Metformin has been the mainstay of treatment for IR and IGT in PCOS women over the past decade. Metformin is a biguanide that acts principally on the liver to inhibit hepatic gluconeogenesis. It also inhibits acetyl-CoA carboxylase activity and suppresses fatty acid production. Metformin acts on skeletal muscle to inhibit lipid production and acts peripherally on adipose tissue to stimulate glucose transport and uptake. Metformin reduces insulin levels and promotes improved insulin receptor activity[64]. Metformin may also have direct and indirect effects on the ovary with respect to insulin action and steroidogenic enzymatic activity. In the endothelium, metformin seems to improve nitric oxide vasodilatory effects. Many other mechanisms of action have been studied in both animal and human models but consistent effects are not always demonstrated with local tissue concentrations that result from therapeutic doses[65].
PCOS treatment is different for different people. While there is no cure for PCOS, taking medicine and losing weight can help  your symptoms. In practice, CC treatment can initiate the menstrual cycle as early as the second day. Classically, this drug treatment has been initiated between the third and fifth day of the menstrual cycle and maintained for 5 days. Ovulation typically occurs seven days after the last CC tablet is taken. Seven days after the probable date of ovulation, follicular rupture can be confirmed by progesterone levels greater than 3 ng/dL (evaluated only at the beginning of the treatment to verify the response to CC when US is unavailable) and pregnancy can be confirmed by measuring the blood beta fraction of human chorionic gonadotropin (βhCG) 7 days after the progesterone measurement. Anecdotally, oral contraceptives are among the most common agents used to treat menstrual irregularities in women with PCOS. My brother was a Heroin addict who started taking Benzos between his shooting up, like Luna, versus the other way around. He was an addict, he took benzos to keep up whatever feeling he was chasing. Thats inherently different than an anxiety or depression patient taking them for relief of symptoms. Im sure it does happen, there's always that risk but I wouldn't go out there and say that Benzo use is always a gateway.

I remember girls around me complaining about there's in high school. But mine never came. They also did an ultrasound and found multiple cysts on my ovaries, but she did not know what it was. So I never went back to that doctor she was not very bright. But when I was 18 I was reading a magazine about a girl that has pcos and the symptoms and everything, and it sounded like me. So i went to My mom's obgyn and talked to him about what I read and my history of not getting my period on my own, he did an exam, ran some blood tests, and did other ultrasound. Some have tried to utilize ultrasound to detect IR. Of note, normoglycemic women often have the phenotypic criteria for polycystic ovaries on ultrasound[54], consistent with other data in young adolescents showing that polycystic ovaries by ultrasound appearance often does not correlate with either anovulatory menstrual cycles or metabolic abnormalities[55]. Therefore ultrasound is too non-specific to use with any reliability in measuring IR.


In vitro fertilization represents the third-line treatment for infertility in women with PCOS 9. However, if the initial assessment demonstrates a bilateral tubal occlusion and/or concentration of recovered motile sperm less than or equal to 5 million, this treatment becomes the first option along with lifestyle changes. The risk of OHSS is the main complication of the highly complexity treatment in women with PCOS. Thus, to minimize this side effect, ovarian stimulation should be initiated with low doses of gonadotropins (100 to 150 IU of FSHr) and the pituitary should be suppressed with a gonadotropin-releasing hormone (GnRH) antagonist because this method is associated with a reduced risk of OHSS compared with an agonist (29 randomized control trials (RCTs); OR 0.43; 95% CI: 0.33 to 0.57) 32. If the patient presents with clinical and ultrasound signs of OHSS, final oocyte maturation should be performed with a GnRH agonist and embryos should be frozen and transferred in a subsequent cycle 33,34. Infertile women with PCOS may present with better general oocyte and embryo quality rates; however, the clinical pregnancy and live birth rates are similar to those observed in normo-ovulatory women without PCOS 35.

