Androgens. All females make androgens (also referred to as “male hormones”), but there are often higher levels of androgens in women with PCOS. The excess androgens are produced mostly by the ovaries, but the adrenal glands can also be involved. Excess androgens are responsible for many PCOS symptoms including acne, unwanted hair, thinning hair, and irregular periods.
Other tests that may be helpful but are not necessary for diagnosis include measurement of LH and follicle-stimulating hormone (FSH) levels to determine a serum ratio of LH/FSH. A ratio greater than 2 generally indicates PCOS, but there are no exact cutoff values because many different assays are used.26 The FSH level is more helpful in ruling out ovarian failure.26
Saw Palmetto (Serenoa repens, Sabal serrulata) This herb has anti estrogenic effects and also has been found to decrease the testosterone levels in the blood. Both effects are very positive for women with PCOS .9 The herb has properties that can block the process of testosterone turning into DHT (dihydrotestosterone, a by-product of testosterone) which in turn lowers male hormones in the body.

Insulin-sensitizing agents, including metformin,11,32 rosiglitazone,19,20,32 and pioglitazone (Actos),17 have been effective in improving fertility and ovulation in women with PCOS. There are contradictions in the literature regarding whether metformin, clomiphene, or a combination of the two agents is superior for improving pregnancy rates in women with PCOS. A 2003 Cochrane review suggested that metformin should be a first-line treatment for infertility in women with PCOS.39 A more recent study confirmed that six months of metformin therapy was more effective than six months of clomiphene therapy for improving fertility in anovulatory, nonobese women with PCOS.8 However, a large randomized trial of more than 600 women found that clomiphene is superior to metformin in achieving live birth in infertile women with PCOS, with no statistical benefit to the addition of metformin to clomiphene.40 Another study also showed no benefit from adding metformin to clomiphene.35 However, two meta-analyses suggested that the combination is better than clomiphene alone.41,42 A more recent study found that, although ovulation rates were better with metformin than with clomiphene, pregnancy rates were similar.43 Finally, two systematic reviews found conflicting results; one suggests metformin does not affect ovulation or pregnancy rates,1 and the other suggests it does.44

Selection of herbal medicines for the management of PCOS often includes the combined prescription of Glycyrrhiza spp. and Paeonia lactiflora[72–75]. We found preliminary evidence for this combination for hyperandrogenism only, and the evidence was more robust for Glycyrrhiza spp. alone than when combined with Paeonia lactiflora. Comparatively, our findings for the combination of Peaonia lactiflora and Cinnamomum cassia demonstrated no change in androgen concentration, suggesting that the anti-androgen activity in the Glycyrrhiza spp. and Paeonia lactiflora combination more likely attributable to Glycyrrhiza spp. However our findings may be complicated by the aqueous extraction methods used in the Paeonia lactiflora and Cinnamomum cassia combination and the preclinical studies into the Glycorrhizza spp and Paeonia lactiflora combination. More research into the anti-androgen effects of the combination Glycyrrhiza spp. and Paeonia lactiflora is needed to clarify the anti-androgen mechanism particularly if this herbal combination remains cornerstone herbal management for hyperandrogenism.
The first step in diagnosing PCOS is to have a complete history and physical examination performed by a trained health care provider. A careful history and physical examination can detect whether androgen excess is causing male-pattern hair growth (hirsutism), acne or hair loss and whether ovulation is occurring normally. Physical examination also can detect high blood pressure and increased abdominal obesity as risk factors for diabetes and cardiovascular disease in individuals who are overweight.

Some other blood tests are suggestive but not diagnostic. The ratio of LH (Luteinizing hormone) to FSH (Follicle-stimulating hormone), when measured in international units, is elevated in women with PCOS. Common cut-offs to designate abnormally high LH/FSH ratios are 2:1[66] or 3:1[62] as tested on Day 3 of the menstrual cycle. The pattern is not very sensitive; a ratio of 2:1 or higher was present in less than 50% of women with PCOS in one study.[66] There are often low levels of sex hormone-binding globulin,[62] in particular among obese or overweight women.[citation needed]
With all of these options, it can’t be stressed enough that a woman will be making a choice about what’s most important to her in the moment. Ovarian drilling has some advantages compared with gonadotropin treatment because it is associated with a lower multiple gestation rate (OR 0.13; 95% CI: 0.03 to 0.52; p=0.004; I(2)=0%; 5 trials; n=166) 29 and does not require US monitoring of follicular development 9. However, the long-term impact of ovarian drilling on the ovarian reserve/ovarian function remains unknown 29.

It is thought that insulin resistance may be a key factor in PCOS. Insulin helps transport glucose into the body's cells. where it is needed for energy production. When there is resistance to insulin's effect, the body tries to compensate by making even more insulin and releasing it in the blood (hyperinsulinemia). Some believe that too much insulin may cause the ovaries to increase androgen production, which in turn causes the symptoms associated with PCOS.

A team approach involving care by primary care and subspecialist physicians can be helpful to address the multiple manifestations of the syndrome. Goals for treatment (e.g., treating infertility; regulating menses for endometrial protection; controlling hyperandrogenic features, including hirsutism and acne) must account for the patient's preferences because therapy selection may otherwise conflict with outcomes that the patient considers important. Metabolic complications should be addressed in every patient via a blood pressure evaluation, a lipid panel, and a two-hour oral glucose tolerance test. Patients who are overweight should be evaluated for signs and symptoms of obstructive sleep apnea. All patients should be screened for depression (Figure 119).

It can be difficult to become pregnant with PCOS because it causes irregular ovulation. Medications to induce fertility when trying to conceive include the ovulation inducer clomiphene or pulsatile leuprorelin. Metformin improves the efficacy of fertility treatment when used in combination with clomiphene.[85] Metformin is thought to be safe to use during pregnancy (pregnancy category B in the US).[86] A review in 2014 concluded that the use of metformin does not increase the risk of major birth defects in women treated with metformin during the first trimester.[87] Liraglutide may reduce weight and waist circumference more than other medications.[88]
