidays. so for a dopehead, that's 3 whole months of getting high without worrying about having to stop anytime soon.
The advantages of CC use are low cost, oral administration, few side effects (flushing, headache, visual disturbances and abdominal discomfort), the induction of monofollicular development in most cases 16 and a low rate of multiple gestations (2 to 13%) 17. The initial dose is 50 mg/day for five days (starting between the second and fifth day of the menstrual cycle) and may be increased to 150 mg/day 17,18; however, doses greater than 100 mg/day usually do not offer additional benefits (may be useful in obese women) 18. The ovulation rate may reach 75 to 80% 19 with a conception rate of 22% per cycle 20 and a cumulative pregnancy rate between 60 and 70% in six cycles 9. There is no evidence that the administration of human chorionic gonadotropin (hCG) in the mid-cycle increases ovulation rates (OR 0.99; 95% CI: 0.36-2.77) or clinical pregnancy (OR 1.02; 95% CI: 0.56-1.89) 21,22. CC treatment should be limited to six ovulatory cycles and US monitoring is not mandatory (it is recommended only in the first ovulatory cycle to adjust the dose based on the ovarian follicular growth and development and for endometrial assessment) 17,18. Additional cycles of ovulation induction with CC (maximum of twelve cycles) may be individually evaluated based on the cost-effectiveness and age of women and after discussion with the couple 9. The incidence of ovarian hyperstimulation syndrome (OHSS; increased capillary permeability with consequent third-space fluid sequestration and hemoconcentration) associated with the use of CC is low, approximately 1 to 6% 17,23.
I reckon probably the latter though, people do actually want to help her, to the extent that people get pissed off as fuck if you express any of that sort of sentiment in here because we've heard it so often. There is growing evidence that mood disturbances, mostly severe depression, are common in PCOS women , in whom impaired quality of life from body image concerns cause fatigue, sleep disturbance and changes in eating habits. In addition, many PCOS patients report feeling abnormal, unfeminine, and embarrassed due to unwanted hair, often hiding their hair growth and covering their face when talking to others. Understanding how a woman feels about her body image and improving this perception are essential components of any management plan that provides overall health care to women with PCOS.
Fish oil has been associated with a long list of health benefits, and some research indicates that omega-3 supplements can decrease androgen levels in women with PCOS. One study found that women with PCOS who were given three grams of omega-3s a day for eight weeks had lower testosterone concentrations and were more likely to resume regular menses than subjects who received a placebo.

PCOS is due to a combination of genetic and environmental factors.[6][7][15] Risk factors include obesity, not enough physical exercise, and a family history of someone with the condition.[8] Diagnosis is based on two of the following three findings: no ovulation, high androgen levels, and ovarian cysts.[4] Cysts may be detectable by ultrasound.[9] Other conditions that produce similar symptoms include adrenal hyperplasia, hypothyroidism, and high blood levels of prolactin.[9]