A secondary analysis of two randomized, double blind, placebo-controlled trials that included 182 children of mothers with PCOS reported that children exposed to metformin had higher BMI and increased prevalence of overweight/obesity at 4 years of age. The study found that at 4 years of age, the metformin group had higher weight z-score than the placebo group; difference in means 0.38 (0.07 to 0.69), p=0.017, and higher BMI z-score; difference in means 0.45 (0.11 to 0.78), p=0.010. There were also more overweight/obese children in the metformin group; 26 (32%) than in the placebo group; 14 (18%) at 4 years of age; odds ratio (95% CI): 2.17 (1.04 to 4.61), p=0.038. More studies are needed to examine this association. [94]

On February 28, 2012, the FDA approved important safety label changes for the class of cholesterol-lowering drugs known as statins, including removal of routine monitoring of liver enzymes. Information about the potential for generally nonserious and reversible cognitive side effects and reports of increased blood glucose and glycosylated hemoglobin (HbA1c) levels was added to the statin labels. In addition, extensive contraindication and dose-limitation updates were added to the lovastatin label in situations when this drug is taken with certain medications that can increase the risk for myopathy. [63]

Given the conditions associated with PCOS, the Endocrine Society, the Androgen Excess and PCOS Society, and the American College of Obstetricians and Gynecologists recommend that clinicians evaluate patients' blood pressure at every visit and lipid levels at the time of diagnosis, and screen for type 2 diabetes with a two-hour oral glucose tolerance test regardless of a patient's body mass index. Patients should have repeat diabetes screening every three to five years, or more often if other indications for screening are present.19–21 The Endocrine Society further recommends depression screening, as well as screening for symptoms of obstructive sleep apnea in overweight and obese patients with PCOS.19 However, routine screening for nonalcoholic fatty liver disease or endometrial cancer (using ultrasonography) is not recommended.19
Human data regarding metformin improvement in IR in PCOS women shows mixed results and is complicated by varying methods of assessing IR. Short term (3 mo) treatment with metformin (1500 mg per day) failed to affect IR as measured by AUC-Insulin after 75-g OGTT. Metformin (1600 mg per day) in obese PCOS women treated for 6 mo failed to reduce IR as measured by QUICKI[66]. This is in contrast to similar length studies on obese PCOS women who demonstrated decreased IR as measured by HOMA-IR, QUICKI and ISI, and correlated with alterations in phosphoproteins related to IR[67]. Longer term metformin therapy (2 years, 1600 mg per day) in young, obese PCOS women reduced fasting insulin, hyperandrogenism and produced borderline reductions in HOMA-IR (P = 0.05)[68]. Metformin was compared prospectively to naltrexone and prenisolone in combination with oral contraceptive pills (OCPS). IR was unchanged despite lowered androgen levels[69]. Metformin has been compared to orlistat and pioglitazone over a 4 mo treatment course and although each treatment reduced IR as measured by HOMA-IR, metformin (1500 mg per day) had the least reduction (< 20%)[70].

When a healthy diet and regular exercise aren't enough, medications can make losing weight easier. Different drugs work in different ways. For example, orlistat (Alli, Xenical) stops your body from digesting some of the fat in your food, so it may also improve your cholesterol levels. Lorcaserin (Belviq) makes you feel less hungry. Your doctor will prescribe the medication they think will be the most successful for you.
The advantages of CC use are low cost, oral administration, few side effects (flushing, headache, visual disturbances and abdominal discomfort), the induction of monofollicular development in most cases 16 and a low rate of multiple gestations (2 to 13%) 17. The initial dose is 50 mg/day for five days (starting between the second and fifth day of the menstrual cycle) and may be increased to 150 mg/day 17,18; however, doses greater than 100 mg/day usually do not offer additional benefits (may be useful in obese women) 18. The ovulation rate may reach 75 to 80% 19 with a conception rate of 22% per cycle 20 and a cumulative pregnancy rate between 60 and 70% in six cycles 9. There is no evidence that the administration of human chorionic gonadotropin (hCG) in the mid-cycle increases ovulation rates (OR 0.99; 95% CI: 0.36-2.77) or clinical pregnancy (OR 1.02; 95% CI: 0.56-1.89) 21,22. CC treatment should be limited to six ovulatory cycles and US monitoring is not mandatory (it is recommended only in the first ovulatory cycle to adjust the dose based on the ovarian follicular growth and development and for endometrial assessment) 17,18. Additional cycles of ovulation induction with CC (maximum of twelve cycles) may be individually evaluated based on the cost-effectiveness and age of women and after discussion with the couple 9. The incidence of ovarian hyperstimulation syndrome (OHSS; increased capillary permeability with consequent third-space fluid sequestration and hemoconcentration) associated with the use of CC is low, approximately 1 to 6% 17,23.

Although these agents have shown significant efficacy in clinical trials (for oral contraceptives, in 60–100% of individuals[74]), the reduction in hair growth may not be enough to eliminate the social embarrassment of hirsutism, or the inconvenience of plucking or shaving. Individuals vary in their response to different therapies. It is usually worth trying other medications if one does not work, but medications do not work well for all individuals.
