Inhibin A levels in PCOS women were not found to correlate with IR in PCOS women. Most of these serum markers share common limitations and have been poorly studied. How they might vary with different PCOS phenotypes is unknown. None are adequately compared to IR measured by clamp studies. Their usefulness serially in clinical practice to monitor patients over time and undergoing treatment is also unknown. Some genetic work has recently shown promise. Although far from clinical use, microarray analysis of genes in muscle, adipose tissue and the liver shows alterations in the setting of IR. Serum genetic markers may lead to future genetic techniques to detect and monitor IR.

The name ‘Polycystic Ovarian Syndrome’ points to the ovaries because it was long considered to be a reproductive issue. But it is now widely accepted that polycystic ovaries develop as a result of endocrine disorders characterized by a series of hormone imbalances: hyperandrogenism (specifically excess testosterone) and Insulin Resistance due to excess insulin that can trigger a cascade of other hormonal problems.4 From a systemic point of view, the continuing and/or increase of PCOS symptoms is likely due to a continuing hormonal imbalance.

Medications to induce fertility when trying to conceive include the ovulation inducer clomiphene or pulsatile leuprorelin. Oligomenorrhoea was defined as menstrual cycle length that extended beyond 35 days (day one being the first day of menses). Amenorrhoea was defined as no menstrual period for three to six months or more . This review was focussed on hypothalamic, pituitary and ovarian causes of menstrual irregularity with associated elevated gonadotropins including LH and prolactin and arrested folliculogenesis typically observed in polycystic ovaries. Hyperprolactinaemia is usually considered a unique cause for oligo/amenorrhoea; however in the present case it was included due to the potential co-existence for elevated prolactin, LH and PCOS, [32, 35].
Because of the menstrual and hormonal irregularities, infertility is common in women with PCOS. Because of the lack of ovulation, progesterone secretion in women with PCOS is diminished, leading to long-term unopposed estrogen stimulation of the uterine lining. This situation can lead to abnormal periods, breakthrough bleeding, or prolonged uterine bleeding. Unopposed estrogen stimulation of the uterus is also a risk factor for the development of endometrial hyperplasia and cancer of the endometrium (uterine lining). However, medications can be given to induce regular periods and reduce the estrogenic stimulation of the endometrium (see below).
Combination birth control pills: Oral contraceptives (OCs) containing estrogen plus progestin help you manage PCOS symptoms by normalizing your menstrual cycle. By reducing levels of male hormones, OCs can help control excess hair growth and acne, too. You’ll usually shed the lining of your uterus every 28 days while taking an oral contraceptive, which also reduces your risk for endometrial cancer. Oral contraceptive pills are sometimes prescribed for women before they begin fertility treatment because these female hormones reduce the “male” androgen levels. But it’s important to know that they won’t help restore ovulation.19
Many women with PCOS have decreased sensitivity to insulin, the hormone that regulates glucose (sugar) in the blood. This condition, known as insulin resistance, is a major risk factor for type 2 diabetes. Women with PCOS often have type 2 diabetes, which occurs more frequently in women with PCOS. Signs of insulin resistance include weight gain (especially around the waist), acanthosis nigricans (skin thickening around the neck, armpits, belly, button, and other creases), and skin tags.

Metformin (1500 mg per day) compared to placebo in a prospective 12 wk randomized control trial decreased arterial stiffness (by peripheral pressure waveforms in the brachial artery) and endothelial function (measured by augmentation index). Metformin did not reduce HOMA-IR. The study population was obese but young (mean age 30 years), demonstrating the ability to reduce CAD risk even in very young women. Metformin has reduced both carotid intimal media thickness and endothelin levels in obese PCOS women. In many studies metformin has reduced both total cholesterol and LDL cholesterol levels[84-86], triglyceride levels and increased HDL levels[87,88]. Animal studies have shown that acarbose given to insulin resistant rats decreased carotid intimal hyperplasia and blood flow velocities. Taken as a whole, the ability of metformin (and likely other insulin sensitizing agents) to elicit an overall reduction in the risk for CAD may be easier than the ability to produce consistent measureable improvements.
