Obesity that occurs with PCOS needs to be treated because it can cause numerous additional medical problems. The management of obesity in PCOS is similar to the management of obesity in general. Weight loss can help reduce or prevent many of the complications associated with PCOS, including type 2 diabetes and heart disease. Consultation with a dietician on a frequent basis is helpful until just the right individualized program is established for each woman.

When the syndrome was first described in 1935 by American gynecologists Irving Stein, and Michael Leventhal, it was considered a rare disorder. Today as many as five million women in the United States may be affected, according to the Department of Health and Human Services, but researchers are still just beginning to uncover the disorder’s full impact.
Pioglitazone (Actos®) and Rosiglitazone (Avandia®) are insulin-sensitizing agents that improve glucose tolerance and insulin resistance. These drugs are approved by the Food and Drug Administration (FDA) for the treatment of diabetes. Although they are not approved for treatment of PCOS, they have been shown to be effective for this purpose in many studies.
Reproductive dysfunction in PCOS women may also be a manifestation of IR. Menstrual cycle irregularity has been correlated with HOMA-IR[37]. Molecular defects in insulin action may be responsible for reproductive difficulties in PCOS women. Although endometrial tissue appears morphologically similar in PCOS to controls and may have similar insulin receptor prevalence, insulin receptor action at the local endometrial level is impaired and may be reflected in lower pregnancy implantation rates[38]. HOMA-IR has been correlated with follicle count in PCOS during in vitro fertilization[39]. Follicular insulin levels correlate with pregnancy outcome after IVF[40]. These are areas of unresolved understanding with regard to PCOS. Proposed mechanisms for insulin reproductive abnormalities include abnormalities of ovarian steroidogenesis, excessive LH secretion and abnormalities in glucose uptake[41]. PCOS women have been found to have post-receptor insulin abnormalities as well as reduced peripheral insulin receptor binding[42].
Fasting insulin level or GTT with insulin levels (also called IGTT). Elevated insulin levels have been helpful to predict response to medication and may indicate women needing higher dosages of metformin or the use of a second medication to significantly lower insulin levels. Elevated blood sugar and insulin values do not predict who responds to an insulin-lowering medication, low-glycemic diet, and exercise. Many women with normal levels may benefit from combination therapy. A hypoglycemic response in which the two-hour insulin level is higher and the blood sugar lower than fasting is consistent with insulin resistance. A mathematical derivation known as the HOMAI, calculated from the fasting values in glucose and insulin concentrations, allows a direct and moderately accurate measure of insulin sensitivity (glucose-level x insulin-level/22.5).[citation needed]

This is an important distinction; given her longtime love for drugs, tendency to abuse everything she gets, and unwillingness to go sober even when she's not on H, there's a good chance that Luna has 'polysubstance dependence' and is chasing the feeling of being 'not sober' by abusing her meds. i would suggest researching a bit online. but from my experience working in treatment centers yes, meth has foul chemicals. when they first come in and are detoxing from it addicts smell like chemical and ammonia. there are so many foul and unnatural things in it it reeks havoc on the human body. i know she mentioned crack but i don't think we have any real proof luna has ever used meth.

