Given the conditions associated with PCOS, the Endocrine Society, the Androgen Excess and PCOS Society, and the American College of Obstetricians and Gynecologists recommend that clinicians evaluate patients' blood pressure at every visit and lipid levels at the time of diagnosis, and screen for type 2 diabetes with a two-hour oral glucose tolerance test regardless of a patient's body mass index. Patients should have repeat diabetes screening every three to five years, or more often if other indications for screening are present.19–21 The Endocrine Society further recommends depression screening, as well as screening for symptoms of obstructive sleep apnea in overweight and obese patients with PCOS.19 However, routine screening for nonalcoholic fatty liver disease or endometrial cancer (using ultrasonography) is not recommended.19

Treatment of PCOS depends partially on the woman's stage of life. For younger women who desire birth control, the birth control pill, especially those with low androgenic (male hormone-like) side effects can cause regular periods and prevent the risk of uterine cancer. Another option is intermittent therapy with the hormone progesterone. Progesterone therapy will induce menstrual periods and reduce the risk of uterine cancer, but will not provide contraceptive protection.

Metformin is among one of the main treatments to target insulin resistance if you have prediabetes or diabetes, including because of PCOS. The advice is controversial, but some physicians believe that PCOS always requires metformin, notes Dr. Dunaif. “There’s no reason to give every woman with PCOS metformin. It’s a good and safe drug, but there’s no point in taking it if you don’t need it,” she says. (She notes that it has such a good safety profile that even when overprescribed, it doesn’t cause any harm.) But it’s not a good way to reduce the male hormone symptoms, like excess hair growth.
Various laparoscopic methods, including electrocautery, laser drilling, and multiple biopsy, have been used with the goal of creating focal areas of damage in the ovarian cortex and stroma. According to the Society of Obstetricians and Gynaecologists of Canada (SOGC), laparoscopic ovarian drilling may be considered in women with clomiphene-resistant PCOS, especially in the presence of other laparoscopic indications. [2] A small French study also suggested that surgical management via ovarian drilling with hydrolaparoscopy may be beneficial in cases of PCOS that are resistant to clomiphene citrate. [87]
Studies have attempted for years to show an advantage to metformin for ovulation induction and as an adjunct to more advanced fertility treatments. In ovulatory PCOS women metformin was associated with improved serum and follicular fluid AMH levels as well as insulin values; these changes were not seen in anovulatory PCOS women[71]. Despite the demonstration of negative effects of IR on reproductive outcome, the vast majority of evidence does not show improvement in live birth rates when metformin is used strictly for fertility[72], although treatment does improve ovulatory status[72,73].
Pharmaceutical treatment for menstrual irregularity includes the oral contraceptive pill (OCP) and ovulation induction with clomiphene citrate (clomiphene) [20, 21] depending on fertility needs. Women with PCOS are however likely to exhibit contraindications for the OCP [3] and whilst induction of ovulation with clomiphene has demonstrated success, pregnancy rates remain inexplicably low [4]. Up to thirty 30% of women, particularly overweight women with PCOS, fail to respond to clomiphene therapy [4, 22, 23]. Management for hyperandrogenism includes anti-androgens and hypoglycaemic pharmaceuticals such as metformin [24]. Metfomin has demonstrated effectiveness for improving insulin sensitivity and hyperandrogenism, however use of metformin is associated with the high incidence of adverse effects including nausea, vomiting and gastro-intestinal disturbances [5].

The principle infertility treatment includes lifestyle changes. The first-line drug treatment to induce ovulation consists of CC with timed intercourse. The second-line treatment consists of the exogenous administration of gonadotropins or laparoscopic ovarian surgery in cases where laparoscopy is indicated. The third-line treatment consists of IVF/ICSI, which is indicated when the previous interventions fail; this treatment can also be the first choice in cases of bilateral tubal occlusion or semen alterations that impair the occurrence of natural pregnancy. There is no evidence for the routine use of metformin in infertility treatment of anovulatory women with PCOS. Aromatase inhibitors are promising, and long-term studies are necessary to prove their safety.

Weight loss achieved through dietary changes and exercise can help women with PCOS in several ways. Like men and women without PCOS, losing weight reduces a person's risk of cardiovascular disease and non-insulin dependent (type 2) diabetes. Weight loss also helps to lower the level of insulin in the body which, in turn, reduces the ovaries' production of testosterone.

Metformin has been the mainstay of treatment for IR and IGT in PCOS women over the past decade. Metformin is a biguanide that acts principally on the liver to inhibit hepatic gluconeogenesis. It also inhibits acetyl-CoA carboxylase activity and suppresses fatty acid production. Metformin acts on skeletal muscle to inhibit lipid production and acts peripherally on adipose tissue to stimulate glucose transport and uptake. Metformin reduces insulin levels and promotes improved insulin receptor activity[64]. Metformin may also have direct and indirect effects on the ovary with respect to insulin action and steroidogenic enzymatic activity. In the endothelium, metformin seems to improve nitric oxide vasodilatory effects. Many other mechanisms of action have been studied in both animal and human models but consistent effects are not always demonstrated with local tissue concentrations that result from therapeutic doses[65].

Obesity that occurs with PCOS needs to be treated because it can cause numerous additional medical problems. The management of obesity in PCOS is similar to the management of obesity in general. Weight loss can help reduce or prevent many of the complications associated with PCOS, including type 2 diabetes and heart disease. Consultation with a dietician on a frequent basis is helpful until just the right individualized program is established for each woman.

Methadone is said to not get people "high," but I beg to differ. It doesn't give any of the euphoria that heroin does, nor the rush, but it definitely has a body high to it. I've never taken a dose of methadone over 30mg (which is a typical maintenance dose, btw) and got so high off that I couldn't walk straight. Mentally I was nearly clear though. Maybe Lurch likes feeling physically retarded and it takes him 130mg to get there? Who knows.

Non-blinding compromised the internal validity of the findings in this study. Confounding variables include variations in participant’s and clinicians attitudes and may have led to differences which were unaccounted for between the two groups. However the outcomes are objective with a statistically powered sample size.
For those women that after weight loss still are anovulatory or for anovulatory lean women, then the ovulation-inducing medications clomiphene citrate[74] and FSH are the principal treatments used to promote ovulation.[medical citation needed] Previously, the anti-diabetes medication metformin was recommended treatment for anovulation, but it appears less effective than clomiphene.[medical citation needed][90]