Trigger injection (Human chorionic gonadotropin Pregnyl) and timed intercourse recommended when dominant follicle (>18 mm) was observed on ultrasound. Serum measurements during follicular phase for FSH, LH and FSH:LH ratio. Mid luteal progesterone. Ultrasound observation of endometrial thickness. Pregnancy rates including twin pregnancies. Adverse events including hyperstimulation. Positive outcomes for Cimicifuga racemosa compared to clomiphene for reduced day 2–5; LH (p = 0.007) and improved FSH to LH ratio (p = 0.06), mid luteal progesterone (p = 0.0001), endometrial thickness (p = 0.0004). Pregnancy rates were higher in the Cimicifuga racemosa group (7/50 compared to 4/50) but not statistically significant (p = 0.1). Adverse events (4 women) and twin pregnancy’s (two women) were not significantly different between groups. No detail for diagnostic criteria for PCOS. Confounding fertility factors not described. Drop-out reasons were not reported seven in Cimicifuga racemosa group and four in clomiphene group.
The cysts (fluid filled sacs) in the ovaries can be identified with imaging technology. (However, as noted above, women without PCOS can have many cysts as well.) Ultrasound , which passes sound waves through the body to create a picture of the kidneys, is used most often to look for cysts in the ovaries. Ultrasound imaging employs no injected dyes or radiation and is safe for all patients including pregnant women. It can also detect cysts in the kidneys of a fetus. Because women without PCOS can have ovarian cysts , and because ovarian cysts are not part of the definition of PCOS, ultrasound is not routinely ordered to diagnose PCOS. The diagnosis is usually a clinical one based on the patient's history, physical examination, and laboratory testing.
It's trendy to be ~body posi~ and have massive self confidence even if you're conventionally ugly right now. Especially if you're not conventionally attractive, actually. Any of the above symptoms and signs may be absent in PCOS, with the exception of irregular or no menstrual periods. All women with PCOS will have irregular or no menstrual periods. Women who have PCOS do not ovulate regularly; that is, they do not release an egg every month. This is why they do not have regular periods and typically have difficulty conceiving.
It can be difficult to become pregnant with PCOS because it causes irregular ovulation. Medications to induce fertility when trying to conceive include the ovulation inducer clomiphene or pulsatile leuprorelin. Metformin improves the efficacy of fertility treatment when used in combination with clomiphene.[85] Metformin is thought to be safe to use during pregnancy (pregnancy category B in the US).[86] A review in 2014 concluded that the use of metformin does not increase the risk of major birth defects in women treated with metformin during the first trimester.[87] Liraglutide may reduce weight and waist circumference more than other medications.[88]
Weight loss achieved through dietary changes and exercise can help women with PCOS in several ways. Like men and women without PCOS, losing weight reduces a person's risk of cardiovascular disease and non-insulin dependent (type 2) diabetes. Weight loss also helps to lower the level of insulin in the body which, in turn, reduces the ovaries' production of testosterone.
Combination birth control pills. Pills that contain estrogen and progestin decrease androgen production and regulate estrogen. Regulating your hormones can lower your risk of endometrial cancer and correct abnormal bleeding, excess hair growth and acne. Instead of pills, you might use a skin patch or vaginal ring that contains a combination of estrogen and progestin.
Pharmaceutical treatment for menstrual irregularity includes the oral contraceptive pill (OCP) and ovulation induction with clomiphene citrate (clomiphene) [20, 21] depending on fertility needs. Women with PCOS are however likely to exhibit contraindications for the OCP [3] and whilst induction of ovulation with clomiphene has demonstrated success, pregnancy rates remain inexplicably low [4]. Up to thirty 30% of women, particularly overweight women with PCOS, fail to respond to clomiphene therapy [4, 22, 23]. Management for hyperandrogenism includes anti-androgens and hypoglycaemic pharmaceuticals such as metformin [24]. Metfomin has demonstrated effectiveness for improving insulin sensitivity and hyperandrogenism, however use of metformin is associated with the high incidence of adverse effects including nausea, vomiting and gastro-intestinal disturbances [5].
Contrary to the implication of “polycystic,” some women with the condition don’t have any cysts. A diagnosis requires only two of the following three criteria to be met: elevated levels of male sex hormones (which can cause excess hair growth, acne, and baldness), irregular or absent periods, and/or at least 12 follicular cysts on one or both ovaries.

Although there isn’t a single cure for PCOS, lifestyle and nutritional changes can significantly improve the hormonal imbalances that are so prevalent in PCOS. Weight loss and exercise will reduce the levels of insulin and targeted nutritional supplements can help balance out the systemic biochemistry. A healthy lifestyle is the best medicine and through a comprehensive and consistent improvement in diet and activity, PCOS symptoms can be effectively managed or diminished, and some can be made to disappear completely.

Selection of herbal medicines for the management of PCOS often includes the combined prescription of Glycyrrhiza spp. and Paeonia lactiflora[72–75]. We found preliminary evidence for this combination for hyperandrogenism only, and the evidence was more robust for Glycyrrhiza spp. alone than when combined with Paeonia lactiflora. Comparatively, our findings for the combination of Peaonia lactiflora and Cinnamomum cassia demonstrated no change in androgen concentration, suggesting that the anti-androgen activity in the Glycyrrhiza spp. and Paeonia lactiflora combination more likely attributable to Glycyrrhiza spp. However our findings may be complicated by the aqueous extraction methods used in the Paeonia lactiflora and Cinnamomum cassia combination and the preclinical studies into the Glycorrhizza spp and Paeonia lactiflora combination. More research into the anti-androgen effects of the combination Glycyrrhiza spp. and Paeonia lactiflora is needed to clarify the anti-androgen mechanism particularly if this herbal combination remains cornerstone herbal management for hyperandrogenism.

As many as 70% of PCOS women are insulin resistant and 10% have DM[20-22]. In PCOS women with normal glucose metabolism initially, the rate of conversion to abnormal glucose metabolism can be 25% over just three years[23]. More alarming, insulin abnormalities are highly prevalent in adolescents with PCOS[24]. Almost 20% of young Thai women with PCOS actually have DM[25]. Overall, normal glucose levels on an OGTT do not predict IR and IR, despite normal glucose levels, is correlated with CRP, dyslipidemia and other CAD risk factors[26]. Therefore, glucose levels alone lack the sensitivity to predict metabolic risk in PCOS patients. Precursor states of insulin abnormalities likely predict long term CAD risk well before glucose abnormalities. IR can be just as severe in diabetics and non-diabetics[27], stressing the seriousness of this metabolic impairment as a precursor and not a separate disease. Animal models have shown that IR alone damages myocardial cells, providing direct evidence of end organ disease[28]. Human data link HOMA-IR to left ventricular dysfunction[29]. Abnormal glucose metabolism short of IGT and DM still deserves attention, identification and treatment[30].
Although these agents have shown significant efficacy in clinical trials (for oral contraceptives, in 60–100% of individuals[74]), the reduction in hair growth may not be enough to eliminate the social embarrassment of hirsutism, or the inconvenience of plucking or shaving. Individuals vary in their response to different therapies. It is usually worth trying other medications if one does not work, but medications do not work well for all individuals.
